gfr 53 is low, means kidneys are affected due to long term diabetes. Apart from gliclazide what other medications are you taking? how are sugar levelsand hba1c%. If diabetes is in check nephropathy can also be controlled. Do you have high blood pressure, if so what are the drugs for that? few precautions restrict salt intake and do not eat more proteins. Please inform me otherdetails.

What are the symptoms of diabetes melitus & High blood pressure? Please help me. Give suggestion.
Diabetes mellitus most often produces very few recognisable symptoms, hence most people fail to identify that they have diabetes. But when fully grown it may produce following symptoms: excess urination, especially getting up at night to pass urine 2 or 3 times, which disturbs sleep, excess thirst, excess hunger, unexplained weight loss and tiredness. Blood sugar level in such situations shows hyperglycemia. High bp also very rarely causes any recognisable symptoms. Hence, mostly it is found out only on checking at clinic. Occasionally one may feel dizziness. Earlier there was feeling that high bp causes headache and nose bleeds. But this can happen in hypertensive crisis, since these conditions are familial, it better for a person to get blood sugar and bp check up done regularly, particularly after 30 to 40 years. Both these conditions can be prevented by regular exercise, weight maintenance, proper diet. In case of bp salt restriction is very essential. But if one has already developed both these conditions then appropriate drug treatment is a must, in addition to adapting a modified lifestyle.

lybrate-user, Thanks for the query. For treating diabetes and hypothyroidism only appropriate medications are from modern medicine. No other paties offer any treatment. A lot of false claims are made. In hypothyroidism supplementation of thyroxine hormone in adeuate dose and maintaining TSH around 3 to 4 mU/L is the only remedy. That will help in checking all the symptoms including weight gain. As regards diabetes control it is necessary to maintain fasting glucose < 100 mg, PP 150 to 170 mg & HbA1c% <6.8. Then only you will feel fit and healthy. Thanks.

Most probably your mother is suffering from oa of knee knee surgery-total knee replacement (tkr) is quite safe & gives uniformly good results, do ask for detailed treatment plan. Surgery should be done when patient is still active & will be able to take benefit of surgery. Do not ignore. It could be beginning of a serious problem. Kindly show me x rays before a final decision can be taken. She will be thoroughly investigated before surgery is under taken. Risk in surgery is less than the risk in crossing road.

Take your meds regularly. Don't skip them at all. Avoid foods with artificial colours flavours preservatives. Keep your bowels clean. Check your TSH after 4-6 weeks.

Since last 6 months my TSH is above 5 and below 10. T3/T4 being normal. Ultrasound of thyroid also normal. Last 15 days having chronic constipation, fatigue and cold feeling. Are the above due to hypothyroidism. Male 46, Weight 90 kgs, Height, 177 cms. BP 150/100 Medicines taking: cilnidipine 10 mg twice daily. Do I need further medical evaluation.

Mr. lybrate-user, thanks for the query. Usually with TSH < 10 mU/L, no treatment is advocated. However, if there are troublesome symptoms are present then treatment is given. In your case symptoms like constipation, fatigue and cold intolerance as well as weight increase are indicating that there is a need for treatment. Your BP is also not in good control (ideally with treatment it should be 120 to 130 / 70 to 80 mmHg). Thanks.

I am diabetic suffering from diabetes since last more than 10 years I am aged 66 years. Since last few months my post lunch counts are more than 300 & now it spiked to 400 post lunch. Should I start taking insulin injection at home, if so, how much In the past I used to take Daonil & now Glycomet 500,(3 times a day). Can u suggest to better option to control my sugar level near to normal?

Dear Lybrate User, *Take Giloya satva half spoon with a glass of water boil it for 7-8 minutes and drink in the morning and repeat the procedure at evening. To maintain proper blood glucose level you need following things to do- 1.Include more fibers in your diet. More soluble than insoluble. 2.Dont give food gap more than 2-3hrs between meals. 3.Drink min 2 ltrs of water during day time. Reduce water consumption after 6pm onwards. 4. Increase your protein intake specially veg proteins. 5.Do exercises regularly min for 1hrs min. 6.Increase your vegetables consumption. 7. Avoid simple sugar

u can do aerobic exercises and pranayams ...besides follow these tips ●Eat regular meals throughout the day at same time everyday..Avoid skipping meals ●Include high fibre foods at each meal e.g. legumes(beans/lentils ),nuts(almonds,walnut),eggs etc ●Take fruit as a healthy snack e.g. apples,pears,papaya,mausami,jamun,guava ●Try to avoid –grapes,mango,bananas,jackfruit(cheeku) ●Vegetables to be included in plenty-at least 2 servings per meal ●Reduce the fat intake,friedfoods,cakes,pastries and foods containing palm oil and coconut oil  ●Include healthy foods like daliya,oats ●Reduce salt intake in the diet to 1 tsp or less per day ●Limit intake of sugar and sugary foods like sweets

Once you have diabetes may be you have to learn to live with it life long .(So far we have not found Cure for diabetes -however in near future we may find some answers) You may not require to take medicines life long if you are over weight and you reduce your weight to near normal .In that case you can get rid of medicines and can be managed on life style modifications along with suitable exercise. Its always very important to understand diabetes properly so that you can get rid of fear and make friendship with diabetes. You can always be person with Diabetes but never be patient with diabetes.

My wife suffering to thyroid and we are met many doctors but they are not controlled thyroid. every time report are updown. Pls assist me.

Mr. lybrate-user, Thanks for the query. Your information is very general, so difficult to comment. There is no mention whether she has hypo or hyperthyroidism. It is better if full details are mentioned. Please understand whichever thyroid condition it may be, treatment has to be continued for lifetime. Particularly in hypothyroidism she has to take prescribed amount of levothyroxine tablets daily without fail for her lifetime. If dosage is frequently altered or drug is stopped in between, then TSH levels will go up. All the symptoms reappear. Plus all the instructions regarding taking of the tablet that is - early morning on empty stomach, nothing there after for 1 to 1 and half hour, should be followed. She has to restrict her diet. Avoid soy beans, soy containing products and use whole grain flour for roti and chapati. Stop taking foods made from maida. If you have any doubts please come back. It is better if you come in a private conversation with all details like TSH, T4 & T3 levels and current dosage, her weight and height and symptoms if any, then I will be able to guide specifically and help in controlling the condition. Thyroid is a hormonal organ. Hormones are signals in our body which effect various vital processes of the body. You can link them to traffic lights for the smooth and safe flow of traffic in the body. Unfortunately due to several known and unknown reasons, some hormonal organs like thyroid start to underperform or over perform leading to hypo or hyper function respectively. When we prescribe medicine for hypothyroid patients - we just provide supplementation of deficient hormone in the body. We currently have no medicine to cure the thyroid gland - we just have medicine to fulfil the hormone that was supposed to be manufactured by the thyroid gland. Since the requirement is daily - that is why the medicine is given daily. Since the requirement is lifelong- thats why the medicine is lifelong!

Avoid regular tea and coffee,take green tea. 2.Avoid milk and milk products like curd,cheese,ice-creams etc. 3.Use omega-3 rich rice bran oil or olive oil 4 Take more of green leafy and salad veges. 5.Include fiber in your diet. 6.Regularize meal time 7.Take small and frequent meals. 8.Avoid sweet fruits instead take dry fruits like almond and cashew.

Dear doctor I am diabetic patient at present my sugar level after food 252. Know I am taking GLYCOMET-GP 2mg Forte tab. And some time my two foots very pain and pinching some times under the foot burning. And I am taking PREGABALIN 75mg also. Mr. Lybrate-user, uncontrolled diabetes, does affect various organs and a number of functions in our body. Peripheral neuropathy is one of those complications of diabetes. This leads to pain and burning sensation in feet as well as burning sensation. The drug pragabalin is quite effective for reducing the pain. But prime need is strict blood sugar control - fasting closer to 100 mg and pp 150 to 160 mg, hba1c%<7. If this is not achieved other drugs may not show much benefit. So see tha blood sugar levels are controlled better.

I have hypothriod last 2 yrs. My first tsh is 20, then 14, then 6, then 8. I asked my doc he said ups and down are normal and not increase the power of tablet I did this. I take thyronorm 50 first six months but no relief so my doc increase its power. Now I take 62.5 mg thyronorm. My recent tsh is 23.82 which is more than my first tsh. I am so scared now. Pls suggest me what to do I am 29 years unmarried girl Guide me.

In fact you should have started with 75 mcg per day in the beginning only. However now you will have take 87.5 mcg and test the tsh after 6 weeks, to if tsh has come down. On treatment tsh should be around 3 to 4 milli units/ml, to control all the symptoms. Thanks for the question.

This is far sure it is the inner heat creating problem .check it .when BP high we have a treat mix of unsaturated fats DHA and right mix of proteins CHO and fibers .
