A high uric acid level can be caused when your body either produces too much uric acid or your kidneys don't eliminate uric acid rapidly enough. A high uric acid level may cause increasingly frequent attacks of gout, or it may never cause problems. A high uric acid level may also cause some people to develop kidney stones or kidney failure. And some people with a high uric acid level also develop high blood pressure, heart disease or chronic kidney disease, but it's often unclear whether this is a direct cause or merely an early warning sign of these conditions. Factors that may cause a high uric acid level in your blood include: Diuretic medications (water pills),Drinking too much alcohol,Genetics (inherited tendencies),hypothyroidism (underactive thyroid),Immune-suppressing drugs,Niacin, or vitamin B-3, obesity, psoriasis, Purine-rich diet — organ meat, game meat, anchovies, herring, gravy, dried beans, driedpeas, mushrooms and other foods Renal insufficiency — inability of the kidneys to filter waste Tumor lysis syndrome — a rapid release of cells into the blood caused by certain cancers or by chemotherapy for those cancers,Also, you may be monitored for high uric acid levels when undergoing chemotherapy or radiation treatment for cancer. Avoid protein diets, avoid more milk products. bad habits , fast food/fat/oily,salty foods. Coffee, smoking, alcohol intake. Non-veg., take whole grain, oats, green veg.,plenty of water in day time, 1 glass of water at night before bed. Take tab. Zyloric 100 mg once a day. Tab. Ulracet sos for pain in joints.

My wife, 66 years, suffered a intracerebral brain hemorrhage stroke more than a year ago. She is not under any medication for this other than her regular drugs for diabetics and hypertension. She still feels numbness around her right cheeks and continues to feel bitter taste in her saliva. Is this a common phenomena as some doctors say? Is there any medicine that can help to improve without much side effects? (She is too sensitive to most of the common medications.)

Quest" What is the initial symptoms of diabetes? How can be people be alerted from such type of diseases.

Quest" What is the initial symptoms of diabetes? How can be people be alerted from such type of diseases.
Mr Faisal, Diabetes mellitus, generally does not produce any symptoms, that is why many people do not come to know that they have a problem. However, when disease develops fully it may cause excess urination. excess thirst, excess hunger, unexplained tiredness, weight loss. Initiation of proper treatment invariably corrects all the symptoms. To know whether one has diabetes is to check blood sugar levels periodically, once in 6 months to one year. Particularly if parents have T2 DM, then all the children must do a periodic check up.

If you are diabetic and on diet & exercise treatment or in addition oral drugs or insulin you are required to test fasting &pp atlest once in 3 months if your control is good as in your case. Also get hba1c atleast 2-3 times a year for monitoring. Every year one should check for various organ complcions such as kidney eyes heart nerves blood vessels etc

When to take insulin in diabetes

From being diagnosed as a new diabetic to many years later after it the question that haunts diabetics is whether i should take insulin now or in the future. If you are a type diabetic (in young people) you start with insulin right away but if you are diagnosed with late onset diabetes (above 35 years- exceptions apply) you delay it for as long as possible.

The answer to this is simple. You continue on diet, exercise and tablets as long as you can maintain your sugar levels within normal and when inspite of this your blood sugar levels are uncontrolled (consitently above 200 mg/dl) and you are developing complications such as high blood sugar levels, infections (unresolving) or ketoacidosis etc. Obviously the trouble to take insulin is not more than being confined to an ICU bed and hence you will need to start taking insulin once twice or even thrice a day.

Mr. Lybrate-useryou have not given the status of the sugar levels and whether taking insulin or not. Leg pains maybe due to diabetic neuropathy and fever due to some infection. Consult a physician and do fasting, postprandial sugar levels and hba1c. Modify the drugs according to report and proper treatment for fever and infection. Contact if need further assistance.

What a person should avoid in diabetes in eating and which precautions and exercise should be done?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
What a person should avoid in diabetes in eating and which precautions and exercise should be done?
Mr Nitin, A person with diabetes has to take a balanced diet containing adequate carbs, proteins and fats, like every one else. However, total calories provided should match his exact need based on ideal body weight, blood sugar levels and extent of exercise. He should avoid sweets, direct sugar and jaggery, deep fried foods, junk foods and pastries. Plus avoid high fat cheese, butter and ghee. Should consume more of green leafy vegetables, salads, and permitted fruits (100 g a day). Refrain from alcohol and smoking. As regards exercise, any exercise that one is capable of doing is good enough. Brisk walking is excellent. Others like surya namaskar, yoga asanas, jogging and running are also good. One that suits is to be done.

Mr. Lybrate-user, your liver enzyme sgpt (alt) is in the normal range. (7 to 56 units/l). Ldl is a bit high, it is better to have it below 100 mg/dl. Gastrointestinal disturbances some times do occur in diabetes like gastroperesis where there is delaying of stomach emptying and related to that there may some symptoms which can be corrected by prokinetic drugs. But most important is to have very strict blood glucose control like fbg near 100 mg, pp 150 to 160 mg and hba1c% <6.8. To keep liver healthy alcohol should be kept away totally. Similarly deep fried foods, junk foods, pastries and other bakery items also be avoided. Regular exercise of 1 hour is a must. Thanks.

Mr metformin can be taken up to 2500 mg per day. But 100 mg is also a high dose only. If it is not causing any gastro intestinal side effects then no problem. It is really good that you have started on insulin because looking at amount of drugs that you are taking, it is necessary for you to shift to insulin. By doing self monitoring of blood sugar daily you can increase the insulin dosage every alternate day till you reach target fasting level. It is good to reduce or take away pioglitazone.

What is a best treatment for the diabetes patient for diabetes type two, insulin is a best ya other is good medicine?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
What is a best treatment for the diabetes patient for diabetes type two, insulin is a best ya other is good medicine?
Mr. Lybrate-user, diabetes mellitus occurs (t2) either due to lack of insulin or resistance to its action or both. In t1 there is total absence of insulin, beta cells of islets of langerhans in pancreas do not secrete any insulin. Since main reason is insulin deficiency, best drug for treatment is insulin. However, in t2 initially metformin is used, which helps in reducing resistance to insulin action and also helps in weight reduction. Then other drugs known as secretogogues (insulin secretion stimulators) are used. If diabetes is very severe with very high blood sugar levels and hba1c% >9, then insulin is preferred. So it depends on individual case and needs of each patient. There are multiple choices now, recent addition to treatment has been sglt-2 inhibitors, which act independent of insulin.
