I did plates n got period for four days. Early to what I was suppose to get. Is it normal.
Yes it is normal at your age. Irregular cycles are common during your age because of decline in estrogen hormone. It may become scanty and can come every 2-3 months also. If you have never been screened with pap test or ultrasound is not done, then it is an opportunity for you to get it tested now.

One of my friend told me that her boyfriend has been complaining about her vaginal odour however she finds it perfectly normal. I have read a lot about it to help her out, but it seems that she is taking all precautions and still the problem persists. Can you explain how and why this situation may arise?

You can insert clingen pessary once a day inside vagina for 3 days. It will clear all the infections in the vagina which produces bad odour.

Hi My wife is pregnant and due date is 1st March given by doc on last visit but not having any symptom of labor pain and water break. Please suggest what to do.

Hi My wife is pregnant and due date is 1st March given by doc on last visit but not having any symptom of labor pain ...
She should wait for natural labour, but in case she doesn't get any contractions, you would probably have to go for induced pains via injections and drips, provided there is no foetal distress and her other parameters like BP etc is controlled.

Foods For Hyperthyroidism

1. Foods Belonging To Brassica Family
Consume Foods like cauliflower, Brussels sprouts, peanuts, cabbage, broccoli, kale, mustard greens, turnips, rutabagas, soybeans, spinach etc which contains goitrogens.
• Goitrogens helps to block the ability of thyroid to use iodine and hence slows down the activity of the thyroid.
• Eat raw cruciferous vegetables to get maximum benefit as goitrogens are active in the raw state.

2. Include More Berries
Add at least one serving of berries in your diet plan to control hyperthyroidism.
• Blueberries, raspberries, strawberries contain anti-oxidants which improves your immune system.

3. Omega -3- Fatty Acid And Proteins

Omega -3- fatty acid helps to control the hyper stimulated thyroid gland. Proteins are necessary for the building of muscles and to provide energy for the body. Protein sources also contain L-glutamine which helps in rebuilding the gut and de-inflaming the body.
• Eat cold water fish, flax seed oil and nuts to get the omega-3-fatty acid necessary for the body.
• Include soy, turkey, walnuts, salmon, tuna and trout in your hyperthyroidism diet.

4. Dairy Products

Hyperactivity of the thyroid gland can remove the calcium from your blood. The body compensates this loss by leaching calcium from the bones. This can cause osteoporosis.
• Include three servings of yoghurt, cheese, milk or cottage cheese in your daily diet.

5. Spinach And Collard Greens

If you are allergic to dairy products, you can get your daily supply of calcium from vegetable sources like spinach collard greens etc.
• Figs, prunes and sesame seeds are also excellent hyperthyroidism foods.

6. Zinc

This is another vital component of hyperthyroidism diet. The hyperactive thyroid gland can deplete the zinc.
• Include almonds, walnuts, pumpkin seeds, peanuts which are rich in Zinc in your diet.

7. Fiber Rich Foods And Anti Inflammatory Foods

Fiber rich foods are beneficial for the proper working of the thyroid gland. Anti inflammatory herbs help to reduce the inflammation of the thyroid which is the reason for hyperthyroidism.
• Include whole grain foods, like oatmeal, whole grain breads, brown rice etc in your food.
• The anti inflammatory herbs to be included in your diet are turmeric, ginger, rosemary, cinnamon and oregano.

8. Antioxidant Rich Foods And Vitamin D

It is also necessary to consume antioxidants and vitamin D rich foods in the diet. This will reduce the oxidative stress caused by hyperthyroidism. Vitamin D can reduce the inflammation and auto- immune reactions of the body.
• Avocados, olive oil broccoli etc contain antioxidants.
• Salmon is a good source of vitamin D. You can get your daily dose of vitamin D from eggs, mushrooms etc.
• Spend 20- 30 minutes in the sun to get the necessary vitamin D for the body.

9. Cassava, Sorghum, And Millet

These food items contain cyanogenic glucosides which blocks the iodine availability of the gland.
• These foods increase the immune function and gives protection from osteoporosis

10. Vitamin C And Vitamin E

These vitamins are helpful in reducing the oxidative stress caused by the hyperactivity of thyroid glands.
• All the citrus fruits are rich in vitamin C.
• Bell peppers, olive oil, wheat etc are a good source of vitamin E.

Menorrhagia (Heavy Menstrual Periods) - A Common Indication For Hysterectomy

M.D. Gyn. & Obstetrics, MBBS, Diploma in Advanced Gynaec Laparoscopic Surgery, D.N.B., F.C.P.S.(Mid. & Gynae), D.G.O., D.F.P.

Menorrhagia (Heavy Menstrual Periods) - A Common Indication For Hysterectomy

Menorrhagia refers to very heavy and prolonged menstrual bleeding or periods in women. The bleeding is abnormal in nature, and if this continues for a long time, a hysterectomy surgery needs to be carried out to solve the problem.

Bleeding during the night time and passage of large blood clots during menstruation are other symptoms of Menorrhagia.

Causes:

The various possible causes of this kind of heavy menstrual bleeding are:

  1. Hormonal imbalance, specifically of estrogen and progesterone is a common cause, which is more likely in adolescents and in women nearing menopause. Dysfunction in the ovaries also causes hormonal imbalance.

  2. Non-cancerous tumors or fibroids may also be responsible for menorrhagia.

  3. Women suffering from a miscarriage or ectopic pregnancy are likely to experience heavy menstrual bleeding.

  4. Using blood thinners may lead to menorrhagia.

  5. An intrauterine device used for birth control measures can cause disturbance and may lead to heavy menstruation.

  6. A condition known as adenomyosis, where the glands in the uterus lining get embedded in the walls of the uterus, also causes menorrhagia.

  7. Pelvic inflammatory diseases, infection in the uterus or fallopian tubes are other common causes.

  8. Heavy bleeding may occur in patients with ovarian or cervical cancer.

  9. Diseases of the kidney, liver or thyroid diseases may also cause menorrhagia.

Diagnostic Tests:

Several tests and methods are carried out for the diagnosis of menorrhagia. They are:

  1. A general physical examination.

  2. Bleeding diary of the woman.

  3. Blood tests to detect anemia or thyroid.

  4. Pap Smear for the evaluation of cervical inflammation, infections or cancer

  5. Ultrasound for evaluation of pelvic organs like uterus, ovaries and pelvis.

  6. Hysteroscopy, where a camera is inserted into the uterus to observe the linings.

  7. Sonohysterography, where a fluid gets instilled into the uterus along with an ultrasound test.

Treatment:

Certain drugs are used to treat menorrhagia. They include:

  1. Iron supplements for treating anemia.

  2. NSAIDS or non-steroidal anti-inflammatories for treating and reducing blood loss.

  3. Tranexamic acid is used for reducing menstrual bleeding.

  4. Several oral contraceptives are used for regulation of the menstrual cycle. Oral progesterone corrects hormonal imbalance and reduces bleeding.

Surgical procedures for treatment of menorrhagia include:

  1. Hysteroscopy, dilation and curettage, where uterus lining is scraped and evaluated.

  2. Uterine artery embolization treats menorrhagia with fibroids.

  3. Focused ultrasound ablation enables the killing of the fibroid tissue.

  4. Myomectomy, endometrial ablation and endometrial resection are other surgical techniques.

  5. Hysterectomy surgery is the penultimate solution, where the uterus, cervix or ovaries are completely removed.

Menorrhagia is characterized by abnormally heavy menstrual bleeding in women. This is quite unhealthy and proper measures must be taken for curing the heavy bleeding.

Leprosy: Treatment and Prevention

MD - Dermatology , Venereology & Leprosy, MBBS
Dermatologist, Gurgaon
Leprosy: Treatment and Prevention

Mentioned throughout history as a curse, leprosy was considered as a punishment of sins by many cultures as it was widespread and common. However, modern science demystified it and de-stigmatized it to the point where the occurrence of leprosy within the population has gone down to very low levels, although it is still prevalent among certain populations.

Prevention of leprosy: Leprosy is caused by the bacterial infection of Mycobacterium leprae and it is classified into multiple types. Leprosy is communicable through contact with common objects or a person by infected droplets within their nose. It primarily affects mucous membranes and the skin and the nerves on the hands, feet, and in extreme cases may cause disfigurement as well. There are no specific ways to prevent leprosy as no specific correlation between transmission and development has been found so far. Leprosy can happen through one time contact or through contact over extended period of time within the same household. Thus preventive measures will primarily include the following:

  1. Regular checkups by going to the doctor
  2. Ensuring that surfaces are kept clean if you share a household with a leprosy patient
  3. Skin biopsies at regular intervals just to check if you may have contracted the bacteria
  4. If a rash has developed, then it is urgent that you visit the doctor. Earlier the leprosy is detected, quickly it can treated and resolved

Incidences of leprosy have reduced over the last few decades due to improvements in medicine and early detection. This has allowed people to nip the problem in the bud and does not allow it to spread within their bodies or to other people.

Treatment of leprosy: The primary weapon that modern medicine has against leprosy is known as MDT or multidrug therapy. Multidrug therapy is the combination of multiple drugs, which attack the various aspects of leprosy, such as skin problems, nerve damage, loss of sensation, deterioration of skin conditions etc. This is the proven way to not only reduce symptoms, but also cure the problems in the long run.

Some of the drugs in the MDT used are as follows:

  1. Rifampicin and dapsone
  2. Rifampicin and clofazimine and dapsone
  3. Rifampicin, minocycline and ofloxacin

However, the combination will depend on the type of leprosy the person is affected with, such as paucibacillary leprosy or multibacillary leprosy and thus can only be prescribed by the doctor after thorough and detailed examination.

Prolactin is the hormone that stimulates milk production by the breasts. The normal prolactin level is < 20 ng/ml. In most women the tumors are detected when they are small (microadenomas) and the prolactin level is only moderately elevated (30 - 300 ng/ml. High levels of prolactin may mean that the pituitary gland is making excess prolactin for unknown reasons (idiopathic hyperprolactinemia). Other conditions that can cause high prolactin levels include pregnancy, liver disease (cirrhosis), kidney disease, and hypothyroidism. MRI is needed to find out the cause and Hyperprolactinemia may occur in patients receiving amisulpride at low dose of 50 mg/day and results in galactorrhoea, amenorrhea and sexual dysfunction. The symptom ameliorates on withdrawing the drug, switching to non-prolactin-elevating drugs, and timely management with dopamine agonist.

Dieting to lose weight during pregnancy is not recommended, because you may not receive enough nourishment for you and your baby, and it may increase your risk for premature delivery. Meal plan during pregnancy: carbohydrate inadequate carbohydrate intake can result in low blood sugar (hypoglycemia) for women taking insulin and in ketone production for women who have gestational diabetes. Excessive carbohydrate intake can result in elevated blood sugar levels. Make sure your meal plan contains: complex carbohydrate, especially foods high in fiber, such as oatmeal, brown rice, bran cereal, whole wheat bread, whole wheat pasta, and beans. Fresh fruits. Milk. Fresh or frozen vegetables. Limit these carbohydrate foods in your diet: refined sugar and foods with a high content of refined sugars (sweets) refined starches, such as highly processed breakfast cereals, instant potatoes, instant rice, or instant noodles fruit juice protein if your kidney function is impaired, your protein allowance may be lowered. Fat monounsaturated fats and omega-3 fats, rather than saturated fats, should continue to be the primary source of fat in your diet. Fiber get enough fiber each day. Fiber can help stabilize your blood sugar levels and relieve constipation, which is common during pregnancy. Most people get far more sodium than they need. Talk to your doctor about how much sodium you should eat. Vitamins and minerals take a prenatal vitamin with folic acid and iron to meet your body's increased need for these micronutrients. Folic acid is needed for the production of blood cells. And iron is needed for red blood cells to deliver oxygen throughout the body. Folic acid has also been proved to reduce the risk of fetal neural tube defects. You may need to take a vitamin b12 supplement, which is important for the production of red blood cells, and a vitamin d supplement if you are a strict vegetarian (vegan). Vitamin b12 can only be obtained from animal sources in the diet. Other vitamins and minerals, such as the b vitamins and calcium, are important during pregnancy for producing energy and preserving your body's calcium stores. Very large doses (megadoses) of vitamins, especially vitamins a and d, are not recommended during pregnancy. Vitamins and minerals should only be taken under your doctor's supervision. Artificial sweeteners saccharin (sweet'n low, sugar twin), aspartame (equal, nutrasweet), acesulfame k (sunett), sucralose (splenda), and neotame are safe to eat when you are pregnant. Avoid using aspartame (equal or nutrasweet) if you have phenylketonuria (pku). Caffeine avoid caffeine. Alcohol do not drink alcohol. No amount of alcohol has been proved to be safe for a developing baby (fetus).

Having excess weight is a problem both for becoming pregnant and also for varicose veins. Also during the pregnancy the veins do become varicose too. So best option for you would be WEIGHT LOSS. Meet a dietician and get your diet planned, and also exercise in GYM under supervision of a doctor or trained GYM trainer. If your weight becomes less, the periods will become regular on its own and so will the ovulation During pregnancy again there is a weight gain of 8-10 Kg, so having good diet will help you before and during pregnancy.
