A am suffering dry cough for the last 2 weeks and I have chest pain whenever I cough hard can you suggest what should be done?

MBBS
General Physician, Greater Noida
Hi lybrate-user. It is important to find out underlying cause. Causes of cough are many like. Cold, pneumonia, bronchitis, sinusitis. Whooping cough. Post nasal drip. Allergy. Asthma copd smoking. Laryngitis. Bronchiectasis. Aspirations. Medication like ace inhibitors for bp. Interstitial lung disease. Heart failure. Rarely lung cancer. Cough may be aggravated by breathing cold dry air, air pollution, inhalation of dust and smoke, excessive use of voice. To findout underlying cause sputum culture, ekg, lung function test, xray of chest and sinuses are done. Dry cough may be relieved by 1. Demulcents that treat dry cough by coating and soothing pharynx and relieve irritation that triggers dry cough. Cough syrup containing sucrose and glycerol. 2. Honey with warm water and lemon. 3. Steam inhalation. 4. Gargling with warm salty water. 5. Cough suppressant containing dextromethorpan or codeine or dihydrocodeine. 6. Sucking lozenges. For chest pain you can take pcm and warm compression at pain site.

My stomach is so big and my cholestrol is so high please tell me how to control ?

MBBS, MD - Internal Medicine
Internal Medicine Specialist, Faridabad
pl. do test lipid profile, take low fat diet, go for walk...Exercise can improve cholesterol. Moderate physical activity can help raise high-density lipoprotein (HDL) cholesterol, the "good" cholesterol. With your doctor's OK, work up to at least 30 minutes of exercise a day. Adding physical activity, even in 10-minute intervals several times a day, can help you begin to lose weight. Just be sure that you can keep up the changes you decide to make. Consider: Taking a brisk daily walk during your lunch hour Riding your bike to work Swimming laps Playing a favorite sport To stay motivated, find an exercise buddy or join an exercise group. And remember, any activity is helpful. Even taking the stairs instead of the elevator or doing a few situps while watching television can make a difference.

If someone feels pain in left arm regularly, is this symptom of heart related problem. When I do jogging for regular 5-7 days then problem goes.

PGD Maternal Child Health, MBBS
General Physician, Akola
May be, may be not. Depends upon the presentation. If the pain appears immediately after exertion, then it could be a sign of poor blood supply through coronaries when the heart needs more blood and oxygen. If there is family history of hypertension, diabetes, heart disease then one must evaluate itself by cardiac check up like ecg, blood pressure, regularity of pulses, 2 d echo etc. If everything comes to be normal, then you may rest assured.

I am 45 years old and having problem of high blood pressure what should I do to control it. Please help me.

BHMS
Homeopath,
Dear Mr. Lybrate-user. As you are suffering from high blood pressure you must be taking some medicines for your problem. If you are not taking any medicines I would advise you to walk for at least 1 hour daily. Abstain from high cholesterol rich foods like red meat, eggs. Don't over-exert yourself. Your high blood pressure will make you to sweat much than normal persons, so don't forget to drink plenty of water. Try to have at least 8 hours sleep. Last but surely not the least, try to avoid any type of mental stress.

My father had a CABG on Mar 28, 2016. After keeping him in CTVS followed by HDU for 4 days doctors shifted him to his general ward. Since Mar 31, 2016 he was complaining that he was not able to remember anything. He forgot names of every family member and friends except one or two people who are not extremely close. He even forgot his own name as well. Doctors found yesterday that he got a blood clot in his left tempro -parietal lobe of the brain which had caused a stroke. This was definitely post CABG because before am operation too he had a CT scan where no such clot was evident. Now the doctors have given him the following medicines: Atova 20mg, diplate A-120, cetil 500mg, paracetamol, ditallox 12.5mg, insulin 6 units in morning, 8 units in afternoon and 6 units in the night. My father already had a mild stroke before in August 2012; that time his homocysteine level in sera was found to be 114 for which the doctor gave folic acid tablets as medicine. Rest a Carotid Doppler found minor plaques. Rest all tests were good. Now in this condition I am almost clueless what to do next? Please help.

Diploma in Obstetrics & Gynaecology, MBBS
General Physician, Delhi
It is not even 1 month when your father had a cabg. He has a history of a mild stroke in past and causes of stroke and heart attack are same, so is the prevention after a warning in the form of anginal pain or a mini stroke, he is a diabetic too, controlled on insulin. Sometimes medicines belonging to opiate group are given post operatively for bone pain, this does affect the memory and it is a temporary change, doctors shift to other pain killers like it has been the case - he is being given paracetamol, statins to bring down bad cholesterol that causes plaque. Carotid doppler has shown minor plaques which is expected because he had blockages which caused heart attack and stroke. May be your doctors will give medicines to dissolve clots in the brain. Continue the treatment as suggested by your doctors, remember it takes minimum 2 months for innermost tissues to recover, outwardly the stitches may look clear and normal, you need to have patience and help him recover. Good luck.

Hi I am 39 year old and have triglyceride level at 359. Please suggest suitable medication.

MBBS, MD - Internal Medicine
Internal Medicine Specialist, Faridabad
Hello dear High cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Change life style .do meditation, yoga, Go walk daily, do any one physically activity daily i.e.play any game., Avoid fast foods, oily foods, junk foods, alcohol, smoking, fatty foods, non-veg.,avoid stress, take plenty of water in day time. And 1 glass of water before bed at night. use flax oil, omega-3 fatty meal or cap., take .green veg., whole grain. garlic,. Take anti cholesterol medicines .
