Gastro-esophageal Reflux in children.
There are many babies who throw up after almost each feed. They fail to gain weight and falter in their growth curves. They suffer from a laxity in their gastro- esophageal junction. This is called GER (Gastro esophageal reflux). In babies it may cause life threatening breath holding episodes and even severe pneumonia. Then it is called GERD. Most GER resolve with age. Most cases of GERD need medical management. If that fails, surgical funfoplication is needed. These days surgical treatment is done laparoscopically without incision and with minimal pain and excellent recovery.

I hear this complain from many mothers . Your baby cries not due to pain but the sensation of full rectum and bladder cannot be conveyed in word by the baby and they cry and announce ti is time to pee or poop . It is normal behaviour or all babies . Give only breast feed and no bottle feeding. Burp well after each feed

Bed Wetting

BHMS
Homeopath, Delhi
Bed Wetting

Bedwetting or nocturnal eneuresis as it medically is quite common in children. Generally, bed-wetting before age 7 isn't a concern. At this age, your child may still be developing nighttime bladder control.

Bed-wetting is involuntary urination while asleep after the age at which staying dry at night can be reasonably expected.

Most kids are fully toilet trained by age 5, but there's really no target date for developing complete bladder control. Between the ages of 5 and 7, bed-wetting remains a problem for some children. But if it still continues after 7 its a matter of concern this means the nervous control over the bladder is not yet reached.

Causes of bed wetting:

Commonest of all is habits. Many children habitually ignore the urge to urinate and put off urinating as long as they possibly can

Urinary tract infection: the resulting bladder irritation can cause pain or irritation with urination, a strongeurge to urinate (urgency), and frequent urination (frequency).

Inability to recognize a full bladder. If the nerves that control the bladder are slow to mature, a full bladder may not wake your child — especially if your child is a deep sleeper.

Stress and Stressful events — such as becoming a big brother or sister, starting a new school, or sleeping away from home — may trigger bed-wetting.

Role of homoeopathy in bedwetting:

Homoeopathy works on the immune system. Homeopathic medicine will increase the muscle or nervous control and hence can cure it in a months time.

3 Undisclosed Facts About Breastfeeding, This World Breastfeeding Week

DNB (Obstetrics and Gynecology), PGDHHM, MBBS
Gynaecologist, Delhi
3 Undisclosed Facts About Breastfeeding, This World Breastfeeding Week
3 lesser known facts about breastfeeding

Breastfeeding is advocated to be the only best thing for a newborn. But while it is the most natural thing to do, it isn't an easy process. It is challenging and full of surprises. Researches throw up facts that surprise all. For example: breastfeeding is like a shield against breast cancer.
Here are 3 such lesser known facts:

1. The milk contents - ever wondered what your baby thinks of the taste of your breast milk. Well, the fact is that your breast milk is tailor-made for your baby. From the taste to its benefits, it is a full course meal. When the feeding begins, the milk is watery. This is to quench the baby's thirst. Then the consistency thickens and the milk becomes higher in fat content. This meets your baby's electrolyte needs as well as energy requirement. Want to know something more amazing? through the day, your breasts milk's properties keep changing. By evening, it has higher sedating properties and helps the baby's body meet nutritional demands. This should explain that satisfied smile babies have when they sleep right after their feeds.
2. Variation in milk production - it is okay and natural if you feel you are lopsided. Many breastfeeding mothers feel that one breast is heavier than the other since it produces more milk. This is possible and happens a lot. Our body is not perfectly symmetrical between left and right. This applies to breasts as well and shouldn't be worried about. Because of this, milk production can also vary between both the breasts.
3. It causes weightloss - some women are known to lose their pregnancy weight by breastfeeding. Yes, this is also a lesser known fact. Breast milk happens to be a high calorie food which the baby literally sucks off the mother's body. This leads to shrinking of the mother's body.

ADHD is not fun for the child!

ADHD or attention deficit hyperactivity disorder is a condition characterized by an inability to concentrate and sustain attention. It is a biological problem of the brain.

Due to poor concentration the child feels bored easily and may become impulsive and appear to be mischievous

He may answer out of turn and will talk even when not spoken to.

He may have difficulty sitting at one place.

He maybe restless, fidgety and jumping about.

This will hamper his ability to remember things and may become forgetful and lose things.

This is something the child does not do on purpose and feels guilty and unhappy about, making the child feel low on self-esteem which may manifest as aggressive and rebellious behavior.

Tips for the teacher and parents

1. Keep the child close to her in the classroom

2. Keep the child away from the windows

3. Shower praise when he behaves appropriately.

4. Write important information down where the child can easily read and reference it

5.Divide big assignments into smaller ones, and allow children frequent breaks.

6. Incorporate Physical movement into classroom teaching.

7. Working with interruptions:

Reducing the interruptions of children with ADD/ADHD should be done carefully so that the child’s self-esteem is maintained, especially in front of others. Develop a “secret language” with the child with ADD/ADHD. You can use discreet gestures or words you have previously agreed upon to let the child know they are interrupting. Praise the child for interruption-free conversations.

8. Dealing with Impulsivity:

Give consequences immediately following misbehavior. Be specific in your explanation, making sure the child knows how they misbehaved.

Recognize good behavior out loud. Be specific in your praise, making sure the child knows what they did right.

Write the schedule for the day on the board or on a piece of paper and cross off each item as it is completed. Children with impulse problems may gain a sense of control and feel calmer when they know what to expect.

9. Dealing with Hyperactivity:

Ask children with ADD/ADHD to run an errand or do a task for you, even if it just means walking across the room to sharpen pencils or put books away.

Encourage the child to play a sport—or at least run around before and after school.

Provide a stress ball, small toy, or other object for the child to squeeze or play with discreetly at his or her seat.

Limit screen time in favor of time for movement. Your child will enjoy fast games like Subway Surfer and Temple Run.

Make sure a child with ADD/ADHD never misses recess or PT period.

Warm cloth is useful particularly in this hot weather. Excessive cough may be due to chest infection which require thorough examination. Pls take another opinion from pediatrician. Blood in poo (stool) may not be associated with cough. Pediatrician shall take care of both ailment.

Diploma in Child Health (DCH), MBBS

The baby should get breast milk at least for 6 months. Mother should eat nutritious diet and take enough milk. She should also try to keep anxiety away. This should ensure enough milk supply. She should also feed the baby more often.

My baby was born premature at 32 weeks and had sepsis. She is now 1.y months old. The OAE screen has come refer twice however the baby responds to sounds of rattles and music when she cries and we use rate she goes quite. Please suggest if she has hearing issues.

C.S.C, D.C.H, M.B.B.S
General Physician,
My baby was born premature at 32 weeks and had sepsis. She is now 1.y months old. The OAE screen has come refer twice...
THere is no hearing loss. But OAE refers to lsoo in certain frequency and repeat it later Pure-tone (PT) audiometry measures throughout the outer ear, middle ear, cochlea, cranial nerve (CN) VIII, and central auditory system. However, OAEs measure only the peripheral auditory system, which includes the outer ear, middle ear, and cochlea. The response only emanates from the cochlea, but the outer and middle ear must be able to transmit the emitted sound back to the recording microphone. OAE testing often is used as a screening tool to determine the presence or absence of cochlear function, although analysis can be performed for individual cochlear frequency regions. OAEs cannot be used to fully describe an individual's auditory thresholds, but they can help question or validate other threshold measures (eg, in suspected functional [feigned] hearing loss), or they can provide information about the site of the lesion. Using current technology, most researchers and clinicians find a correlation between frequency-specific analysis of TOAEs/DPOAEs and cochlear hearing loss. However, at this juncture, the correlation cannot fully describe auditory threshold. Naturally, a correlation would not be expected for noncochlear hearing loss.

Hi sir. My son has 6 year old but still he is doing toilet in bed. Pls. Suggest what we do.

Pediatrician, Mumbai
Toilet meaning urine or stool? if urine then you can get a urine routine done to rule out infection. If reports normal then you can wait for some few months before starting therapy. If stool the you need to consult a pediatrician.

My son 3 years Old have constipation problem can you suggests me any good medicine ?

Pediatrician, Mumbai
It is better to increase fibre in diet , rather than giving any medications. You can give him green leafy vegetables , corn , multigrained rotis , fresh fruits to increase fibre content and increase fluid content per day. Inspite of that , if contipation persists , you may give him Fybogel or Isapgol 1 tsf in 1 glass of water or milk at night . This is natural plant based fibre and is safe . Occasionally , if required you may also use glycerin suppository , however it should not become a practice.
