Here are symptoms, effects and procedure of cochlear implants.

Hi! I am Dr. Manish Prakash senior ENT consultant at Mayom Hospital, Gurgaon. Today we are going to talk about Cochlear Implants. Cochlear Implants are the boom for the deaf. They are primarily indicated for severe to a profound Sensorineural Hearing loss in children and adults.

Hearing loss is of two types Conductive Hearing loss and Sensorineural hearing loss.

Conductive Hearing Loss primarily means that the sound is not able to reach inside the inner ear into the cochlea and that is why it is called as Conductive Hearing Loss whereas Sensorineural hearing loss means that the sound is reaching into the cochlea but because to the damage in the cochlea hair it does not reach to the auditory nerve and it cannot reach to the brain.

Cochlear implants are mainly indicative for the treatment of Sensorineural hearing loss. Cochlear implants consists of two parts. One is internal component and the external component. Internal component is the surgically implantable electrode which is inserted by a surgery inside the cochlea which happens to reside behind the ear. The external part is a processor which is also having a microphone and a transmitting coil which is a house behind the ear and is supposed to capture all the sound and transmit it to the electrical signals. Thereby the signals enter to the cochlea via the electrode and these signals are transmitted through the auditory nerve to the brain. Cochlear implants is majorly for pre lingual children. Pre lingual mean before the development of the language and children who are born deaf and children under 8 years of age are good candidates for such kind of intervention and procedure and also children who are post lingual deaf which means that after the acquisition of language. Afterwards who become deaf post lingual children and adults are also good candidates for the cochlear implantation program.

1.Do a complete blood examination like CBC, ESR, Mantoux test, X ray chest, Sputum examination. 2.Consult Chest physician with report. 3.Do warm saline gurgling 3-4 times daily, 4.Avoid smoking if you smoke. 5.Consult me for further advice

Cochlear Implants - How Do They Work?

A cochlear implant is beneficial if you are deaf or have difficulties in hearing. Cochlear implants are not hearing aids which just raise the volume of the different sounds you hear; they are surgically implanted inside your head. The implant transmits impulses straight to the auditory nerves which send the signals to the brain.

Cochlear implants can help you with sounds such as telephones, alarms or doorbells ringing. You can even understand speech or listen to music again with the help of cochlear implants.

How Cochlear Implants Work?

Cochlear implants are for adults and children with sensorineural (hearing loss caused by damage to the inner ear) hearing loss.

The device consists of two parts:

  • The receiver-stimulator: this part is surgically implanted under the skin.

  • The speech processor: this part is placed externally, behind the ear.

The receiver-stimulator is placed first, and two weeks after the procedure, the speech-processor is installed. If there are sounds around you, the microphone on the speech processor will pick them up and transform them into electrical impulses. The receiver-stimulator receives these impulses through the electrodes inside them. The electrodes then send the electrical impulses to stimulate the auditory nerves, and ultimately, the sounds picked up will reach the brain.

Advantages Of Cochlear Implants-

Some advantages of cochlear implants include:

  1. Being able to hear at normal volumes.

  2. Understanding speech without the need to lip-read.

  3. Listening to and talking over the phone get easier.

  4. Being able to hear music better after the surgery.

  5. Picking up loud, medium and soft sounds.

  6. Modulating your own voice gets better, so others understand you easily.

Disadvantages/Risks Associated With Cochlear Implants-

The risks of cochlear implants are common surgical after-effects such as infection, bleeding and anaesthetic side-effects. Some other complications include:

  1. Alteration in sense of taste due to nerve injury.

  2. Paralysis or weakness on the face due to nerve damage.

  3. Dizziness.

  4. Tinnitus (Ringing in the ear).

  5. Fluid leakage around the brain.

  6. Device can be faulty or get infected.

  7. Meningitis (An infection in the brain).

People Who Can Get Cochlear Implant-

You are eligible to get a cochlear implant if you have:

  1. Profound to severe hearing loss.

  2. Problems with the hearing aid.

  3. A strong wish to improve your hearing.

  4. Even children with severe hearing loss in both the ears can get a cochlear implant.

Usually peripheral vertigo happens during sudden movement of head like when you look behind suddenly. Hence can check like that. If it still persists then consult for medication followed by certain excercises.

Hi, you may require to take your medicine for a day or two more. Do review treatment progress with your physician once. Simultaneously, I would advise the following for relief - (a) healthy & balanced diet with focus on fresh fruits/ leafy greens. Avoid yellow banana. Take a diet, with leafy greens including cabbage, cauliflower, broccoli, tomatoes etc. Fresh fruits including orange, amla, apples, pineapples, grapes etc help. Take hot carrot and tomato soup daily. (b) consume flesh foods in moderation. Try avoiding constipation at all costs. (c) take plenty of water daily and stay hydrated. (d) regularize life style, reduce stress/ strain. (e) protect yourself from intense heat/ cold air. (f) do not take extremely hot/ cold/ spicy/ sour drinks as well as foods. (g) warm saline gargling 2 - 3 times daily. (h) gargle made of tulsi, cinnamon, turmeric or fenugreek seeds and honey are also beneficial. (i) vitamin c tablets (limcee 500 mg) once daily before food for 7 days. Then after, take a tsp of good quality chyawanprash once daily for about 2 months. (j) avoid overcrowded places for the time being. Hope this helps. Take care and all the very best.
