My girlfriend is suffering with the problem of stones in kidney. What are the precautions to be taken to be get cured.

MBBS
General Physician,
My girlfriend is suffering with the problem of stones in kidney. What are the precautions to be taken to be get cured.
Prevention of renal stones depend on type of stone. Also there are various diseases and drugs that can predispose to stone formation. A usg, complete metabolic panel and urine cytology and ph check up is needed in young aged patients. To prevent oxalate stones thats very commonly found you need to restrict oxalate rich foods like: peanuts, rhubarb, spinach, beets, chocolate and sweet potatoes. To prevent uric acid stones, cut down on high-purine foods such as red meat, organ meats, and shellfish, and follow a healthy diet that contains mostly vegetables and fruits, whole grains, and low fat dairy products. Limit sugar-sweetened foods and drinks, especially those that contain high fructose corn syrup. Limit alcohol because it can increase uric acid levels in the blood and avoid crash diets for the same reason. Eating less animal-based protein and eating more fruits and vegetables will help decrease urine acidity and this will help reduce the chance for stone formation. Take plenty of water also.

Thank you for the query. From the symptoms you have mentioned, it seems that your father is probably suffering from prostatomegaly. In this condition the prostate gland enlarges and causes an obstruction in the urine outflow pathway. This leads to incomplete evacuation of the bladder at once, and hence the need for frequent urination and urinary retention. Your father needs a usg of the lower abdomen to see the status of the prostate and measurement of the residual urinary volume. A serum psa may be done. You need to consult a urosurgeon who can give you further opinion of the same. Some medications are available but in most cases a small surgery may be needed. A uroflowmetry may be needed. Regards

This is not an arthroscopic problem if it is related to a kidney stone. We put across a stent in the tubes which carry urine out of the kidneys, so that they are not blocked by the stone - this means that there is little pain and the stone, if it is small, comes down the tube and may be passed in the urine. It is called endourology.

Respected lybrate-user hi your case requires clinical evaluation dear but from your presentation I assume it could be muscular / developing hernia / small ureteric stone in few cases. I will advise to get an ultrasonography of abdomen pelvis done. Thanks regards.

Hi I am 34 male my coronary angiography report. Clinical resume. Recent amwi; thrombolysed with stk here on 9.1.2017....
As per your mentioned report, you had an heart attack which seems to have been managed well due to timely thrombolysis and as per this report only minimal blockage is present in this report. Based on this report the medical advice that has been given to you is fine. But we cannot comment with 100% surety without looking at the angiography CD, because sometimes the study is either not interpreted or done correctly, but assuming that this is the report your present treatment is ok.

Modern treatment for Kidney Cancer

Felllow in Endourology, MCh Urology, MRCS, MS, MBBS
Urologist,
What is kidney cancer?
Kidney cancer (renal cancer or renal cell carcinoma) is one of the ten most common cancers in both men and women.
It occurs when the cells in kidney grow uncontrollably and start invading surrounding tissues (i. E, undergoes malignant change). The risk factors are age > 40 years, smoking, obesity, long term dialysis, certain genetic conditions, family history, high blood pressure and male gender. However, it can occur to anybody.

What are the symptoms of kidney cancer?
1. Blood in urine
2. Pain or lump in side of upper abdomen
3. Weight loss or loss of appetite
4. Long term fever
However, many of kidney cancers are detected on routine ultrasonograms done for other purposes. Thus, regular ultrasonogram testing can pick up these tumors very early.

What are the diagnostic tests?
Contrast enhanced computed tomography (ct scan) of abdomen is the current gold-standard test to diagnose kidney tumors and evaluate for local spread. Chest x-ray or ct chest helps in diagnosing spread to lungs. Some blood tests will help to evaluate function of kidney and general health condition.

What are the best treatment modalities?
Surgical removal is the best treatment for kidney cancer. Chemotherapy or radiation therapy does not act on kidney cancer and hence are not used.
If the size is small, only the tumor is removed and rest of kidney can be preserved (nephron sparing surgery or partial nephrectomy).
For large tumors, the whole kidney, surrounding tissues and lymph nodes are removed totally (radical nephrectomy).
Both these surgeries can be done through keyholes (laparoscopy), thus reducing the pain and leading to faster recovery.
If the tumor has spread all over the body, then targeted therapy is given using specific drugs (usually in the form of tablets) like pazopanib, sunitinib, sorafenib etc.

How to follow-up after treatment?
Regular check up using ultrasonogram, ct scan, chest x-ray, renal function tests, liver function tests are needed to maintain good health.

Take home message:
Regular ultrasonograms after the age of 40 years (at least annually) can help in diagnosing these tumors and curing them 100%.
