Hi, I was experiencing numbness in my extremities (only when sitting in awkward position or if I put pressure on them) since the last two years when I started gaining weight. I have gained almost 30 kg in the last 2 years from 62 to 92 kg. I had my blood sugar, thyroid and Nerve Conduction Velocity tested. All came out to be OK except vitamin b12 deficiency which was severe. I have some fatigue and short term memory problems. Please advice me on the best method to tackle this problem.

Hello sir being a dietitian I can help you in weight loss, for vitamin b 12 deficiency you can ask your Dr. About its injection, its very effective. Hello you need to understand the concept of weight loss, it requires cut down in calories (under the guidance of dietician) along with exercises like brisk walking or cycling or yoga and stretching 5 days a week for at-least 25-30 mins. Include foods like green tea, lemon water, jeera water, salad vegetables like onion, tomato, cucumber, kakri, cabbage and fruits like melon, watermelon, plums. Avoid fried foods, carbonated drinks, sweets and processed foods. Drink plenty of water. Take small and regular meals like 3 main meals and 2 side meals. Take chamomile tea before going to bed. Sleep for 7-9 hours per night. Don't skip breakfast. Thank you.

Advice... Avoid sitting Cross legged. Avoid Squatting- Quadriceps Exercises- Lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core Strengthening Exercise- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Hams Stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. repeat 10 times, twice a day. Sports Taping- stretch the tape from both ends and apply on the affected area Contrast Fomentation (Hot and Cold).

Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Inform after 5 days.

You are too young to suffer from cervical spondylosis. This treatment is being suggested on bases of the information provided. I would like to examine & investigate you in detail. Rule out Hypertension, Diabetes or other metabolic disorder. Any way it may be tried, --. Dolokind Plus (Mankind) [Aceclofenac 100 mg +Paracetamol 350 mg] 1 tab. OD & SOS. X 5 days. --. Caldikind plus (Mankind) 1 tab OD x 10 days. (You may need help of your local doctor to get these medicines.) --. Fomentation with warm water. Avoid direct flow of AC or Cooler. --. Sleep on a hard bed with soft bedding. --. Use no pillow under the head. --. Hello, Dr. I am a 21 year old male. I have back pain from 7-8 months in my back bone. I have done a x-ray and the Dr. said it is normal. And the pain didn't reduce its coming back after 2 or 3 days again. I have tried to jogging and swimming but still no effects. I have try ashwagandha power if I continue eating that then pain will be less if I stop it the pain is back. From past 2 days the pain is very much. Backache in young age is commonly due to 1. Muscle strain resulting from improper posture. 2. Intervertebral disc bulge or prolapse. X ray cannot diagnose soft tissue or muscle causes of back pain. MRI of lumbo sacral spine is needed for diagnosis. Follow these to get relief. 1. Take anti inflammatory drugs like Diclofenac or Etoricoxub with muscle relaxant combination round the clock for 7 days 2.Take rest for 3 days but avoid total bed rest. 4. Do back strengthening and stretching exercises daily once pain is relieved. 7.Apply analgesic cream over back followed by warm fomentation. 6. Keep pillow behind your knees at bedtime. While sleeping on sides keep pillow between knees. 7. Do not bend and lift weights. To lift anything from floor bend your knees and pick up. 8. If back pain is accompanied with radiating leg pain or tingling, numbness or weakness, consult immediately. Many cases of back pain do not repond to conservative treatment and require intervention like sacro iliac joint injection or epidural steroid injections to relieve pain. Treatment decision can be done only after detailed evaluation. Consult an interventional pain specialist in your city. For further queries consult again.

What You Should Know About Spondyloarthritis?

The bones and joints orchestrate very well with each other for smooth movements of various joints and the spine. However, due to various reasons, some identified and others unknown, there could be inflammation of the areas where the bones join the muscles leading to impaired movement. The range of motion could be reduced or it could be painful, thereby, limiting movement. Spondyloarthritis includes a group of inflammatory diseases that involves the bones and joints and enthuses which are areas where the ligaments and tendons attach to the bones. This causes conditions like plantar fasciitis, ankylosing spondylitis, psoriatic arthritis and reactive arthritis. The term spondyloarthritis is a blanket term to describe any of the above-mentioned conditions.

Read on to know a little more in detail about it:

Causes: While the exact etiology is not identified for sure, most patients with spondyloarthritis have an autosomal dominant gene (HLA-B27). Chronic infections could trigger the onset of the disease too.

Symptoms:

1. In all forms of spondyloarthritis, there is always inflammation of the spine, joints and entheses. If the fingers or toes are involved, there could be swelling leading to what are typically known as 'sausage digits'.

2. There is sacroiliitis which is inflammation of the joint between the pelvis and the spine, and spondylitis which is inflammation of the joints between the spines. 3. Family history and autosomal dominant gene.

Diagnosis:

A detailed medical history and physical examination form the cornerstone of a confirmed diagnosis. In addition, radiographic examination and diagnostic blood tests also could be done to confirm the diagnosis. Radiographs, especially, of the spine and the low back show typical changes including sacroilitis in suspected cases of spondyloarthritis. If the spine does not show these changes, but there is a high likelihood of spondyloarthritis, then MRI (magnetic resonance imaging) can be used to confirm the diagnosis.

As a final step, a blood test for the HLA-B27 gene can also be confirmatory. However, it is not true that a person with the gene will definitely develop the disease.

Treatment:

  1. The most important aspect of treatment is the use of pain medications and anti-inflammatory drugs (NSAIDs) which help manage pain and inflammation.
  2. Corticosteroid injections may sometimes be given directly into the joint spaces or the membranes to provide quick relief of these symptoms.
  3. ESWT / RSWT has a definite role to cure enthesopathy. Plantar fasciitis, and achillodynia can be cured with ESWT. Even small joints of hand and feet are amenable to treatment with RSWT. It is nonoperative and noninvasive treatment and has no side effects.
  4. Very rarely, stronger drugs such as methotrexate or sulfasalazine may be used.
  5. Another set of drugs known as TNF-blockers (tumor necrotizing factor blockers) are also shown to be highly effective in controlling the pain of the spine and joints.
  6. In very severe cases, surgery may be required to correct joint damages or correct severe degenerative changes.
  7. Exercise plays a very vital role in controlling the issue and improving range of motion. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to have a persistent back pain as a health issue. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like dynamic x rays of lower back and few blood tests for evaluation. Hi, Where is the pain exactly in leg? Usually flat-foot people do face this problem which can be corrected only by wearing specially designed arched-shoes or slippers. Never ever use flat slippers. Talk to your Orthopedician for further details.
