Importance of Occupational Therapy in Psychiatry

Psychiatric treatment is never in the hands of a single person. Mental health treatment is a collaborative effort between the psychiatrist, occupational therapist, individual and his or her support system. The primary goal of occupational therapy is to engage people in some sort of activity to enhance their ability to lead a meaningful life. This need not be restricted to work but can include simple things like cooking, exercising, painting etc.

To understand what kind of treatment a patient needs, an occupational therapist will first assess the person, their needs and concerns. They understand how an illness or a particular life event can impact a person's ability to perform routine tasks. The counselor and therapist will then work together with the patient to explore possible lifestyle changes and set goals related to this. These changes are centered around the patient's strengths and preferences and take into consideration the occupations that are the most meaningful to the patient. An occupational therapist can then help the patient develop the necessary skills and find ways to make their daily lifestyle easier to handle. Thus, their lives become meaningful and satisfying and they feel a sense of achievement.

Occupational therapists also address social barriers towards the rehabilitation of a person in a community. This is done through interventions that focus on building the patient's existing skill set, creating opportunities for social integration, preventing a relapse and modifying the environment. Some of the barriers it addresses include:

  1. Stigma: By providing the patient with an opportunity to master a skill, occupational therapy boosts self confidence. This helps address the stigma against people with mental difficulties being able to form strong interpersonal relationships.
  2. Safety: A person suffering from a mental disorder is often understood to not be able to take care of himself. By helping the patient create routines for himself and giving him the confidence to perform daily tasks like cooking etc, he is at a much lower risk of accidents than before.
  3. Socioeconomic status: By giving the patient the necessary skill set to hold a job and collaborating with employers and educators, an occupational therapist can help raise a person's socioeconomic status.

Occupational therapy can benefit any person diagnosed with a mental illness or who has recently undergone a traumatic accident. Having a baby not only changes the equation between the couple but also, in some cases, the equation between the couple and other friends and family. Many parents do receive a lot of support at the time of the delivery and also receive some initial support on making the shift back home from the hospital. After that, in most cases, you are left to fend for yourselves and you have to iron out the details in front of you as you go. In order to do this in the best way possible, it is very crucial for both the parents of the baby to work as a team.

Questions that you need to answer in an objective and frank manner include those which pertain to who does what for the baby, how you will manage adequate rest as well as how you will divide other activities that need to be carried out around the house.

Quite a few couples who, prior to the baby, have amicable relations end up arguing a lot more after the baby is born. The level of exhaustion and responsibility, which comes as part and parcel of not only looking after oneself and providing a level of care to the partner, but also caring jointly for another human is quite a task.

For many couples, the stress shows by the way of strain. Taking a few deep breaths will help tone down anything, which may spark into an argument and will help you solve your issues in a calm manner. Spending time together while running chores for the baby will help you bond better. Taking time out for each other while the baby is asleep or away to a day care or a school is also very important for maintaining a healthy relationship.

मिर्गी उन बिमारियों में से है जिनका पहचान काफी पहले ही हो चुकी थी. आज हम मिर्गी के लक्षण, कारण और उपचार को लेकर बात करेंगे. विशेषज्ञों के अनुसार मिर्गी की बीमारी तंत्रिका तंत्र में विकार आने के कारण होती है. तंत्रिका तंत्र का सीधा सम्बन्ध मस्तिष्क से है. जब मस्तिष्क में विकार आता है तो इसकी वजह से तंत्रिका तंत्र प्रभावित होता है. फिर मिर्गी का अटैक आता है और पीड़ित का शरीर अकड़ जाता है.
इस बीमारी को लेकर पहले समाज में कई तरह की भ्रांतियां प्रचलित थीं. हलांकि अब भी कई लोग इसे भुत-प्रेत से जोड़कर देखते हैं. कई बार तो मिर्गी के मरीज को पागल की तरह भी ट्रीट किया जाता है. यहाँ ये बताना आवश्यक है कि मिर्गी भी बस एक बीमारी के सिवा कुछ नहीं है. इस लिए इसके मरीजों को तुरंत किसी चिकित्सक के पास ले जाना चाहिए. हम मिर्गी के के विभिन्न पहलुओं पर प्रकाश डाल रहे हैं.

मिर्गी होने का क्या कारण है?
शरीर के सभी अंगों की संवेदनशीलता मस्तिष्क द्वारा तंत्रिका तंत्र को दिए गए निर्देश के अनुसार ही काम करती है. मस्तिष्क में स्थित न्यूरॉन्स ही तंत्रिका तंत्र को सिग्नल देते हैं. लेकिन जब इस प्रक्रिया में बाधा पहुंचती है यानी कि न्यूरॉन्स तंत्रिका तंत्र को सही निर्देश नहीं दे पाते हैं तब मरीज को मिर्गी का दौरा पड़ता है. इसमें शरीर का अंग विशेष कुछ देर के लिए निष्क्रिय हो जाता है. इसके कुछ संभावित कारण निम्लिखित हैं-

1. न्यूरोलॉजिकल प्रॉब्लम्स जैसे कि अल्जाईमर.
2. अनुवांशिक कारण यानि आपके खानदान में किसी को रहा हो या हो.
3. ब्रेन ट्यूमर
4. जन्म से ही मस्तिष्क में ऑक्सीजन का आवागमन पूर्ण रूप से बंद होने पर.
5. मस्तिष्क ज्वर और इन्सेफेलाइटिस के के संक्रमण से.
6. ब्रेन स्ट्रोक के कारण ब्लड वेसल्स को नुकसान पहुँचने से.
7. कार्बन मोनोऑक्साइड की विषाक्तता से.
8. अत्यधिक मात्रा में ड्रग लेना.

क्या हैं मिर्गी के लक्षण?
जाहिर है हर बिमारी के कुछ न कुछ लक्षण होते हैं जिसके आधार पर हम इसकी पहचान करते हैं. ठीक उसी तरह मिर्गी के भी कुछ प्रारंभिक लक्षण हैं. इन लक्षणों के नजर आते ही आपको डॉक्टर के पास जाना चाहिए. हलांकि जब मिर्गी के दौरे पड़ते हैं तो एकदम स्पष्ट हो जाता है.
1. मरीज का पूर्ण रूप से बेहोश हो जाना या आंशिक रूप से मूर्छित हो जाना.
2. अपना जीभ खुद से काटना या असंयमित हो जाना.
3. सर और आँख की पुतलियों का लगातार घूमना.
4. हाथ पैर और चेहरे की मांसपेशियों में खिंचाव उत्पन्न होना.
5. मूर्छा से उठने के बाद मरीज का उलझन में होना 
6. पेट में गड़बड़ी होना.
7. चिकित्सकीय पहचान: एक्सपर्ट्स का कहना है कि इन सब लक्षणों के बावजूद मरीज का मेडिकल करने के बाद ही उसके रोग का निर्धारण किया जाता है. डॉक्टर मरीज़ के पल्स रेट और ब्लड प्रेशर को चेक करते हैं. फिर इसके बाद न्यूरोलॉजिकल साइन का भी परिक्षण करते हैं. इसके साथ ही इस रोग का पता ई.ई.जी, सी.टी.स्कैन या एम.आर.आई और पेट के द्वारा लगाते हैं.

क्या है इसका उपचार और कैसे करें रोकथाम?

मिर्गी एक बेहद संवेदनशील बिमारी है. इसके उपचार के लिए सबसे महत्वपूर्ण उपाय बताया जाता है कि जब भी मिर्गी का दौरा आए उस समय सीजर को नियंत्रित करना. इसे नियंत्रित करने के लिए एन्टी एपिलेप्टिक ड्रग थेरपी और कुछ सर्जरी होती है. हलांकि डॉक्टर्स का ये भी कहना है कि जिन लोगों पर दवाई का असर नहीं होता है उन्हें सर्जरी करने की सलाह दी जाती है. मिर्गी के मरीजों को ज्यादा वसा वाले खाने से दूर ही रहना चाहिए. खाने में ज्यादा से ज्यादा कार्बोहाइड्रेट वाले खाद्य पदार्थों का इस्तेमाल करना चाहिए. इससे सीजर की आवृत्ति में कमी आती है.

रोकथाम के उपाय
आपको जानकर हैरानी हो सकती है कि अभी भी मिर्गी के कारणों का ठीक से पता नहीं लगाया जा सका है. हलांकि शिशुओं के जन्म के दौरान जेनेटिक स्क्रीनिंग की सहायता से माँ को बच्चे में इसके होने का पता लगाया जा सकता है. इससे बचने के लिए आप इस बात का ध्यान रखें की आपके सर में चोट न लगे. इसके अलावा आप ये उपाय कर सकते हैं-

1. जितना हो सके तनाव से दूर रहें.
2. खाने में संतुलित आहार लें.
3. डॉक्टर द्वारा दी गई दवा का सही तरीके से सेवन करते रहें.
4. नियमित रूप से चिकित्सक से सलाह लेते रहें.
5. पर्याप्त नींद लेना भी अत्यंत जरुरी है.

You can chew Nicorette gum which is nicotine replacement. There are medicines available. Which can be prescribed if asked personally Cigarette smoking is a leading preventable cause of mortality. It kills more than 60 lakh people worldwide every year. More than 50% of regular smokers would invariably die from tobacco-related illnesses which includes heart attack, cancer or asthmatic lung diseases. Best age to stop smoking is before 40 31st May is “World No Tobacco Day” Quitting smoking before the age of 40 is associated with larger decline in premature death than stopping it at a later date. Best age therefore to stop smoking is before 40. However, stopping smoking even after the age of 50 is still associated with lower risk of death as compared to those who continue to smoke. Even in smokers aged 80 years or more, quitting smoking appears to reduce some mortality.

Diet has no role with concentration and you need to control mind and do yoga and meditaion and follow these study tips CONCENTRATION TIPS Here’s some tips to get into a state of deep concentration where work / or studies flow easily so that you can do well in examinations/ and or do your work well. 1) Cut Off the Noise Getting into a state of concentration can take at least fifteen minutes. If you are getting distracted every five, you can’t possibly focus entirely on your work. Request that people don’t interrupt you when working on a big project. If you are required to answer phones and drop-in’s immediately, schedule work when the office is less busy. 2) Structure Your Environment The place you work can have an impact on your ability to focus. Try to locate yourself so you are facing potential distractions such as doors, phones or windows. This way you can take a glance to assess sounds that would otherwise break your focus. 3) Clarify Objectives Know what your goal is clearly before you start. If you aren’t sure what the end result is, the confusion will make it impossible to focus. 4) Divide Blobs. If you have a large project that needs work, clearly identify a path that you will use to get started working on it. If the sequence of actions isn’t obvious, it will be difficult to concentrate. Taking a few minutes to plan not only your end result, but the order you will complete any steps, can save hours in wasted thinking. 5) Know the Rules Get clear on what the guidelines are for the task ahead. What level of quality do you need? What standards do you need to follow? What constraints are there? If the rules aren’t clear from the outset, you will slip out of concentration as you ponder them later. 6) Set a Deadline Deadlines have both advantages and disadvantages when trying to force concentration. A deadline can make it easier to forget the non-essential and speed up your working time. If you give yourself only an hour to design a logo, you will keep it simple and avoid fiddling with extravagant designs. Time limits have disadvantages when they cause you to worry about the time you have left instead of the task itself. 7) Break down Roadblocks Roadblocks occur whenever you hit a tricky problem in your work. This can happen when you run out of ideas or your focus wavers. Break down roadblocks by brainstorming or planning on a piece of paper. Writing out your thought processes can keep you focused even if you might become frustrated. 8) Isolate Yourself Become a hermit and stay away from other people if you want to get work done. Unless your work is based on other people they will only break your focus. Create a private space and refuse to talk to anyone until your work is finished. Put a sign on your door to steer away drop-ins and don’t answer your phone. 9) Healthy Body, Sharper Mind What you put into your body affects the way you concentrate. Nobody would expect peak performance if they showed up drunk to work. But if you allow yourself to get chronic sleep deprivation, overuse stimulants like caffeine or eat dense, fatty foods your concentration will suffer. Try to cut out one of your unhealthy habits for just thirty days to see if there is a difference in your energy levels. I’ve found even small steps can create dramatic changes in my ability to focus. 10) Be Patient Before I write an article, I often sit at my desk for a fifteen or twenty minutes before I put finger on the keyboard. During this time I feel a strong urge to leave or do something else. But I know that if I am patient, I’ll stumble upon an idea to write about and enter a state of flow. Without a little patience, you can’t take advantage of flow when it rushed through you. If you need strong concentration I recommend periods of 90-120 minutes. Any less than that and you will waste too much time getting started before the flow can continue. More than this is possible to sustain focus, but you will probably benefit from a quick break. These are the general guidelines for concentration, studying well or doing your job. Regards.

I was a very active person since school days. From past 2 years I have noticed my mood swings have varied alot. sometime I will be very active and sometime don't even want to move. In past couple of months, life has been tough on me. Too many Downfalls in life at once. And now my this mood has gone dark. I try to stay happy but I end up being inactive. Cry for no reason. Then suddenly for few hrs I go active again and happy. Then back to darkness. I just want to stay alone. Avoid talking. Am I suffering from depression? And if yes, is it bipolar? I am confused. You seem to be suffering from Borderline Personality or Emotionally unstable personality. The cause is unknown but research suggests there is an interaction between adverse life events and genetic factors. Neurobiological research suggests that abnormalities in the frontolimbic networks are associated with many of the symptoms There is a pattern of sometimes rapid fluctuation from periods of confidence to despair, with fear of abandonment and rejection. There is a particularly strong tendency towards suicidal thinking and self-harm. They have love-hate relationship with close ones. Transient psychotic symptoms, including brief delusions and hallucinations, may also be present. It is also associated with substantial impairment of social, psychological and occupational functioning and quality of life. People with emotionally unstable personality disorder are particularly at risk of suicide. Its course is variable and, although many people recover over time. Kindly consult a psychiatrist for remedy.
