Dear Mayank, Overthinking is not necessariliy a sign of OCD. However, assuming that your condition is indeed OCD, and it completely settles with fluvoxamine, the key thing to look for here is the duration of treatment. very often, the symptoms re-emerge because of premature stoppage of medicines. So first thing is to do a detailed interview and rectify these things. Psychotherapy (in the form of BT or CBT or CT) ca be done to make you self reliant and wean you off medications.But this will need considerable effort and time from your side as well as from the therapist. But that is really the way to go if you want to stop medicines. Take care!

I am 32 years old male having pain in my heel both feet. The pain would rise when I get up from my sleep that would make me difficult to walk. Eventually it would subside after an hour or 2. But the pain remains if you press your heel with your hands. At this point it is bearable. Also I have a pain in my lower back. Just 2 of the bones in the lower back. I am not sure which specialist should I consult because my friends say either visit a medicine Dr. Some has suggested an Orthopaedic and others a neurologist. Please suggest.

Requires detailed examination and investigation. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5 days, inform

I have spinal tb in l3 and l4. I have done 3 months of treatment and now have stopped this ethambutol medicine from 3 days and my throat is very sore and dry. I am having trouble in swallowing saliva! is it because of medicine or is it something else?

5 Winning Ways to Stop those Nasty Muscle Cramps
5 Ways to Stop Muscle Cramps

Muscle cramps are feared and loathed by all of us alike – they hurt, they slow us down and they just give us a helpless feeling! The most common cause of these cramps is premature fatigue which affects the reflex control of muscles. Here are 5 ways guaranteed to beat these troublesome muscle cramps:

1. Take rest
The most easy and sure-shot way to kill a muscle cramp is to take rest. Just rest your body, be at one place and the cramp will not last more than 3-4 minutes. Moving around will only cause the pain to be aggravated and eventually affect other areas as well.

2. Consume solutions rich in salt
Whenever you encounter a troublesome cramp, the first instinct is to reach for water. However, studies have shown that salt solutions are far more effective in countering cramps as compared to plain water. You can have pickle juice and similar drinks which have a high salt concentration.

3. Be physically active
The most common reason for muscle cramps is over-straining of the body. If you are planning to indulge in an extremely intensive physical activity, make sure to train well in advance. Also, keep the duration and intensity of the training at a similar level to that of the actual activity.

4. Keep your body hydrated
Dehydration is the root of a number of troublesome conditions, muscle cramps being one of them. Make sure to always carry a water bottle with you and drink a minimum of 7-8 glasses of water on a daily basis.

5. Indulge in light passive muscle stretching
When faced with a muscle cramp, in addition to resting, try and gently stretch the affected muscle. This continuous and light passive stretching lets the brain know that it can relax the muscle and help it return to normal.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you, if there is an associated muscle spasm, you are also advised to add a muscle relaxant for next 5-7 days - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute for 3-4 times in a day - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. follow advises given below 1.Avoid using computers continuously, take rest for 5 min after working for half an hour 2. In pain free interval do neck exercise (kindly learn exercise by registered Trainer) 3. you can use thin pillow while sleeping so that normal posture of neck can be maintained 4.Maintain proper posture of body while watching TV/ studying Kindly go for cervical X ray AP and lateral and review with reports Consult Physician/me for Further Management

Stretch calves get a custom made insole. Hams stretching. Sports taping. Hot fomentation. For leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day.

Hi Apply pranacharya restopain oil or prasarini oil on your affected part then give hot fomentation. Take maha rasnadi kwath 2-2 tsf twice a day. Take pranacharya vatari syrup and capsule twice a day. And agni tundi vati and maha yograj guggul 1-1 tab twice a day.

Actually pain at calf/leg region can be due to many reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you - serum potassium levels, esr and crp - also please take adequate calcium and vitamin d intake as per requirement and also supplement adequate amounts of vitamin b 12 as well.
