I have a cut in my sole of foot like if cutting an apple with knife. I have been using turmeric, aloevera and vit e paste, Now it is healing. My blood sugar is high. I don't feel pain even it is cut. Please advice me. Yours grateful.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, if you have a big cut on the sole of the foot, first thing is to control blood sugar strictly, with fasting around 100 mg and pp 150 to 160 mg and hba1c%<7. Otherwise healing may be affected. Then use some systemic antibiotic to control or prevent any infection, then do proper dressing by using antiseptic ointment.

I'm asking questions for someone else. She is 33 years old. She is diabetic patient. Now she is suffering from bulky ovaries. Which is diagnose by ultra sound scan. What are the treatment for it? Surgery is necessary or it can be cure with the help of medication?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, you are probably talking about poly cystic ovaries. (pcod). This condition can be treated with drugs. Surgery is hardly needed. It is not a completely curable condition. Various methods are available, like life style change, particularly exercise and weight reduction. Medications to regulate menstrual cycle etc. Surgery is not an option. Thanks.

My father is 78yrs old, he is having severe burning sensation in stomach, so pl guide us, before giving any guidance I want to mention one thing that my father is diabetic & heart patient.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
The reasons for burning pain in epigastric & hypochondriac region could be many. But invariably such a symptom may occur due to excess acidity and or peptic ulcer. But since he has cardiac problems sometimes those conditions also mimic such symptoms. So it is better to consult your local physician. If simple excess acidity is there then that can be controlled by using drugs called proton pump inhibitors, on empty stomach.

Mr Prabhat, To give any guidance I need to know present fasting and PP sugar levels, HbA1c%, weight and height, dietary pattern and extent of exercise. It is better if you come in a private conversation with all the details, where I will be able to give specific advice. In the mean time take diet that provides exact calories based on your ideal body weight, sugar levels and extent of daily exercise. Refrain from alcohol and smoking. Consume good deal of green leafy vegetables and salads.

To know the whole things you have to understand basics of medical science. As of stopping medicine, pl do not think of it. Having been dependent on such drugs for three decades, you can't switch to other things suddenly and that too at this stage. When chemical diuretics are given (in HTN/ oedema/ renal problems etc), electrolyte depletion takes place as a side effect such as potassium depletion etc which is again supplied through potassium drugs. This vicious cycle goes in. Here Ayurveda is different. SLOW but natural. Diuretics are such that there would not be electrolyte imbalance/ depletion. But this doesn't mean that allopathy is bad. It has wonderful life saving developments to its credit. On the other hand Ayurveda's stress on lifestyle and natural medicine has wonderful results but its limited too esp in critical life conditions. Need is to respect both approaches where needed. It has to be understood by both ailing humanity and medical fraternity. I would suggest you to gradually supplement your mother's treatment with herbals for introducing little comfortable life ahead without compromising on medical facilities. If agree, pl consult with more details.

I am suffering with hypothyroidism since more than a. Year is there any permanent cure for it? Am taking 25 mcg thyronorm.

MBBS, MRCGP ( UK), Diploma in Diabetes (UK), DFSRH (UK), DRCOG (UK)
Endocrinologist, Hyderabad
Hello, Hypothyroidism is a fairly common condition. As of now there is no cure for this (initially 20-30 years back when hypothyroidism was due to iodien deficiency, by replacing the iodine the condition could be treated. Now a days this iodine deficiency is rare due to government's initiative of fortifying common salt with iodine). Now-a-days hypothyroidism is mainly autoimmune. So you need to take your tablets regularly and keep checking your Thyroid function tests once in 3 -4 months if the levels are stable. Thank you.

Treating Infertility Caused by PCOS With IVF

MBBS, DGO, MD, Fellowship in Gynae Oncology
Gynaecologist, Delhi
Treating Infertility Caused by PCOS With IVF

PCOS is an abbreviation for Polycystic Ovarian Syndrome. It is a problem that is caused in women when their hormones are out of balance. It affects menstrual periods and can even hinder conception when aggravated to an advanced stage. Many women having PCOS develop small cysts on their ovaries. These cysts might not be harmful, but they lead to hormonal imbalances.

Common symptoms include:

  1. Gain of weight or Weight loss problems
  2. Acne
  3. Excess hair on body and face
  4. Irregularities in periods
  5. Fertility problems
  6. Depression

It is possible for most women with PCOS who fail to attain pregnancy easily to use other treatments to get pregnant. Using In vitro fertilization (IVF) is one such mode of treatment. The procedure is helpful if done within the age of forty. You should undergo IVF only in a high quality clinic under the supervision of an experienced doctor.

There is a considerable amount of risk of having a higher order multiple pregnancy due to treatment of PCOS infertility using injectable gonadotrophins. Recent developments have allowed the patients to maintain high rates of pregnancy while simultaneously controlling the risk of multiple pregnancy. A variant of the standard IVF treatment known as Blastocyst embryo transfer is useful in the control of multiple birth rates. Generally, success rates for IVF treatment in cases where infertility is caused due to PCOS is quite high.

IVF Protocols for PCOS
There are two simulation protocols that are commonly used with PCOS:

  1. Ganirelix with Lupron trigger protocol: Used to avoid ovarian hyperstimulation syndrome.
  2. PCOS down-regulation protocol

This protocol combines:

  1. Oral Contraceptive Pills
  2. Metformin

A calendar is followed for the down regulation protocol for PCOS. Before starting with Lupron, a dosage ( 500 mg, 3 times a day) of metformin is followed daily. After this, oral contraceptive pills are prescribed for around 21 days. Lupron coincides with the last 3 pills of birth control.

Lupron is a subcutaneous injection of a medicine that is taken once daily. Simulation of the ovaries is initiated after 6 or 7 days without using pills. It is very important with polycystic ovary that the dose of FSH hormone is properly controlled and that the ovarian response is cautiously monitored. Over response to the treatment can cause ovarian hyperstimulation syndrome. This should be avoided.

Can a diabetic have tea, an article says having tea reduces diabetes and stroke by 37%. Is it true.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. lybrate-user, A person with diabetes can definitely take tea, but without sugar, as sugar adds extra calories. One can take about 2 to 3 cups in a day, but too much is not good as it does stimulate stomach acid secretion. Some reports keep cropping up on tea and coffee about their benefits, harms etc. Best is to ignor them, and just take a moderate quantity per day. Thanks.
