Sciatica - How Physical Therapy Can Reduce Pain?

Sciatica is a very common neuropathic pain that is caused by damage to the sciatic nerve, a large nerve that runs from the lower back region down the end of each leg. It is the largest single nerve in the body.
This condition affects around 4 out of 10 people at some point of their lives. The nature of the pain varies from a mild ache to a sharp stinging sensation and may cause feelings of numbness and extreme weakness.

Causes of Sciatica
The causes for the onset of Sciatica are numerous. Some of them are as follows:

  • Slipped lumbar disc
  • Muscle inflammation
  • Internal bleeding
  • Pregnancy
  • Obesity
  • Awkward sleeping position
  • Uncomfortable footwear
  • Spinal injury

How can physical therapy help?
There are many ways of treating Sciatica, such as medication, acupuncture and surgery, which can provide relief. The most common and easy treatment method however is physical therapy. A progressive and controlled exercise routine that is specifically tailored to the nature of the condition in consultation with a physical therapist acts as an effective remedy in treating the problem. It alleviates pain and also eliminates the chances of relapse.

Physical therapy exercises used to treat Sciatica are of many types. They are a combination of core strengthening, muscular stretching and aerobic conditioning routines, which aim at correcting the root of the problem.

The different types of exercises employed to treat Sciatica include the following:

  1. Strengthening exercises: These aim at strengthening the spinal column and the pelvic region, which includes the hips and buttocks. They help in making the muscles and ligaments in this region more robust and allow for improved flexibility and support.Lunges and squats are some of the examples.
  2. Stretching exercises: These provide pain relief through improved agility and flexibility. The most important aspect of these routines is stretching and exercising the hamstring. Hamstring, stretches, calf stretches and tricep stretches are a part of stretching exercises.
  3. Aerobic conditioning: These are cardiovascular exercises of low intensity, which provide pain relief through the release of endorphins and the facilitation of fluid and nutrient exchange. Examples of these are swimming and brisk walking.

Try this for joint pains try this. Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. I am diabetic since 9 yr now taking zomelismet 50-500 and triglicomet 1 tablet at morning and triglicomet 1 at night also ecosprin' 75 at night. I want to know if I can control my sugar level with exercise I have fracture in my right leg 6 month back can not walk smoothly still. What type of exercise I can do. My fasting sugar is 135 and pp 235 around.

MD - Internal Medicine
General Physician, Bangalore
Hi, thanks for posting your query. Well yes you can control your sugars with exercise. But know one thing you are on quiet a high dose medication for diabetes so don't except that only with exercise you can control blood sugars and avoid taking medication. With exercise you requirement for medication comes down and still you will require some medication. Best exercise are brisk walking and jogging but since you have limitations doing it, you can try cycling or stepper exercise. Also to some extent upper body exercise like weight lifting etc.

Mr. Lybrate-user, continuous posture of any will build pressure over that paeticular part like the spine also. You try to change posture while studying&if possible lean to soft back/use any pillow, r keep a small cushion under the neck. Try to change postures often. Practice few asanas in yoga which help to relieve the back pain&tone up the spine.&also practice breathing exercises n pranayama which will relieve you from stress. Try to lie on back without pillow while sleeping.

If you have burning sensation while passing urine - then consult a urologist, if you have only back pain - then an orthopaedic surgeon.
