Hi, there are a few suggestions for you to improve your child's health. Give him milk about 300ml per day plus 1milk cereal like suji kheer, dalia, rice kheer or fruit custard. Give him variety of fruits but specially include banana which is a wholesome diet for kids. Give him dals, boiled chana, rajma rice or rajma tikki, moong dal and chana sprouts, dal parantha, chana dal or moong dal pakodis. Use soya in cutlets or add soya atta to wheat atta or suji to make pancakes. You can make idlis with mixed grains and added vegs veg dalia pulav is a complete healthy meal for kids. I feel it is important to have a nutritious meal and feel physically and mentally active. Your son's weight is ok for his age. With regular nutritional foods, it would further improve plus will ensure his overall physical and mental development.

Lifestyle Changes to Manage Asthma Better

MD PULMONARY, DTCD
Pulmonologist, Faridabad
Lifestyle Changes to Manage Asthma Better

Asthma is an inflammatory disease, which affects the airways of your lungs (respiratory tract that pumps oxygen into the lungs). It is long term in nature. It is characterized by shortness of breath, coughing and wheezing. It occurs few times a day or few times a week depending on the person it affects. Sometimes it worsens during the night.

What triggers Asthma?

  1. Airborne particles such as dust mites, mold spores, pollen and cockroach waste.
  2. Infections in the respiratory system such as common cold.
  3. Cold air can also trigger asthma.
  4. Physical or outdoor activities which make you pant such as running, skipping, jogging and jumping can cause shortness of breath, thus triggering asthma.
  5. Pollutants and irritants in air such as smoke.
  6. Some medications can also trigger asthma.
  7. Stress and nervous attacks (panic attacks) also trigger asthma.

How to manage asthma better by changing your lifestyle:

  1. Try to avoid keeping animals in your house such as dogs, cats and rabbits. Many times animal fur, waste, urine and saliva contains particles which can cause allergic reactions and make you cough uncontrollably. This in turn triggers asthma attacks. If you have pets try keeping them away from your bedroom.
  2. Dust mites also cause allergies which trigger asthma. So wash your carpets, rugs, bed-linen and pillowcases in hot water.
  3. Bugs and insects such as cockroaches are not only unhygienic, but can also trigger asthma. Cockroach droppings near your bed, kitchen and food are dangerous and you should call an exterminator to get rid of these bugs.
  4. Sometimes, heavy fragrance also aggravates your respiratory tract and triggers shortness of breath. Avoid using heavy perfumes, deodorants and room fresheners.
  5. Pollution also triggers asthma. Dust particles come out from the vehicles and mix with the air. If this air is inhaled, it might trigger uncontrollable asthma. So try to cover your mouth with a cloth when you step out of your house.

Asthma can be very severe and can bring many hurdles in your day to day activities. Changing your lifestyle and certain habits can reduce asthma to a great extent. If you wish to discuss about any specific problem, you can consult a pulmonologist.

Fever always has a cause for it. You can treat fever with paracetamol and meftal p. If the fever still does not come down, I suggest giving tapid sponging until the fever is less than 100 f. Sponging is the most effective treatment of fever! however as I said earlier, we should find out the cause of fever, if infection, than the focus of infection and treat accordingly. If it is due to viral causes, supportive treatment for a couple of days would be sufficient. Keep the baby clean, prevent the baby from rapid climate changes, and make her/him drink lots of fluids. Your baby will do well. Feel free to contact anytime!

I suggest you to stop or avoid formula milk as it can not replace the breast milk in its nutritional value. Try to breast feed your baby completely atleast for 6 months of age and later shift to formula feeds. Stress and other factors contribute to lesser production of breast milk so avoid stress and let your baby suckle to your breast as much as possible as it will stimulate milk production. Consult your gynaecologist if possible so that she can prescribe you some medication to increase your milk production.

She has child asthma what are precautions and consult about good treatment for my daughter.

MD Paediatrics, MBBS
Pediatrician, Hyderabad
As a parent of asthmatic child there are few precautions to be followed apart from giving medications. Avoid child getting exposed to very cold air/breeze, avoid dust (roads, construction areas, while cleaning home) avoid wollen clothes or hairy cloths or beddings, avoid artificial colours, flavours. Be extra cautious in winter and rainy seasons. Always keep inhalers and medications related to asthma even if you are travelling.

General Physician,
Vitamin d deficiency historically has been defined as a blood level of 25hydroxyvitamind below 25nmol/l. There is scientific debate about the optimal vitamin d blood level. Current practice in the uk, as recommended by the british paediatric and adolescent bone group, is to continue to use that as the defined level of deficiency, and to define? insufficiency? as between 25 and 50nmol/l. Some laboratories and authorities use higher levels but this current practice is based on robust evidence of benefits to bone health when levels are more than 50nmol/l. Vitamin d deficiency can cause seizures and cardiomyopathy in infants, rickets and poor growth in children and muscle weakness at any age. Need to discuss more details, you are free to ask privately in ask privately option with, birth and development history and complaints which made you test for d levels. Mention your name and mail id with phone no. I am thinking of writing an article on vitamin d deficiency among other deficiency states and diseases. You can read them in health tips there is already an article in diseases category by me.

Hello Dr. I would like to relactate my baby who is now 7 months. Since, 2 1/2 months I stopped breast feeding. Even if he does not want to latch again, I would be very glad if I could give him breast milk during feeds. I really want to breast feed again. It's my third baby. Due to lack of information I quit breast feeding very early. My milk has dried up, but I'm trying to pump, using spectra s1 double electric pump, to bring my supply back. But, still nothing. Not even a drop. Hope you can help me!

herbs: fenugreek, blessed thistle, and ordinary oatmeal can all be helpful in increasing supply - drugs: as a last resort, Reglan and Domperidone are prescription drugs which are sometimes used off-label to increase milk supply. Both have additional side-effects and are not right for every mom, so speak to your healthcare provider before taking either one.

Oppositional Defiant Disorder in Kids - How to Deal with it ?

MBBS, DNB (Pediatrics), MD - Paediatrics, Diploma in Child Health (DCH)
Pediatrician, Gurgaon
Oppositional Defiant Disorder in Kids - How to Deal with it ?

A child’s tantrums, especially during teens, are quite common. However, there could be some children who could be exhibiting an extreme version of these symptoms. This is known as oppositional defiant disorder.
Children with this disorder become easily irritable, angry, argumentative, defiant and feel vindictive against most elders (parents, teachers, and others). While this is something very common and can be ignored to be a part of teenage tantrums, the issue is when these symptoms do not seem to end. If they persist for beyond 6 months, it is time to worry. These may then begin to interfere with their daily activities including schooling, where they may not be easy for the teacher to manage.

Diagnosis of ODD: With the changing behaviour of teenagers, it is often difficult to pinpoint and say there is ODD. However, some guidelines for diagnosis are listed below. Angry/irritable, argumentative, defiant and vindictive. If these symptoms are seen for more than 6 months with no inducing reason, happens with non-siblings, and is affecting learning and playing, it is highly likely the child has ODD.

These symptoms can occur at home, at school, or in other settings – seen respectively in one, two, or more settings. Some of the symptoms are listed below.

  1. Repeated temper tantrums
  2. Anger bursts, swearing, using obscene language
  3. Extremely argumentative, especially with people in authority (teachers, parents, etc.)
  4. Annoying others and getting annoyed easily
  5. Noncompliance to rules and regulations at school and institutions
  6. Defending one’s mistakes and blaming others for it
  7. These result in poor academic performance, antisocial behavior, substance abuse, and higher suicidal tendencies.

Treatment

Treatment depends on the presenting symptoms, the age of the child, and supportive care available. The child should be able to actively take part in psychotherapy to reap good benefits. It would otherwise be a task with no results.

  1. Psychotherapy will help the child improve its cope and express and control anger. This also improves problem-solving skills.
  2. Cognitive-behavioural therapy tries to mould the behaviour.
  3. If required, the parents also would be involved to improve family’s involvement in the treatment. Caretakers are given special training if required so that they can support in long-term medical care.
  4. The child also needs to be trained for appropriate behaviour under different circumstances.
  5. Rewards for positive behaviour and punishments for negative behaviour are useful ways.

Prevention: Early identification can help in minimizing distress to the family and help in the early arrest of the disease. The family is also taught basic and simple steps which can help in supporting therapy. Early rejection at school and loss of learning, can happen which can be managed with early intervention. A nurturing and supportive family can help manage the child very well. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.
