I have pcod problem. We are planning for pregnancy. I'm using metformin and evacure since from six months. Please suggest.

MD - Obstetrtics & Gynaecology
Gynaecologist, Mumbai
As your bmi is normal (55kg weight), there is no need to take metformin. It is given only in obese patients to reduce weight. Evacure is vitamin tablet, and it is not going to make you pregnant. In pcod, to get pregnant, clomiphene citrate to be started from day 3 of menstrual cycle and egg growth to be monitored by follicular scan. This is the normal course of treatment initially for any woman with infertility with pcod.

Hello, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned that your mother has l3/4 disc problem which is actually a mri finding. I would like to know from you a detailed history of her back pain which means duration of pain and mode of onset. I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias ( electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. It can be very helpful in decision making if I can see her detailed neurological examination ( motor & sensory examination - can be done by a physical therapist) so if you can send me these 2 things: - her mri images - her neurological status then I will be able to give an opinion. Do not hesitate to contact me if you need any further assistance.

Why Do I Get Knots in Muscles in Neck and Back ?

BPTh/BPT, MPTh/MPT
Physiotherapist, Noida
Why Do I Get Knots in Muscles in Neck and Back ?

What are muscle knots?
Muscle knots are points within a muscle where contracted fibers are unable to release, or, as the dictionary puts it: highly irritable localized spots of exquisite tenderness in a nodule in palpable taunt bands of muscle tissue.

A common problem for active people, muscle knots technically called myofascial trigger points, or mtps feel like a small knot to your fingertips. These knots can range from the size of a pinhead in smaller muscles to the size of your thumb in larger muscles. 2

Muscle knots can cause pain in two ways: (1) latent trigger points, which are knots that only hurt when you put pressure on them, and (2) active trigger points, which are knots that actively refer pain along your neural pathways, causing it in non-localized areas.

Trigger points in a constant state of contraction have excess metabolic waste and oxygen use because blood flow to this area stops this sends pain signals to the brain. Because your brain wants to stop the pain, it commands the muscle to rest, which leads to under-usage of the muscle. This is what makes the muscle shorten and tighten up.

What causes muscle knots?
The most common causes of muscle knots are:

1) accidents acute trauma, such as bad falls sports injuries that strain you joints and muscles

2) postural stress sitting too long with poor posture, sitting with no support, lifting improperly

3) overstimulation

Dear lybrate user. Many people think that they are ADDICTED to masturbation / sex, where as they are actually just used to masturbate / have sex regularly and can’t quit easily. Addiction is problematic and being used to is not, though there is only a thin line separating both. Masturbation / sex is not harmful as long as it is not an addiction. We need to determine if you are addicted or just used to. Sex thoughts, Sex addiction, masturbation addiction, porn films addiction etc comes under behavior addiction. Behavioral addiction is a form of addiction that involves a compulsion to engage in a rewarding non-drug-related behavior – sometimes called a natural reward – despite any negative consequences to the person's physical, mental, social or financial well-being. Behavior addiction needs to be curbed or controlled using your own self will power. Diverting your mind and body away from the circumstances which compels you for the addictive habit, with the help of physical exercises, games, entertainment etc can help you. Never sleep alone. Be always with your family even at night etc are some other tips. Willpower is the key always. Behavioral therapy also works well. Take care.

Causes of headaches may include fatigue, sleep deprivation, stress, the effects of medications, the effects of recreational drugs, viral infections, common colds, head injury, rapid ingestion of a very cold food or beverages, and dental or sinus issues. This can trigger your headache: foods : spices, wine , chocolate, citrus •food additives •sleep : both too much and too little •stress : mainly offset Home remedies: •Stay Hydrated •Mix equal parts of ginger juice and lemon juice. Consume it once or twice a day. •Menthol and menthone are the primary components of mint which are very effective in alleviating headaches. •Peppermint contains menthol that helps open up clogged blood vessels that cause headaches. It also has calming and soothing effects. •Basil works as a muscle relaxant, making it a helpful treatment for mild headaches caused by tense muscles. Plus, it has calming and analgesic effects.•Simply smelling the soothing scent of lavender essential oil can be of great help in relieving tension headaches. Research suggests that it can also help improve migraine symptoms. •The cold from ice helps reduce inflammation that contributes to headaches. Plus, it has a numbing effect on the pain. •Simply massage your forehead and temples with a few drops of rosemary oil mixed in a tablespoon of carrier oil. •Crush a few cloves gently and put them in a sachet or a clean handkerchief. Inhale the smell of the crushed cloves whenever you have a headache until you get some relief from the pain. •Simply eat an apple with some salt. •Accupuncture: Press and massage the fleshy area between the thumb and index finger, at the highest spot of the muscle when both the thumb and finger are brought close together. Do this for one or two minutes and then repeat on the other hand. Exercise Regular exercise can reduce the frequency and intensity of headaches and migraines. When one exercises, the body releases endorphins, which are the body’s natural painkillers. Exercise reduces stress and helps individuals to sleep at night. Stress and inadequate sleep are two migraine triggers. Homeopathic treatment In order to prescribe a homeopathic remedy, it is crucial to discover what is unique about the way an individual experiences a headache. For example, all migraine sufferers may experience nausea, but a particular individual may feel thirsty for cold drinks with the headache. Thirst for cold drinks is the characteristic symptom that sets one migraine sufferer apart from others. There are hundreds of possible remedies for the treatment of headaches, so the key is finding which homeopathic remedy most closely matches your symptoms.
