Mr. lybrate-user Akabar, Hair loss is very common in Hypothyroidism and it is usually the last symptom to resolve. On treatment it is essential that TSH level is maintained between 3 to 4 mU/L. Then only all the symptoms are in check. So please see what is the present TSh level. Thanks.

Hi Doctor, I am a 53 year old male, insulin dependent diabetic and a hypertensive. To add to this I am having BI polar disorder. Sugar control is average or below. BP under control. Now my medicines Lantus 20 units at Bedtime, Novorapid 10 units before lunch Glycigon 80, two times a day, two tablets each time For BP Olmin 40 LN Bloc 20 pantoprazole with Domstal one time a day As far as Bipolar problem is concerned I am cleared by psychiatrist to take all the above with the medicines he prescribed. I consider this a sensitive matter. I am always in stress because of my Job, I am the GM or second in command of a large turnover construction company. My morning sugar is between 95 tp 110, but post breakfast after 2 hours is 225. After taking my lunch I take novorapid 10 units hence for few hours my sugar drops to normal range. Again in the evening it when Novorapid effect is gone it climbs to 250-275. Remains like that perhaps till I sleep after taking Lantus 20 units, again in the morning it is normal. HB1C today's test shows a reading of 7.0. What should I do to take this reading to 6.0.

Mr. Lybrate-user, firstly, I congratulate you for having taken very good care of all those multiple problems you are suffering from. Secondly, I must commend the detailed manner in which you have described your present status. Thirdly, you have managed your diabetes pretty well. For your lifestyle & busy work schedule, an hba1c of 7% is quite good. You might be able to bring it down marginally, to 6.5% if you take care of your diet a bit more meticulously & exercise regulary. Beware of hypos when you exercise. All the best!

It may not have any relation with your Diabetes. It seems there is some element of hyperactivity of sympathetic system causing sweating.

I hope that blood pressure and blood sugar levels are under control. Only thing bothering you is lack of sleep. Take a glass if warm toned milk with a spoon of honey added to it. Take care about rest of calories, you can balance by deleting other items worth 200 call in this milk food. Listen to soothing music, read good books, don't watch violence on tv. Meditation before sleep helps.

Dietary recommendations: avoid foods high in iodine, such as seafood, iodised salt, eggs, curd, and milk. Eat cruciferous vegetables like cabbage, cauliflower and broccoli, preferably raw. Green leafy vegetables like mustard greens, spinach and kale are believed to depress the function of your thyroid. Consume flaxseed oil and ground flaxseeds, as they are ideal sources for omega 3 fatty acids and vitamin d from eggs, salmon, mackerel and mushrooms. Soybeans and soy products can help an over-stimulated thyroid. Whole grains like millet and brown rice, combined with nuts and seeds are rich sources of zinc, an important element necessary for the body functions that might be depleted due to hyperthyroidism. Avoid alcohol, smoking, tea, coffee, colas, and chocolates. Take a high potency b-complex supplement daily to replenish the b vitamins used up for increased metabolism.

Hi. Thanks for the query. There are no benefits of drinking wine. The specific nutrient benefit can be got from other healthy foods a s well. Remember you can end up with troubled blood sugar levels if not taken in the right amounts. Even if had it has to be planned into a diabetic diet. Will not advise random intake.

Pay close attention to your food choice. ·        Eat less carbohydrate food sources like chapatti, rice, noodles, pasta, bread etc as they help in gaining weight and storing fat. ·        While having lunch or dinner eat vegetables and dal to fill your stomach rather eating lots of chapattis or rice. ·        Have a healthy breakfast like poha, idli, upma, plain dosa, oats, dalia or khichdi with vegetables. ·        Eat a balance diet including good quality proteins like: fish, lean chicken and eggs. If you are a vegetarian then include skimmed milk, low fat paneer, butter milk, soy bean or soy milk in your diet. ·        Avoid ghee, butter or vanaspati. Prepare your diet with vegetable oil like sunflower, safflower soybean or olive oil. ·        Eat plenty of salads. ·        Eat fresh fruits including apple, orange, pineapple, papaya, sweet lime, berries, guava, watermelon, mask melon etc. ·        Avoid eating outside junk and fried foods like pizza, pasta, roll, burgers, cakes, chocolates and cookies. ·        Have an early dinner. The earlier the better, giving your system more time to digest the food. ·        Avoid cold drinks/alcohol as they add empty calories to your diet. ·        Avoid fat enriched milk products like cheese, sweets, full fat cream etc. ·        Drink 3 liters of water in a day. ·        Try to have less sugar in food and beverages. ·        Try to drink 2 cups of green tea in a day as green tea helps in weight loss by aiding digestion. ·        Avoid red meat like mutton, beef, lamb or pork. ·        Eat sprouts or roasted chana, whenever you feel hungry.

Your insulin is not being utilized fully or doesnt produce. That is what type 2 and type 1 is. You have to get the support of certain herbs which are openly available in market but since not selected with precision decisions, some people do not get desired effects. Please write any other history/tests etc. For herbal combos initially you have to take as supporting therapy. Depending upon response, we ll take further step. Pl write in confidence. We may advise or send package also.

I was diagnosed with dm 2 about two years back. Iam on amaryl-m1 bd & has hba1c of 7. I started feeling extra sensation under the feet, heaviness in foot, leg, thighs from about a month or two, some pain in hip area and finally difficulty in lifting one leg about 2 weeks days back along with knee weakness in one leg although other leg has been affected to a lesser extent till date. I never experienced very sharp pain & 2 weeks back had mild pain in my buttocks for about a week. The loss of movement in the 2 directions direction in the leg & knee weakness was quite quick about 10-12 days back. Now I do not feel any pain but had some falls primarily on account of knee weakness or weakness in right leg. Mri scan, ct scan were ok but problem was reported in nerve conduction test in both legs but mainly in right leg. Was diagnosed with peripheral neuropathy or diabetic amyotrophy which correlates with my overall symptoms. Doctor put me on 8 day course of prednisone (40mg-10mg) with insulin. My sugar with amaryl m1 as on now is under control & I take light food with reasonable physical activity & sugar levels now seems to be ok 92 fasting /140 pp. Also started visiting a physiotherapist from today. As of know there is appears to be some improvements in my walking (can walk 15-20 min at a stretch inside home) though weakness in one leg & knee persist. I cannot drive due to difficulty in movement of my right leg when trying to move it sideways while sitting. It is also not possible to lift the right leg against gravity when laying down though backward lifting & movement on sideways when lying on one side are intact. Have been prescribed pregaba m 75 (hs, neurkind+. Have no pain of any type. My questions 1) has peripheral neuropathy reached a plateau ie. Stopped or not 2) is pregabalin needed in my case as I have no pain, i. E. Whether it play a role in nerve regeneration or is overall beneficial in such situation 3) can I expect improvement in my right leg in 1-2 months time with sugar control & exercises.

1. Peripheral neuropathy is a on going process even in normal individuvals as age advances due to deficiency of vitamins mainly b12 2. In your case pregabalin is needed for some time 3. Nerve regeneration is impossible for anyone 4. Definitely you can expect improvement in the right leg in 1-2 months with good control of sugar & exercises.

Mr. Lybrate-user, insulin is a hormone produced by beta cells of islets of langerhans in the pancreas. So taking insulin is absolutely safe, it does not produce any harmful effect on any of the vital organs. In diabetes either there is lack of insulin or resistance its action or both, particularly in t2 diabetes. In t1 it is totally absent, so those patients have to take insulin for whole life dor survival as well as for blood sugar control.
