Graves' Disease - Things You Should Know About It

The body has multiple chemicals called hormones, which regulate many of its functions. Increased or decreased levels of these hormones affect various functions including metabolism, growth and sexual functions. Thyroxin produced by the thyroid (situated in the front of the neck) is one such hormone, which has a significant role to play in metabolism.

Graves’ disease is one of the main causes of hyperthyroidism, where there is excessive production of thyroid hormones. Graves’ disease is an autoimmune disorder, wherein the body reacts against its own tissues. It is very common in women than men, especially after 20 years of age. Family history also has a strong correlation in developing Graves’ disease. The high levels of thyroid hormone increase the rate of metabolism, thereby altering weight, mental energy levels, physical stamina, and also mood.

Thyroid hormone is related to metabolism and more amount of it lead to higher metabolism. This causes the following symptoms.

  1. Intolerance to heat
  2. Excessive sweating
  3. Nervousness and anxiety
  4. Inability sleep, as the mind is always excited
  5. Increased appetite (sometimes despite weight loss) due to higher metabolism
  6. Irregular menstrual cycles
  7. Palpitations (rapid, loud heartbeat)
  8. Irregular heartbeats (tachyarrhythmia)
  9. Fine tremors of the extended arms
  10. Breast enlargement in men, known as gynecomastia
  11. Extreme moodiness, causing irritability and anger
  12. Inability to focus and concentrate
  13. General fatigue and shortness of breath with any exertion
  14. Increased frequency of bowel movements

In addition, the eye symptoms are quite diagnostic including:

  1. Protrusion of the eyes (exophthalmos), giving an impression that they are going to fall out
  2. Double vision
  3. Excessive tearing
  4. Increased irritation in the eyes

Diagnosis: The first symptom would be the presence of an enlarged thyroid gland (front of the neck above the collarbone), and the some of the above symptoms would be present.
In addition, tests to check for thyroid gland functioning including T3 and T4 levels would reveal increased amounts of hormone in the blood stream.
As a confirmatory test, the radioactive iodine uptake test also is done, which will indicate increased uptake by the thyroid gland. This indicates that the gland is functioning at an increased pace and requires additional iodine for the production of thyroid hormones.

Treatment: There are two approaches to it, one to control the symptoms and the other to control the thyroid gland per se.

  1. Beta blockers are very useful in controlling rapid heart rate and anxiety.
  2. Prednisone may be used to control eye irritation and swelling
  3. Antithyroid drugs are used to control the production of thyroid
  4. Radioactive iodine is given orally to control excessive thyroid production
  5. In severe cases, thyroid gland may be surgically removed partially or completely

Graves’ disease is not life-threatening and once symptoms are controlled, the patient’s quality of life improves drastically. If you wish to discuss about any specific problem, you can consult a endocrinologist.

I am 27 yrs old male and have sugar problem from last 5 yrs my fasting sugar level is around 300 and hb1ac is 12.6. I am not taking any medicine now. The doctor advised me to take triglycomet tds, volix m 0.3 mg bd, janumet 50/500 od, amlopress 5mg od but when I taking medicine my weight is gaining regularly even 6 kg in a month, I also examined my thyroid levels which are normal. please advice me what can I do.

Your hba1c level is too high. You must start the anti diabetic medicines immediately or else it may lead to severe complications. After 3 months once the hba1c levels are normal you can opt for ayurveda medicines.

Hypothyroidism is not a contagious disease. It does not spread because somebody got married and most importantly it is a treatable disease. Any hypothyroid person with treatment is like any other normal person. Please don't stigmatise a perfectly easy to cure illness into a mysterious disease!

Lybrate-user kailashji, from the details mentioned it appears there is a sudden increase in blood sugar after food. Fasting as in the evening low sugar can be countered by proper selection of type of insulin sued along with dosage. Plus there has to be a closer look at the dietary intake and exercise that you are doing daily. Plus now there a continuous glucose monitoring system available which when applied to your arm remains with you for 14 days, checks blood glucose level every 15 minutes (96 times a day), for 14 days, gives various graphs showing highs and lows of blood sugar, which should help your doctor to give clear guidance in terms of insulin dosage and food intake. You are definitely diabetic and it may not any symptoms initially. You need treatment .Diabetes affects all organs in the body – Eyes, kidneys, nerves, reproductive system all are affected, In fact, from toe to hair. For diabetic control, patient and doctor should be in constant touch for regular follow up and advice. The common symptoms are :-Both types of diabetes have some of the same tell-tale warning signs. Hunger and fatigue. Your body converts the food you eat into glucose that your cells use for energy. But your cells need insulin to bring the glucose in. If your body doesn't make enough or any insulin, or if your cells resist the insulin your body makes, the glucose can't get into them and you have no energy. This can make you more hungry and tired than usual. Urinating more often and being thirstier. The average person usually has to pee between four and seven times in 24 hours, but people with diabetes may go a lot more. Dry mouth and itchy skin. Because your body is using fluids to make pee, there's less moisture for other things. You could get dehydrated, and your mouth may feel dry. Dry skin can make you itchy. Blurred vision. Changing fluid levels in your body could make the lenses in your eyes swell up. They change shape and lose their ability to focus. Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For diabetes, hypertension (high BP) and many long-term illness control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. Infertility is a condition, which is diagnosed as the inability of the woman to conceive a child even after a prolonged duration of unprotected intercourse (for about 6 months to 1 year). it is control by medicines and life style, diet. take iodine -rich meals, banana, intake at least 8 glass of water in day. eat more fiber rich foods, coconut oil, green tea, tyrosine -rich foods, low fat milk, avoid soya - products, go for walk daily, do exercises---yoga. Hi my mom is having diabetes. She was on medicine. She was having Glynase -MF 1 and half tablet twice a day. But from last few days she is having a lot urine at night. She is having a loss of weight as well. If there is any cut or injury its taking a lot of time to recover and heal. We did the fasting and blood test. Mr. lybrate-user, Thanks for the query. Since she has been taking oral anti-diabetic drugs for controlling blood glucose, it has to T2 diabetes. In T1 only Insulin Injection has to be given. No other drug works. She now has an uncontrolled diabetes that is the reason for excess urination at night and weight loss, because more of glucose is being lost in the urine. She needs to achieve a strict blood glucose control with FBG < 100 mg, PP 170 to 180 mg & HbA1c% <7. That means there has to be a change in the current treatment. New drugs have to be added or Insulin may have to be started. There are other options also, but in this communication we are supposed to give general guidelines only. No prescriptions are allowed. If you are keen for improving her blood glucose levels and achieve better control, please come in a private conversation with following details: FBG, PP glucose levels, HbA1c%, dietary pattern extent of exercise or physical activity, lipid profile, BP readings. Based on those details I will be able to guide specifically and help improve the condition. There is no need for C- peptide testing. Thanks.

Thank you for the query. It would have been helpful if you would have mentioned your Fasting blood sugar, 2 hrs post prandial sugar and HbA1c level. 1. If your HbA1c level is less than 7.5%, it would imply a fair control on blood sugar. Dietary regimen (avoidance of refined carbohydrate-pasta, white bread, noodles, burgers etc and carbonated beverages) and exercise (30 min per day for 5 days a week) may be helpful. Sometimes, a low dose of the medicine metformin may be added, which may be beneficial. 2. If the HbA1c level is more than 7.5% one or more oral medications are used in addition to diet and exercise. The medications are given as per the patient's tolerance. 3. If HbA1c is more than 9% it is advisable to go for Insulin. However, if someone is completely non-compliant or averse to Insulin a combination of 2 or more drugs may be used with caution. 4. Since you are diabetic for 4 years it is advisable to get the following tests done if not done already Urine for spot ACR- to check the renal status Eye check up - to rule out diabetic retinopathy. 5. Foot care is very important and even a minor injury must be taken care of. 6. Always remember whatever dietary intake you are having, your calorie intake must be less or equal to our calorie expenditure. It is advisable to get a diet chart done, as per your body weight and activity level. 7. You can take a course of vitamin B complex for nerve protection. For a hypothyroid condition there is no specific or special diet. It is best advised to have a diet planned as per the thyroid levels, lifestyle and history. Though you can follow some guidelines - have small frequent meals. Do not fast for long hours. Avoid a very heavy meal at one time. Avoid fruit juices, aerated drinks or sugarcane juice, honey, jaggery or sugar if having insulin resistance. Avoid junk foods. Exercise and stay active. Do not attempt to try out too many remedies as you could go into lowered metabolism. Sleep well. It's all a matter of discipline.

Mr. lybrate-user K, Thanks for the query. Diabetes develops because a person has either less secretion of insulin or there is resistance to its action or both the conditions are present. Once diagnosed (with Fasting 126 mg and above, PP 200 mg and above & HbA1c% 6.5 and above), it can only be controlled by proper medication, diet and exercise. Plus maintaining near normal weight is also very essential. To control diabetes it is essential to adapt a modified lifestyle. Which means taking restricted diet, that gives exact amount of calories needed based on ideal body weight, extent of daily exercise, blood glucose levels. Refraining form alcohol and smoking. As regards efficacy of other alternative therapies being effective in controlling diabetes, well, there is no clinical evidence of it. Unfortunately lot of tall claims are made by some saying diabetes can be cured, which is totally untrue. If you still want to try then it is your choice. Thanks.

Causes, symptoms and treatment of Infertility

Hi, I am Dr. Jayanti Kamany. I am an IVF Consultant practicing at Srishti Fertility Centre and Women's Clinic in Mumbai. So, today i'll be talking about Infertility. The problem of infertility is on a steep rise. Some of the causes for the rise in inferility are increased age at the time of marriage for women, postponement of child bearing. Even if a women gets married earlier she postpones her child bearing for various purposes like her career.

Increased stress levels today, increased levels of sexually transmitted diseases and of course Tuberculosis, which is a very common disease in our country. So, who is to be blamed for the infertility. We have found that 3 out of 10 couples are battling with this problem of infertility. 30% of the faults is found in the female, another 30% is found in the male partner, again 30% is found in both partners and about 10-15% cannot be explained because inspite of all the tests being normal, there is no problem which can be found and we call it unexplained infertility.

There are two types of infertility:

1. Primary infertility- Primary infertility is when the couple has never conceived

2. Secondary infertility-  Secondary infertility is when the couple had a child but cannot conceive again, or the couple has conceived, has had an abortion but there is no conception again.

When should a couple consult or take treatment for infertility? 

Usually if the ovum is young, if she is below 28 years she can wait for one year after trying to consult an Infertility Specialist. That is so nice. But every month you must check fasting and post lunch blood sugars. Diet and exercise plus daily schedule maintenance are the keys to control the diabetes.
