Few symptoms are as follows: I think she is into "Psychotic depression" as I read from one of the forums and especially if you read some of her recent signs what she is going through: 1. Give all money to beggars, kinners. And other old people who's asking for money 2. She thinks she is holding Shiva so she keeps yelling on family members all time 3. She thinks she bought our rental apartment, bought an Audi, and got 500+ crores with me 4. She listen to 3-4 particular songs and keep repeating them the whole day/ night 5. Doesn't want to sleep 6. 3-4 AM starts washing cloths 7. She doesn't care even our bedroom is messed up 8. Talks the whole day even with a strangers 9. Stops listening/ keeping her self away from all closed family or friends, and she gets super hyper if one of us try to say something 10. No fear of public and she doesn't feel embarrassed herself even walking with bare feet 11. Started putting everything on Facebook (in real life she doesn't that) 12. Dear s user, thank you for writing to lybrate. I understand your concern about treatment of your wife. Elevated mood, grandiosity (claiming power, wealth, supreme identity, knowledge), diminished need for sleep, excessive talking or pressured speech, racing thoughts or flight of ideas in mind, clear evidence of distractibility, increased level of goal-focused activity at home, excessive pleasurable activities or spending too much often with painful consequences all are symptoms of manic episode of bipolar mood disorder, severe with psychotic symptoms. I suggest you to consult a psychiatrist who has indoor treatment facility. She requires hospitalization and indoor management. When patients with severe mania with psychotic symptoms are brought to my hospital, I hospitalize them. I give them medications like lithium carbonate, olanzapine and lorazepam. With availability of latest brief pulse and ultra brief pulse ect devices, electroconvulsive therapy (ect) is a safe and effective biological treatment which improves mania rapidly. I also recommend ect for her. She will definitely improve with treatment. I recommend treatment earliest. Warm regards.

I am of age 36 male. Recently a significant post residual volume of urine (150 ml out of 380 ml) was found in scanning. My urologist examined and first suggested a tablet for 15 days. After that, he told to do cystoscopy, but then changed his mind, and observed my urine flow. He told cystoscopy is not necessary and inserted a cathetar tube and told there is no serious problem, as the tube was able to reach bladder without any pain. He told me to continue the tablet. But the problem remains unsolved. Kindly tell me what can possible step further?

Fellowship of the Royal College of Surgeons (FRCS), MS - Urology, MBBS
Urologist, Ahmedabad
Dear , it appears that you have obstruction in the flow of urine. Passage of catheter by your urologist rules out stricture of urethra. Most likely cause would be bladder neck obstruction. It is not a disease4 but a mechanical prolem. For this medication s are available like alpha blockers ; like temsulocin 0.4 mg at bed time. It would have been better if you had mentioned the name of the tablets you are taking. Ideally you should have uroflometry and cystourethroscopy to confirm the diagnosis and to see sevearity of obstruction. Though medicines help a lot ideal treatment would beendoscopic surgery. Start drinking a lot water for now. Dr. Kiram mehta.

Hyperhidrosis - Making Summer Miserable

DVDL, MBBS
Dermatologist, Delhi
Hyperhidrosis - Making Summer Miserable

Sweating is a normal function of the body, but some people sweat so much that their clothes are often drenched in sweat. Hyperhidrosis is a condition where the amount of sweat produced by the sweat glands cannot be regulated. It is characterized by excessive sweating commonly seen in the underarms, palms and soles. This disorder can affect a person at any age and if left untreated, can become a lifelong condition. Though this condition is not a serious threat to your health, it can be quite embarrassing and can hamper your lifestyle.

In many cases, there is no obvious trigger for Hyperhidrosis, and it is caused by the malfunctioning of the nervous system. This is known as primary Hyperhidrosis. Sweat is a function which helps the body cool itself. In cases of primary Hyperhidrosis, the brain sends a message to the sweat glands to produce sweat even though the body does not need to be cooled. A genetic mutation may also be the cause for some cases of Hyperhidrosis.

Hyperhidrosis with obvious trigger factors is known as secondary Hyperhidrosis. While primary Hyperhidrosis mostly affects the underarms, groin, palms and soles, secondary Hyperhidrosis can affect all parts of the body. Some of the common triggers for this condition are:

  1. Pregnancy
  2. Menopause
  3. Anxiety
  4. Low blood sugar
  5. Hyperthyroidism
  6. Medication side effects
  7. Alcohol or drug withdrawal
  8. Parkinson’s disease

Hyperhidrosis can be challenging to diagnose and treat. Lifestyle changes are the easiest way to address this disorder. Some changes that can help you fight Hyperhidrosis are:

  1. Avoid spicy food and alcohol
  2. Replace deodorants with antiperspirants
  3. Choose cotton fabrics over synthetic fabrics
  4. Wear loose fitting clothes.
  5. Try using armpit shields and wearing absorbent socks.

Anticholinergic or antimuscarinic medicines can also help treat Hyperhidrosis. This keeps the nervous system from activating the sweat glands. Dry mouth, blurred vision, constipation, abdominal cramps and difficulty in passing urine are some of the side effects associated with this medication. Iontophoresis is a more aggressive form of Hyperhidrosis treatment. This involves passing a weak electric current through a wet pad to the affected area to block the sweat glands. It is usually used to treat excessive sweating in the palms and soles.

Botox injections can also help treat Hyperhidrosis by blocking the brain signals to the sweat glands. However this is not a permanent cure and may need to be repeated after a year or so. Endoscopic thoracic sympathectomy or a surgery to remove the sweat glands may also be recommended in extreme cases of Hyperhidrosis. This is usually seen as a permanent solution for excessive sweating, but are relatively new procedures and hence not very common. If you wish to discuss about any specific problem, you can consult a dermatologist.
