When Is Knee Replacement Required?

Knee replacement surgery- which is also known as knee arthroplasty, helps to relieve the pain in the knee thereby restoring the knee function. During this procedure, a surgeon cuts the damaged cartilage and bone from the kneecap, thighbone, and shinbone and replaces the same with an artificial joint comprising of high-grade plastics, alloys, and polymers. A patient/surgeon has the liberty to choose the type of knee joint he/she prefers based on factors such as weight, age, overall health etc. 

Signs that might indicate a knee replacement surgery -

  1. The pain in the knee keeps coming back all the time. 
  2. The knee aches after a brisk walk or during an ongoing exercise session. 
  3. The mobility of a person has gradually decreased over time as compared to last few years. 
  4. A supporting cane or medication isn’t delivering the desired support or pain relief. 
  5. The knee tends to stiffen when a person sits for a long time. Example- witting through a movie or sitting in the car for too long proves to be a major hurdle. 
  6. The pain proves to be a major problem that intervenes with sleep. 
  7. The knee motion significantly decreases once a person tries to bend the knee. 
  8. Climbing stairs turns out to be a major hurdle. 
  9. Getting out of bathtubs becomes a major challenge. 
  10. The knee becomes and stays stiff or at least half an hour after getting up from bed in the morning. If the stiffness lasts for more than 60 minutes, it is a sign of inflammatory disease. 

How is a knee replacement surgery performed? 

  1. Knee replacement surgery requires overnight stay in the hospital. 
  2. It involves a general or spinal anesthesia
  3. Once the skin over the surgical site is cleansed with an antiseptic, the doctor makes an incision and takes out the damaged surface of the knee joint. 
  4. The knee joint is then resurfaced with a prosthesis made up of plastic and metal. 

A cemented prosthesis is most commonly used during these days. The prosthesis contains 3 components: 

  1. Tibial component 
  2. Femoral component 
  3. Patellar component 

What are the advantages of knee replacement surgery? 

  1. The biggest advantage of this surgery is the freedom from constant and throbbing pain. 
  2. The fact that surgery can increase the mobility also proves to be a blessing for the general condition of the patient.

Since you r sitting for long tym you r having back m neck pain. Make a habit of doing basic exs every day. N after every half an hour get up n walk a bit, it will help you a lot. N follow proper ergonomics sitting wit proper support. Exs lying straight bend both knees pull them up towards your stomach hold for 5counts m leave. Next with knees bend rotate your legs sideways. Dan wit knee bend lift your pelvis hold for 5counts 10 reps. Lie on your stomach hands towards your face n try n cum up like bhujangasan. 10 reps. Dan again on stomach n lift one leg straight up hold for 5counts alternate legs. Each 5 leg 5tyms.

I had undergone ACL reconstruction surgery and today (21.11.2017) is the 10th day. Presently I face the following problems. (1) Swelling &pain in the knee (mild) and swelling in the foot (mild). (2) Knuckle occurs (with a popping sound and pain) when rising up from the bed (bending the knee from a fully stretched position. The knee is also very stiff when rising up. Hi. 1) Pain will be there for some time and it will reduce gradually. Swelling over the knees and foot will take some time to go as it is the dependent edema. Keep leg elevated over pillow and apply ice pack. 2) Physiotherapy should be done as the Quadriceps are now not firing so do your Quadriceps stretching and static strengthening exercises. 3) Yes definitely you should do exercises or else swelling may remain and your muscles will get wasted.

Anterior Cruciate Ligament Injury - Know Everything About It!

In general, anterior cruciate ligament injuries, or ACL injuries, are understood to be tears in any of the several knee ligaments joining the upper leg bone and the lower leg bone. This can vary from minor injuries, such as small ligament tears, to more serious cases, like complete tears or when the ligament and one of the bones gets displaced from the other. These injuries usually occur during sports activities, like soccer, basketball, football, gymnastics, tennis, volleyball, etc. An untreated ACL injury can lead to a condition called an ACL deficiency. This can cause a lot of problems with regard to knee movements as the bones may rub against each other, causing a lot of pain. In some cases, the cartilage covering the ends of the bones may get damaged, eventually tearing and trapping the cushion pads supporting the knee joints.

Symptoms

Signs and symptoms of anterior cruciate ligament may include:

  1. You are unable to move properly.
  2. You experience immense pain while walking up and down stairs.
  3. You are unable to engage much in physical activities.
  4. Your knee begins to swell gradually.
  5. You can sense a loud popping sensation when moving your knee.
  6. You experience feelings of instability.

Causes
Anterior cruciate ligament injuries generally occur during sports. Here are some common possible instances that might happen during a game:

  1. Sudden arrest in motion
  2. An unsteady landing from a jump
  3. Direct blows to the knee from tackles
  4. Sudden shifts in direction

Treatment
Treatment of ACL injuries normally involves the following:

  1. Immediate medical attention along with intense first aid care
  2. Sincere adherence to the R.I.C.E model of self care -
    • Rest: Proper rest for a couple of weeks or months depending upon the severity of the injury
    • Ice: Using ice packs to help cool the nerves
    • Compression: Ice treatment further helps in compression
    • Elevation: When lying down in bed, it is strongly recommended that you keep your affected leg at a slightly higher position, through the use of pillows or cushions
  3. Success rehabilitative therapy for several weeks
  4. Anterior cruciate ligament surgery or reconstruction

Hi Priyanka Gawde Madam I am in accounting profession so basically whole day sitting job. But in Morning I am doing exercise half an hour for frozen shoulder & half an hour stretching & some yoga for weight loss. In whole day I consume 3 cup of tea having less sugar. Breakfast and lunch taking together in that sometimes taking kanda poha, upma sometimes biscuits with tea. In noon taking fruits one bowl like watermelon, Papita, Orange, Pineapple etc. In evening taking an Apple sometimes if I feel more hunger then eat Pani Puri at night 3 small chapati, pulses and vegetables (I am not eating all vegetables like only few vegetables to eat) one big spoon rice (Not every time with dal avoiding dal instead of that using specs) then went for walking every night after dinner walking 4 to 5 kilometres daily and also walking nearly 2 kilometres during the day and consuming 8 to 10 glasses of water.

Hello, As you have mentioned, having 3 cups of tea is fine, better to use double toned milk. Secondly, the first rule for weight loss happens to be small and frequent meals and no skipping of meals. Skipping meals is very dangerous. So in place of a heavy meal, cut it down to half a quantity if necessary, but never skip and/or combine your meal specially breakfast, lunch and dinner. You can always have lesser quantity for dinner. Also, tea and biscuits can be your snacks item, not a full meal. Let me tell you what happens when we do something like this in a regular pattern, our body goes into energy saving mode and starts storing energy thinking that it will help to generate energy during crisis period (skipping meals and/or avoiding food, ie, starving intentionally). This becomes the main basis for putting on weight (or getting fat). Coming on to the next thing, papita, apple, bananas, mangoes (not more than 2 a day) should be avoided, because these fruits basically helps in putting on weight not in weight loss. Daal & daal products, soy bean, paneer and non-veg food items is better be avoided during night time and/or in dinner. You can incorporate tender coconut water, more pulses, fruits like pomegranates, kiwis in your diet. Also, always try to have the seasonal fruits and vegetables, reason behind in that particular season our body does need those nutrients specifically. Like most of the summer fruits and vegetables tends to detoxify our body and maintain a stable body temperature in the hot climate. Coming up to the next issue, it is advisable not to lay down immediately after dinner but we should not strain our body with the amount of walking or exercise as in evening walk or morning walk. It may be due to flatulance. Take hot water in small cup in the morning and evening. Take light diet and apply Ayurvedic massage oil on your back. Don't worry it is not a big problem.

My mom age is 54. She suffered from severe joint pain in fingers and body leg pain. We went to rheumatologist. Doctor suggest tablet. She took tab for 8 months. But crp level is 17. Only reduce 3 point 20 to 17. Also she suffer body pain, back pain, feel vomit and stomach problem sometimes. Sometimes feel OK. How to reduce crp level and how long continue tablets. Is there any side effect. Other than crp level. Every other report is fine. No hormone, diabetes etc.

Hi there! Your mother probably suffers from and is being treated for Rheumatoid Arthritis. As a rule, we do not chase CRP levels for a patient. We chase the symptoms and aigns i.e. The problems actually faced by the patient. CRP levels might or might not come down, with or without medicines. But there is nothing wrong with it. I suggest you stop getting repeated CRP levels assessed at your level. You'll be much happier. Let CRP levels be assessed, as and when required, by your Doctor. If clinically o/ symptomatically, your mother is better. That's the best thing. If she still has problems of pain, stiffness, etc. Her medication might have to be increased. Consult as required. This is a condition known as Plantar fasciitis. You should do contrast foot bath with alternate hot and cold water fomentation. Apply anti-inflammatory gel. Avoid running, jogging, jumping. Use silicone Heel cushion in shoes.

• Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. • Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day.

• Quadriceps strengthening exercises- • Quad Clenches: Lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, Hold for 3 secs. Repeat 10 times twice a day. • Short Arcs: Lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. • Straight Leg Raise: Lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

• Apply Hot Fomentation twice daily • . Avoid bending in front. • Postural Correction- Sit Tall, Walk Tall. • Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times.

What Are The Possible Causes Of Shoulder Pain?

Anyone can get shoulder pain, and the cause might be simple to serious. You may get shoulder pain because of minor injury, carrying too much weight on the shoulders, poor posture, and any underlying health condition. Sometimes the pain comes and goes on its own without any need of taking medicines, but at times, it becomes difficult to treat them.

Conditions which may lead to shoulder pain may include rotator cuff disorders, instability of shoulders, frozen shoulder, broken arm, broken collarbone, and acromioclavicular joint problems.

The following are a few common causes-

1. Rotator cuff disorder:

A rotator cuff is a group of tendons and muscles which surround the shoulder joint. Tendons are hard and rubbery cords that link muscles to bones. The rotator cuff holds the joint in the right position so that it can move in a desired and controlled way. There are different types of rotator cuff disorder, and each of them leads to various symptoms. But common aspects of the problem include:

  • Pain which starts worsening during activities involving the arm above shoulder level. This may include brushing the hair.
  • Pain occurring on the front side of the shoulder which tends to worsen during the night
  • Ache that occurs on the front and side of the shoulder

Different types of rotator cuff disorders and their characteristics are as follows:

  • Tendonitis and bursitis- Tendonitis refers to the swelling of tendon while bursitis is inflammation of the bursa. Bursa is the fluid-filled sacs that are typically placed on the joints and in between bones and tendons.
  • Tears- A torn tendon or muscle cause possible weakness and severe pain in the arm and shoulder. Some people also feel a popping sensation when they move the shoulder.

2. Shoulder instability

The shoulder is a type of ball and socket joint and shoulder instability takes place when the ball part of the shoulder fails to fit in correctly in the socket. It can range from catching or slipping feeling to total shoulder dislocation where the ball entirely comes out of the socket.

3. Frozen shoulder

Also referred to as adhesive capsulitis, frozen shoulder is a painful and persistent stiffness present in the shoulder joint. This makes it very difficult for carrying out the full range of normal shoulder movements. Frozen shoulder takes place when there is swallowing, thickening, and tightening of the flexible tissue which surrounds the shoulder joint. This doesn’t allow enough space for the upper arm bone or humerus in the shoulder joint and makes shoulder movement painful and stiff. A person affected with frozen shoulder may find it difficult to carry out the daily chores such as driving, exercising, and sleeping in a comfortable position. Some people may find it absolutely impossible to move their shoulders at all. The symptoms of frozen shoulder can vary to a great extent, but it starts advancing at a gradual pace.

When left untreated, shoulder pain may or may not worsen. When it does worsen it is important to see your doctor.
