One of my friend is suffering from thyroid. What should she do to cure it. Her age is 50.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr K D, If you are referring to hypothyroidism, then she has to be treated with thyroxine tablets. Dose will depend on TSH levels and body weight. Plus should exercise regularly for 1 hour a day so as to maintain near normal weight. After starting the treatment periodically tsh levels should be checked in 6 to 8 weeks to see if any change in dosage is needed.

Lybrate-user, taking metformin while breast feeding the baby, produces no effects on baby's blood glucose level. A number of clinical studies have reported that metformin taken while breast feeding a baby does not affect the baby adversely at all. So you need not worry about it. Thanks for the question.

I got my thyroid results. It is hyperthyroidism. Doctor suggests to go for radioactive iodine. What to do? Are there any alternative treatments?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Lybrate-user, hyperthyroidism is treated in the following manner: 1) using anti thyroid drugs like methimazole, carbimazole or propylthiouracil. One of these drugs is given for a period of 18 to 24 months. This causes suppression of thyroid gland. Occasionally after 18 to 24 months normal function returns. Treatment is discontinued. However, in many cases suppression does not occur. Then other options are considered. 2) radio active iodine: this is given in a small dose, absorbed by thyroid gland. It destroys the secreting cells. Thyroid function is lost, hypothyroidism develops. Patient has to then take thyroxine hormone for life time. 3) surgical removal of the gland: this is another option employed in some patients. After surgery patient develops hypothyroidism and has to be treated by thyroxine for lifetime. Thanks.

Mr. lybrate-user, Do you mean blood glucose veries between 23 mg to 444 mg? Because 23 mg is a very low and dangerous level. That can lead to severe weakness even coma. It will be better if you give following details Fasting, PP current levels, HbA1c%, drugs being taken for diabetes with dosage, dietary pattern and extent of daily exercise. Based on those details I will be able to give some suggestions. Thanks.

Your diabetes and thyroid seems controlled for now but the requirement keeps changing to your body needs. So keep track of your sugars atleast once a week, and tsh atleast once in 3 months to know if your current doses are optimum for you.

Your blood sugar levels clearly show that you have developed t2dm, at a much earlier age. It would be better if you start treatment with insulin for first few months and control it better, later you can be shifted on to oral anti diabetics. The reason I am saying this is the glucotoxicity that harms the beta cells of pancreas can be better controlled and there is good possibility that beta cell activity revives rapidly with this treatment. Later insulin can be withdrawn and oral drugs can be substituted. Because of your younger age you have to be very careful in getting the sugar levels in good control. For any help you are welcome to ask me.

He is suffering with diabetes and don't want to leave sweets. How can I manage the things? Please help me.

MBBS, CCEBDM, Diploma in Diabetology

He is suffering with diabetes and don't want to leave sweets. How can I manage the things? Please help me.
the reason sweets should be avoided in diabetes is intake will increase blood sugar straight away. High blood sugar causes multiple problems affecting eyes (retina), small and large blood vessels, kidneys, heart, legs and feet (ulcers not healing, leading to amputation), severe neuropathic pain etc. Plus sweets also contain high fat content, increases caloric intake and may lead to weight increase. So if one has diabetes then there is no option but has to stop eating sweets. You have to convince him and impress that sweets will cause serious problem in the course of the disease. Another possibility is to use sucralose in stead of sugar while making sweets. But still gheee will be plenty.

Fatty liver is the deposition of excessive fat in the liver and it may be asymptomatic mostly. usually there is no treatment for the same except making some dietary modifications and excercise which may relieve it. Homoeopathy offers good solution for the same . Its done after identifying the cause of this condition by studying the body and the mind of the patient which is genetically determined. if you are interested opt for a homoeopathic consultation. usually fatty livers wont cause a problem but some cases may go for liver damage and cirrhosis

Will diabetes have any impact on our sexual life especially when it is under control.

Will diabetes have any impact on our sexual life especially when it is under control.
Dear lybrate-user, ideally it should not. Since you are already on insulin it means that you are absolutely normal in all other aspects like a non diabetic individual, provided your sugar levels are maintained well. Please start or continue self glucose monitoring and a 3 monthly hba1c record and do not worry.

I am 49 years old male having diabetes and high bp for the last 12 years. For the last 4 years my diabetes and bp have been more or less under control. I have been taking the following drugs for the last 4 years without any change: for diabetes: galvusmet 50/500 twice daily. Glimepride 2 mg twice daily. For high bp: olmat am 20 once a day. I regularly check my blood sugar and bp at home. My total cholesterol is under control. Mt hba1c reading last checked in nov 2014 was 7 (good control). I go for regular walking and jogging for at least 45 minutes 5 days a week. I am slightly overweight 83 kgs. I wish to know should I continue with the above medicines further or I need to change them, and if yes, what medicines should I take ?

It seems u r quite conscious for your health and taking medicines regularly and everything pretty under control. Still send me your last hba1c report, last 2 fbs pp2bs report, lipid profile report. As u r having diabetes and hypertension for 12 yrs, check your serum creatinine level and urine albumin level also, get an ecg and get fundus examination by ophthalmologist. Your wright is static or increased recently? any symptoms like east fatiguability, cold intolerance etc? and continue diet and exercise.
