Dear lybrate-user, Your father in law is suffering from Diabetic neuropathy which means that his nerve cells are affected by high sugar and it starts burning, tingling. The treatment is to control sugar very well, stop smoking, avoiding alcohol and taking appropriate treatment from your doctor. Good medicines are available to control these symptoms but may take 6 weeks to 4 months time Regards, Dr. sachdev.

I am a type 2 diabetic. For the last 10 yrs. Was on glycomet gp1 forte + valibo 0.3 till now. Another specialist has advised to use trajenta duo 2.5/500 + diamicron xr 60 mg. Is the to new medicine safer to use?

It is safe. The new medicine does not cause low sugar levels but it potentiates the effect of the other medicine. Hence, you should be careful to avoid low sugar levels.

Mr. Lybrate-user, a person with diabetes should take a restricted diet that gives exact calories based on his blood sugar levels, weight and extent of daily exercise. Few guidelines to be followed are: avoid sweets, sweetened aerated drinks, deep fried foods, junk foods, pastries, highly processed foods, high fat containing milk products and fructose corn syrup containing products. He should always use whole grain products and flour for chapati and roti. Should consume good deal of green leafy vegetables, salads and permitted fruits (100 g a day).

Diabetes affects all organs in the body ? Eyes, kidneys, nerves, reproductive system all are affected, In fact, from toe to hair. For diabetic control, patient and doctor should be in constant touch for regular follow up and advice. It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose ? SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me privately with all details like your daily eating pattern and medicines you take. I assure you a proper guidance... if you keep in touch.

Mr. Lybrate-user, diabetes mellitus, is a disease occurring due to either lack of insulin or resistance to its action or both. Plus over weight, inactivity and junk foods. This disease can only be controlled by taking regular medicine as prescribed by the doctor, plus dietary control and regular exercise for about an hour, so that normal weight is maintained. As regards the drug mentioned, is an ayurvedic (herbal) combination. Supposed to be helping in controlling diabetes. If you want you may take it. But do not stop your current treatment, otherwise blood sugar control will become haywire. I do not recommend it as a primary treatment of diabetes. World over there is no treatment that can cure diabetes. Thanks for the question.

from given data it is clear that both diabetes and high bp are not in control. Further you are overweight with a bmi of 27.77kg/m2. Therefore, we need to look at your over all treatment and if possible upgrade it. Please let me know how much time you spend on exercise daily and type of diet you have. It would be better if you come in a private conversation, so that I can give proper advise.

Lybrate-user, if you have no symptoms related to hypothyroidism and tsh levels are below 4 milli units/l, then you might reduce the dose of thyroxine to 12.5 mcg per day, then check tsh after 6 weeks, and see if that dose has been adequate in maintaining tsh level within limit. It is better not to discontinue the dose completely. Because hypothyroidism once developed is unlikely to disappear. Also check your tpo antibodies. Thanks.

I am having sugar thyroid and bp now I have problem of nerve pain in lower portion of leg I take revlin m is it ok or I need suggestions.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. V N lybrate-user, Long standing diabetes does affect peripheral nerves leading to neuropathy and pain. Primary need is to have a strict blood glucose control with FBS closer to 100 mg & PP 150 to 160 mg /100 ml & HbA1c% <7. If this is achieved there can be a faster improvement in the condition. Plus if pain is more, one may have to use certain drugs to control it along with strict blood glucose control. Thanks.
