I'm 48yrs man suffring with diabetes since two years. Bascically my mother, brother and one sister is also suffring. I've one more problem. Ten years ago I got operated both eyes due to retnal ditachment. I was high myopic. I lost my one eye due to glucoma. I've one working eye. I always avoid to do morning walk alone because of eyes problem. I would like to know two things. 1. How can we keep our children away from diabetes. 2. How can I bring my suger level towards normal level.

Mr Iqbal, If your sugar levels are as follows : Fasting <100mg and PP closer to 150 to 160 mg and HbA1c% <7, then it can be said that diabetes is under control. For controlling pyorrhea, maintain good oral hygiene, use mouth wash twice a day, brush teeth at least twice a day. Plus better to see a dental surgeon.

Do your hba1c. Hba1c will show you exact diabetes control. Fasting and post meals sugar will show particular day glucose reading. Fbs and ppbs is trailer of diabetes, hba1c is entire movie. Do your hba1c from srl or metropolis. Start lifestyle modifications and medications.

Can anyone please explain what is the meaning of hba1 level of diabetes patients.
Hba1c is glycosylated hemoglobin which indicates sugar levels over last 3 mths. Whereas fasting, post prandial sugar tests give idea about levels on that particular day only.

I have diabetes from age age of 32. Currently age is 47. I am not comfortable with drug called metformin and all other drugs doesn't work for me to control diabetes independently. Doing yoga and pranayam since 5 years has good effect on fitness but no effect in bringing diabetes down. Previously jal-dhoti was bringing diabetes down by 40 points now that also has no effect. Spending 1.30 hours in day behind exercise. Current fasting reading 170-190.

Switch to insulin particularly basal analogue like injection basalog, begin with 6 units in the morning, self monitor your sugar levels daily. Increase insulin dosage alternate days by 3 units till target level of sugar is achieved.

It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (self-monitoring of blood glucose? smbg). Though difficult, this can be done by a glucometer and keep a record. Get an idea of fasting sugar (fbs) and 2 hours after food (ppbs) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ insulin will be needed. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me privately with all details like your daily eating pattern and medicines you take. I assure you a proper guidance.

Mr. Lybrate-user, if you are not diabetic, then there may be a possibility urinary tract infection, so get a routine urine examination done and see if there is any infection. Infection increases frequency of urine. Also please check your blood sugar once again to rule out diabetes.

You need to continue medicine for at least 1.5 to 2 yr before we can think of stopping this. Regarding eye you will require clinical examination which will let us know if you require any treatment for same. You can see endocrinologist near you or can show if you r from delhi or around.

You should go for walk in the morning. Do little bit exercise. Diet should be based on ideal body weight, sugar level, extent of daily physical exercise, and you should avoid sweet, sugar, deepfried foods, junk foods, pastries, acrtated drink, fructose corn syrup containing products, highly processed foods, complex carbohydrates and fibre should be included in diet. You should avoid fasting and feasting. Use sufficient amount of ginger, onion, garlic, coriander, lemon, greenchilli, black paper, fenugreekseeds, cinnamon, bay leaves, in food iteams. Except fruit juice you should eat complete fruit because it contain fibers which is good for a diabetic.

Some people are genetically made who prduce more uric acid. Any thing than increase muscle breakdown increases uric acid. It can be endogenous that is increase breakdown of muscle due to disease or heavy exercise. Or exogenous due to heavy nonvegetarian meal. Or due to taking of food vegetarian dietcontaining high uric acid levels like palak, tomatoes, turnips etc.

I .Meal Planning

Eat smaller , more frequent meals
Control carbs
Ensure variety in diet - whole grain foods, fruits and vegetables daily
Limit fats - 3-4 tsp/day
Seasoning tips - use salt sparingly, avoid adding sugars.
II.Practical tips to make you meals healthier

Eat lots of fruits and vegetables- Except for few fruits like banana, mango, chickoo, jackfruit and grapes, and vegetables like potatoes,sweet potatoes , yam . ,both fruits and veggies are cornerstone of diabetes,- gives you vitamins, minerals and antioxidants.
Eat more whole grains- brown rice, ragi, bajra , jowar,wheat
Use healthy fats and oils- Choose- MUFA's - olive oil, canola oil , less often use PUFA- corn , sesame and safflower oil. Minimise SFA- high fat meat , dairy, coconut,
Choose heart healthy proteins- beans, peas and dals (good source of protein and fibre), Eat fish and shell fish 2-3times/weel, choose skinless, white chicken, nuts in moderation ( almonds , walnuts)
Select low fat dairy products- skim milk and its products like curd, paneer and low fat cheese

An egg white omelette with a bran paratha and fruit wedges
Oats with low fat milk and spirinkling nuts and fruits

Thank you for writing to us. Please note that your blood sugar is within normal limit. However I advise you to cut down cold-drinks/sweets in you diet if taking too often. Secondly do morning walk regularly and take a balance diet. For further answers get back to me. Thanks.

Some times uncontrolled diabetes and thyroid problems can result in to weight loss along with skin changes. See to that your sugars/hba1c/tsh reports are in acceptable range if not you need to adjust your dosage of medicines. Some times you get leg pains as complication of diabetes or due to nutritional deficiencies or vitamin deficiencies. You need to improve your diet and take vitamin supplements like b. Complex, vit e, vit d after consulting doctor.

Sunitaji, Do you mean to say, you have hypothyroidism? If so then only option is to start treatment with Thyroxine tablets daily. Dose will depend upon body weight and TSH levels. I am sure you are already on it. In addition you have to avoid soy beans, soy containing products, cabbage, cauliflower, broccoli, mustard, radish, peanuts and coffee. Consume more of green leafy vegetables, salads and fruits. Use whole grain flour for making chapati and roti. Exercise regularly for at least 1 hour a day, brisk walking id ideal. Plus you have to follow instructions on dosing that is to take thyroxine tablets early morning on empty stomach and nothing to be eaten for next 2 hours.

Your blood sugar is still high and tsh is also slightly high 1. Do regular aerobic exercise for 30 minutes daily like brisk walking 2. Reduce weight if overweight 3control diet a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato, a handful of walnut daily (akhrot) e) avoid white/polished rice, take unpolished rice like brown rice/wheat/jowar/ragi, moong and chana dal. F) avoid ghee/ butter./coconut oil/excess salt intake like papad/pickle. G) avoid red meat. Take fish/chicken. H) avoid alcohol and carbonated drinks like soft drink 4. Adhere to a strict diabetic diet by consulting dietitian 5. Monitor your fasting, pp blood sugar 3 monthly and hba1c six monthly
