Here are some tips for how to handle your back pain: -stop normal physical activity for the first few days. This will help relieve your symptoms and reduce any swelling in the area of the pain. -Apply heat or ice to the painful area. One good method is to use ice for the first 48 to 72 hours, and then use heat. Take pain relievers medicine. --While sleeping, try lying in a curled-up, fetal position with a pillow between your legs. If you usually sleep on your back, place a pillow or rolled towel under your knees to relieve pressure. -Do not perform activities that involve heavy lifting or twisting of your back for the first 6 weeks after the pain begins. -Avoid bending forward and sitting in a place for long time -Stretching and strengthening exercises are important. But, starting these exercises too soon after an injury can make your pain worse. -Some Physiotherapy treatment like IFT current can give you some relief . -A physical therapist can tell you when to begin stretching and strengthening exercises and how to do them.

Rotator Cuff Tears: What Are My Options?

Rotator cuff is the name given to four muscles and their tendons, which keep your arms in your shoulder sockets, allowing you to lift your arms and do day-to-day stuff like combing your hair, cutting vegetables etc.

The shoulder joint is a very important joint of the body and this is why any tear that happens in the rotator cuff can be devastating.

Causes

Tears can happen in rotator cuff due to a number of factors like injury and degeneration due to age. Tears that happen due to an injury to the shoulder are called acute. Degenerative tears are tears that happen due to degeneration of the tendon over time.

Common causes of degeneration are lack of proper blood circulation, which impedes the rotator cuff’s ability to repair itself. Repetitive stress is another cause where we stress the tendon by repeating the same shoulder motions again and again while playing games like tennis, cricket etc. and doing jobs that require us to extend our arms upwards a lot. Lifting heavy objects can also lead to tears.

Symptoms

Chronic shoulder and arm pain are good reasons for you to see your doctor. If you suffer from pain in your shoulder at rest, while sleeping on the affected shoulder or pain while lifting and lowering your arm, then there is no time to wait.

The doctor will do a physical examination by moving your arm in different directions. He may follow it up with X-rays and MRI to check how big the tear is in your rotator cuff.

Treatment

Once confirmed that you indeed have a rotator cuff tear, your doctor will start the treatment. The goal of any treatment is to reduce pain and make your shoulder joint work properly again.

Nonsurgical Treatments: 

Nonsurgical treatments include rest, using of a sling to protect the shoulder and modifying your daily activities. This means you may not be able to do activities that cause shoulder pain. Your doctor can also prescribe pain and anti-inflammatory medicines. Strengthening and physical therapy bring up the rear of this type of treatment.

What you must remember is that nonsurgical treatment relieves pain and improves function in the shoulder, but does not improve shoulder strength. The size of your tear may also increase over time.

Surgery: 

Surgery, which attaches the rotator cuff to your bone, is done if you suffer from continuous pain and symptoms over a year or so. If you have a large tear, conservative treatments will not work and surgery will have to be resorted to. 

If you have been suffering from chronic shoulder or arm pain, waiting and watching is definitely not an option. You must consult an orthopedic surgeon at the earliest. Remember, if treatment is not started soon enough, your rotator cuff tear will get larger and will require only surgery.

. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

Dear Lybrate user Do muscle streching exercise for legs & back with best advice of physiotherapist only. Take hot formentation. I wud like to suggest you one more thing that you must try to reduce your body weight. Over weight causes so many problems regarding to health as bad effects on viceral organs ,hypertension, diabetes,backache and joint pain, etc. Some dietary changes as add high fiber diet &juicy fresh fruits, fluids intake. Avoid spicy & junk food. Try to add in routein walking morning or evening start from fifteen min. Then progressivly increase day by day.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are ways to keep your uric acid value in body to low levels. General recommendations are: - decreasing intake of red meat, alcohol, rice in your diet and other purine rich sources - also increase your water intake so that uric acid is excreted from body well. - keeping your weight to normal levels will also help you. - intake of fresh fruits, citrus fruits and cherries has been implicated in reducing uric acid values in the body as well. Nowadays there is a medication by the name of febuxostat which helps in lowering uric acid levels in body but kindly check out that it suits your body. Most leg pain results from wear and tear, overuse, or injuries in joints or bones or in muscles, ligaments, tendons or other soft tissues. Leg pain can also be caused by blood clots, varicose veins or poor circulation. To relieve mild pain: stay off your leg as much as possible apply an ice pack or bag of frozen peas to the affected area for 15 to 20 minutes three times a day elevate your leg whenever you sit or lie down please go to a physiotherapist if: you have pain during or after walking you have swelling in both legs your pain gets worse your symptoms don't improve after a few days of home treatment you have painful varicose veins leg pain happens when your muscles are tired or dehydrated. Drink more water if you're prone to leg cramps. Also low blood levels of either calcium or magnesium directly cause cramps. Pleas check you calcium and magnesium levels.

My both arms and thumb fingers are always painful. The consulting ortho surgeon told me it is age factor. However he suggested Depomedrol injection to take locally. Please suggest. It might be arthritis. Do you suggest any medicine.

It could be sprain.or spondylosis which should be managed. You haven't mentioned any radiation of pain in leg. Seems sprain only. Bath with hot water. Apply herbal oil muscle relaxants. Drink turmeric in hot milk or water. Avoid pain killers. For further advise, may furnish more details.

Sir. I am 23 year old male. From last 2 days there is lot of gas formed in my body. I used eno 2-3 times but no relief also visited a doctor he gave medications but no improvement. This gas has induced a lot of high intensity pain in my legs specially in left leg. This pain is not constant it moves from thigh to feet. Sometimes I feel like screaming due to this. Use laxatives for 2-3 days and change your dietary habits. Use more fibre rich food, take prebiotic (curd), proper timings of your meals, go with food pyramid, proper hydration, water therapy. My bone density is -1. 5. I am around 60+. What do you sugest me about medicine? is it calcium with d3?

Welcome to lybrate, this is Dr. Hitin mathur from new delhi. You are suffering from osteopenia or low bone density. I would suggest that you should get your vitamin d levels done fasting and if the levels are low, you'll need supplements depending on the vitamin d levels. Besides that you can start tab osteophos 70 mg's once a week on empty stomach. Remember not to lie down for half an hour at least after taking this medication as it may cause acidity. This drug is meant to improve your bone density and is to be taken for 1 year. Calcium supplements will continue lifelong. Get your bone density repeated after 1 year.
