Sprained Thumb- What Causes It?

A sprained thumb is a painful condition caused due to an injury on the ligament of a thumb. A ligament is a soft tissue, which connects bones to each other at a joint. The ligament might get partially or fully torn due to extreme backward bending of thumb causing the ligament connecting the thumb and index finger to suffer injury.

The problem is quite common among basketball, rugby and football players. For this reason, it is known as a sports injury. Skiing may also cause extreme pressure to the thumb, causing a ligament tear. Injury due to skiing is called skier’s thumb.

The injury causes pain, bruising and tenderness. It can cause swelling on just the thumb or sometimes on the entire hand. In severe cases, weakness and instability in the region can be seen. A sprained thumb weakens the ability to pinch and grasp things. The patient will have trouble holding a glass, turning a knob or writing. An untreated injury may lead to joint stiffness, muscle weakness, scar tissue formation, instability of the thumb or even arthritis. It is strongly advised to visit a professional therapist for an early diagnosis and treatment.

Usually, an X-ray leads to an apt diagnosis of the injury. The doctor also ascertains damage to the bone and ligament and develops a treatment plan. Some ligament tears can be treated using a splint or cast. A chronic ligament tear might not heal and would require surgical intervention.

A partial or mild ligament tear can be managed through useful home remedies like putting an ice pack, bandage or splint. It is also advised to keep the thumb elevated and restrict its movement. A heat pack is beneficial as it can ease pain and relax the muscles. However, heat pack must be avoided during the first 48 hours of injury as it can lead to bleeding.

If there is a chronic injury and the ligament is completely torn, a surgery is needed. A surgery helps join a new ligament to the bone in case the torn ligament fails to heal. It also helps reconnect the injured ligament to the bone. Sometimes, there might be bone damage as bone fragments might get pulled away with the ligament. In such cases, bone fragments need to be put back in their position through surgery. After the surgery, a cast or splint is put till it heals completely.

An early visit to a physiotherapist has potential advantages. A therapist can fasten the healing process, help relieve pain and improve a person’s ability to resume daily tasks. Massages and strengthening exercises are initiated to avoid re-injury and instill strength in the injured region. Recovery After Hip Replacement Surgery - How Physiotherapy Can Help?

Physiotherapy plays a pivotal role in helping a person recover after a hip replacement surgery. It brings back the old flexibility and prevents blood clots in the area where the procedure has taken place. After the surgery team gives its green signal, a physiotherapist starts working with a patient for a full recovery.

Here is a list of some common exercises that a physiotherapist suggests after a hip replacement surgery:

  1. Walking: It is important to use the crutches for a time period of 4-5 weeks post the procedure. Until the surgeon or the physiotherapist advises against using the crutches, it makes sense to use it while walking. It makes sense to retain the normal walking posture by using the crutches rather than limping without it. A physiotherapist reviews the pressure exerted by the arm while holding a crutch, before advising the patient to get rid of the crutches.
  2. Increasing hip flexibility: This is a basic form of exercise where an individual lies on one side of the body with knees and hips bent. A pillow is placed in between the knees. While doing this, it should be ensured that both the feet is kept together and the back muscle is not twisted. The top knee should be slowly raised like an opening of the calm shell. This position should be maintained for 4-5 seconds.
  3. Strengthening the hips: The body should rest on the stomach. The buttocks should be tightened and one leg should be slowly lifted. This position should be maintained for a stretch of 3-5 seconds. The same should be repeated for both the legs.
  4. Strengthening the hip and the stomach muscle: The body should rest on the back with a leg bent. The stomach muscle should be tightened and the bent leg should be lifted carefully while ensuring that the bend does not supersede more than 90 degrees. While keeping the stomach muscle tight, the lifted leg should slowly come down to touch the surface. This act should be repeated for 5-6 times.
  5. Strengthening the thigh muscle: While sitting in a steady chair, an elastic loop should be wrapped around the ankles. The elastic loop should be stretched with one leg while keeping the other foot firmly rooted to the ground. The lifted foot should slowly come down to the normal position. The same act should be repeated for 4-5 times.
  6. Improving balance: An elastic loop should be tied to one leg of a table and the other end of the loop should be tied to one ankle. The table should be held on for maintaining balance. Maintaining an erect position, the tied up ankle should be stretched sideways. The position should be maintained for 3-5 seconds to return back to normal standing position.

Frozen Shoulder - Why Physiotherapy Is Important For It?

Frozen shoulder is a condition when somebody encounters continuous shoulder stiffness and pain. It could last for weeks. It is likely to occur when swelling increases around the shoulder which reduces inability to move or stretch properly. This ordinarily happens when somebody is recovering from an injury. Frozen shoulder at times happens when somebody is wearing a sling or a cast. It also occurs, if someone is recovering from surgery, experiencing joint pain or facing restraint of movement for other reasons.

A frozen shoulder can go on from a couple of months to as much as 3 years and the precise cause is unknown. Physiotherapy is often to treat this condition. It has been found that posture plays a huge part in your recovery from a frozen shoulder. The urgent goal is to get the shoulder join moving once again. The sooner this is done, the better are the odds of a quick recovery. Anti-inflammatory medicines, heat application and delicate stretching activities are performed with the assistance of a physiotherapist.

Physiotherapy is very crucial for a frozen shoulder since it is very difficult to get movement in the stiff joint without any exercise, stretching or other methods used by a physiotherapist. It is mostly crucial to get physiotherapy to:

  1. To reduce pain in the shoulder
  2. To increase the flexibility and strength of the shoulder
  3. To enhance movement of the shoulder.
  4. To increase endurance of the muscles

Physiotherapy treatment for a frozen shoulder depends on the stage you are on and is customized according to the needs of your condition. Physiotherapy treatments for a frozen shoulder include:

  1. Pain relief: Pain diminishing techniques including gentle shoulder movement exercises, muscle releases, needle therapy, dry needling and kinesiology taping for pain can help during the painful swelling phase.
  2. Defrosting: Shoulder mobilization and stretching are the best in order to achieve a full shoulder recovery. As your scope of movement increases, your physiotherapist will then be able to give you muscle strengthening exercises to control and keep up your recently found range of movement.
  3. Warm Up: Before beginning shoulder exercises for a frozen shoulder, try to warm up your shoulder keeping in mind the end goal. That is to help blood supply in the affected area and avoid future injuries. The best approach is to extend and warm up the shoulder by applying heat for 10 to 15 minutes, scrubbing down or showering with Epsom salt.

Physiotherapy does not bring out immediate results, but gradually. Flexibility will increase gradually. One should keep in mind, not to stretch their shoulder too quickly at once. This can lead to a ligament tear. The exercises should be tender and gradual.

Lower Back Pain - Can Physiotherapy Help?

Low back pain is one of the most common medical problems and occurs due to injury associated with the nerves, bones, and muscles of the back. The pain can vary from a sharp shooting pain that happens at various intervals to a gnawing pain that can last for weeks on end. While the former can go away with some muscle relaxant, the latter may need a combination of medications and exercise.

Common reasons include improper sleeping posture, lifting, fracture, lack of exercise, disk injuries, arthritis, and many more. Most of these instances would lead to medium to long term pain that will not subside on its own. Physiotherapy has been proven to provide very good relief and render it pain-free.
The aim of physiotherapy is 3-fold - reduce pain, improve movement, and maintain the spinal function in the long run. Most often, the therapy sessions would last for about 4 weeks when the regular course of medications have not helped much.

The nerves, muscles, bones, and disks of the low back are supported by the abdominal muscles and the back muscles. The intent of the exercise is to strengthen these two sets of muscles so that the spine/low back per se is not very susceptible to injury.  The exercises would be a combination of stretching followed by strengthening and conditioning, all aimed at these two sets of muscles.

Some common exercises include bottom to heels stretch, knee rolls, back extensions, deep abdominal strengthening, and pelvic tilts.

In addition to being used a sole means of treatment, physiotherapy also is used in back pain to treat severe cases both pre and post surgically. What is corrected by surgery needs to be enhanced by the regular exercises as prescribed by the doctor.

One disclaimer that a doctor might want to give is that the physiotherapy may not be beneficial if exercises are not done properly or if the patients don't adhere to the scheduled prescription. The long-term success depends not just on getting the relief but on following the maintenance exercises that are prescribed. This is more important and helps avoid relapse of the back pain.  A good posture and improved core stability with ongoing exercises is the key to success of physiotherapy in low back pain. 
While most people consider low back pain as a chronic condition, it can be managed effectively. It is important to realize that there is no overnight cure and that it may not get better on its own. Physiotherapy is a very good intervention; however, it will depend on the severity of pain and how it affects the patient's lifestyle.

Usually, this kind of back pain is because of awkward posture while studying, so try to change the position every 1 hr. To reduce pain timely apply ice packs and do back muscle stretching that will help you.

Knee pain is one of the most common and painful ailments that can limit mobility. Injuries can result in cartilage or meniscal tears or sprains too. Obesity is another reason because of which knee pain is on the rise. Also, regular wear and tear of ligaments and tendons around the knees due to old age is another cause. These days, with reduced physical activities and sedentary lifestyles, the rate at which knees are wearing out has also become accelerated. Whatever the reason, a correct diagnosis followed by prompt treatment can help. Over the years, physiotherapy has been playing an important role in treating knee injuries. Whether to supplement the various drugs to reduce pain and inflammation or to avoid surgery, physiotherapy has helped many and its popularity is growing every day. Here are some ways that physiotherapy can help reduce pain.

  • Improve flexibility of tendons: The knee cap is an area where there are lot of tendons and ligaments, coordinating to enable movement. Careful exercising can ensure that the flexibility of tendons and ligaments is improved, thereby making your knee movement easier and less painful.
  • Improves blood supply: As we all know, proper movement requires good blood supply. Exercising with special focus on the knee ensures that blood supply is improved, and thereby the supporting ligaments and tendons function optimally.
  • Enhances the knee cap function: The knee cap is that portion which joins your thigh bone to your calf bones. This is prone to slip out of place, especially with arthritis and injuries. This can be painful and limit movement to a great degree. Focused exercises aimed at improving knee cap function will ensure that the knee cap has a wider range of function, and does not slip out of its socket easily.
  • Reduce rigidity: Once you start physiotherapy, you will feel your knees getting more flexible. The rigidity that had set in as a result of injuries and/or old age will also reduce over a period of time.
  • Slow down the wear and tear: When you start physiotherapy after knee damage, continuing the exercise routine, of course with expert guidance, can reduce the pace at which further damage might happen. A word of caution though. Be sure to consult an experienced physiotherapist before you begin. After every hour, take mini-breaks from your work desk and do some stretches to loosen your shoulder and neck muscles and shift position.

I have pain in my left leg muscle between knee and upper body. I think it is due to leg exercise I have done on saturday. Give me some solution for this.

I have a pull in a nerve behind my knee and pain in my leg muscles and bones while sleeping. Only on my left leg. I had this kind of pain in my right leg 2 years back. I did some knee exercises and also had dries 60K for a few months. It got better. Then stopped. When pain started again, I had it again for 3 months. It has been a month since I stopped. Now this new pain in my left leg has started. Is it a Vit D problem or something else?

First of all it's not a nerve pull, just muscle stiffness. It's not vitamin D problem, it may be because of electrolytes imbalance that only causing muscle cramps. Just try have plenty of water and do hamstring and calf muscle stretch. Just try this. Simultaneously meet physio and get evaluated, sometimes there may be other possibilities which need clear assessment.
