My Brother in law got an accident fee day ago and his both legs were broken. And now bone marrow has moved to his chest doctor had said. Is that bone marrow in his chest dangerous? Can it be cured by medicine or there is another way?

MCh Ortho, ATLS (AIIMS), Diploma In Orthopaedics (D. Ortho), MBBS, MS - Orthopaedics
Orthopedist, Kolkata
What your doctor probably meant to say was that FAT from the bone marrow had moved into his lungs. This condition is known s fat embolism. It is a very serious condition and symptoms depend on the amount of fat blocking the arteries. Minimal amount of embolism will cause only mild respiratory distress while massive fat embolisms can be fatal. There is no medicines which can cure this condition. In most cases it can resolve spontaneously. Only supportive therapy like assisted ventilation, oxygenation, etc. Can help. Sometimes steroids are given also.

My wife 45 yrs has occasional pain in legs and soles. Uric acid 5. 9. Hb 11.Please do help

Hiee, pain in legs and soles can b radiated from back. The nerves from the legs arise from the back so if there is any prob in the back u may feel pain in the legs and soles of feet with or without back pain. Tell her to b active and go for walks and maintain the back straight while sitting and walkin. Follow diet for uric acid. Avoid dals and pulses. I think this should help

Foot Exercises: Contrast bath---It is a form of treatment where a limb or the entire body is immersed in warm water followed by the immediate immersion of the limb or body in ice water. This procedure is repeated several times, alternating hot and cold. Note that the treatment should always end in the ice water, as heat will induce the body's inflammatory response, while cold helps to decrease inflammation. • Point your toes • Raise your heels • Squeeze your toes • Roll a ball. Roll a golf ball or tennis ball under the ball of your foot. Apply light pressure for about two minutes. according to the APMA.

I got a ligament tear in my ankle around a year back. But it healed on its own. Now whenever I extend my ankle beyond a certain distance while playing it tears again. It had healed 3-4 times but it is not happening this time? What should I do?

Fellowship of the Royal College of Surgeons (FRCS), Membership of the Royal College of Surgeons (MRCS)
Orthopedist, Trichy
if the ligament injury had been a complete tear then it wouldnt heal spontaneously and that would give rise to recurrent instability symptoms. if you have pain and a sense of giving way in the ankle then you would need a proper orthopaedic examination. you may be advised a course of physio or surgery depending on the examination findings and imaging results

Hi I am suffering from knee problem since one months. So please suggest me. Thanks.

MSc in Orhopedic Physiotherapy (UK), BPTh/BPT
Physiotherapist, Bangalore
Hi I am suffering from knee problem since one months.
So please suggest me.
Thanks.
Knee pain is a common complaint that affects people of all ages. Knee pain may be the result of an injury or some other medical conditions including arthritis, patellar tendinitis, acl injury, knee bursitis, torn meniscus, gout and infections. Try some home remedies for your pain- - ice is useful for the control of pain and swelling. It can be applied to the knee for 15 to 20 minutes as often as every two to four hours. The swollen knee should be elevated above the level of the heart while icing. -activities that cause pain should be avoided temporarily. -if the knee is swollen or sore, the following positions and activities should be avoided until knee pain and swelling resolve: squatting kneeling twisting and pivoting jogging aerobics, dancing playing sport specific exercises will help you improve range of motion, strength, and endurance. It is especially important to focus on strengthening and stretching your quadriceps since these muscles are the main stabilizers of your kneecap. Core exercises may also be recommended to strengthen the muscles in your abdomen and lower back. Also maintaining a healthy body weight to avoid overstressing your knees are very important. -a full history of your problem should be taken and your movements assessed by a physiotherapist. Please consult a physiotherapist for the same. Best wishes.

Know More About Gout!

What is gout?

Gout is characterized by inflamed, painful joints due to the formation of crystal deposits at the joints. It also known as the disease of kings and the king of diseases because it was associated with overindulgence of rich foods and wine. In truth, anyone can get gout. Gout affects more men than women and is often associated with obesity, hypertension (high blood pressure), hyperlipidemia (high levels of lipids in the blood), and diabetes.

This condition is a form of inflammatory arthritis that results in painful attacks in the joints. It can cause swelling and redness, and in some cases, it can lead to lumpy deposits that can be seen under the skin. It can also lead to the development of kidney stones.

What causes gout?

Gout is caused by monosodium urate crystal deposits in the joints, due to an excess of uric acid in the body. The excess of uric acid may be caused by an increase in production by the body, underelimination of the uric acid by the kidneys, or increased intake of certain foods that metabolize into uric acid in the body. Foods that are high in purines (the component of the food that metabolizes into uric acid) include certain meats, such as game meats, kidney, brains, and liver; seafood, such as anchovies, herring, scallops, sardines, and mackerel; dried beans, and dried peas. Alcoholic beverages and sugary drinks high in fructose may also increase levels of uric acid in the body. Gout attacks may be triggered by any or all of the following:

  • Consumption of alcohol

  • Consumption of protein-rich foods

  • Fatigue

  • Emotional stress

  • Minor surgery

  • Illness

What are the symptoms of gout?

Anatomy of the foot

Gout is characterized by sudden, recurrent attacks that often occur without warning. Severe, chronic gout may lead to deformity. The following are the most common symptoms of gout. However each individual may experience symptoms differently. Symptoms may include:

  • Severe, sudden pain in one or more joints (most often the joint in the big toe)

  • Swollen joint(s)

  • Red or purplish, tight, shiny skin over joint

  • Warmth in joint area

  • Fever

  • Chills

  • General feeling of illness

  • Hard lumps of urate crystal deposits under the skin (called tophi)

The symptoms of gout may resemble other medical conditions or problems. Always consult your doctor for a diagnosis.

How is gout diagnosed?

In addition to a complete medical history and a physical examination, a diagnosis of gout may be confirmed with the examination of a fluid sample from the joint for the presence of urate crystals.

Treatment for gout

Specific treatment for gout will be determined by your doctor based on:

  • Your age, overall health, and medical history

  • Extent of the condition

  • Your tolerance for specific medications, procedures, and therapies

  • Expectation for the course of the condition

  • Your opinion or preference

Treatment may include:

  • Nonsteroidal anti-inflammatory medications to relieve pain and inflammation

  • Colchicine, an oral or intravenous medication to relieve pain and inflammation; may cause diarrhea and other side effects

  • Corticosteroids to reduce inflammation

  • Increasing fluid intake while avoiding alcoholic beverages

  • Reducing the intake of protein-rich foods

  • Reducing weight (if obesity is a factor)

  • Medication to lower the uric acid level in the blood

  • Medication to block production of uric acid in the body

  • Surgery to remove extremely large tophi

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You complaints could be due to a soft tissue affection of the structures surrounding the shoulder joint like tendons, bursa, capsule etc. If there is no h/o any injury to the shoulder joint or any s/s of inflammation like instability, swelling, skin redness etc, then you can initiate a short course of anti inflammatory medications and physical therapy, and we will observe your progress for next 2-3 weeks, if you do not feel better, the we might get few investigations like mri of the affected shoulder region done. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.
