Hi, Wisdom tooth pain often gets referred to other areas such as Ears. It is Advisable to visit a dentist and get a X-ray of that area done .Depending on the severity of the cavity either a root canal or removal of the tooth is the choice of treatment.

1. Miconazole ointment or candid ointment - need to apply to the corners of your mouth usually 2-4 times daily for 2 or more weeks 2. Chlorhexidine mouthwash used four times daily 3. Course of tablets such as fluconazole 4. C. Becosules 5 cap, for five days in the morning after meals.

Oral Health Complications During Pregnancy

Recent findings have reported that oral bacteria and their by-products can possibly pass the placental barrier keeping oral health at prime importance during pregnancy. An awareness related to oral health during pregnancy cannot be neglected or avoided in order to minimize the complications.

This article focuses on few points related to the necessity of oral health during pregnancy. 

Pregnancy comes with many responsibilities making expecting mothers more concerned about their health. However, many pregnant women may not be aware of oral health and its effect on the developing child. Negligence of oral health is quite common during pregnancy due to lack of alertness of oral hygiene and its effect on systemic health.

Moreover, pregnancy discomforts like frequent bouts of nausea, increased physical demands, weight gain or certain dental problems like pregnancy gingivitis or inflammation of gums may be dangerous to developing fetus.  

Risk of oral problems during pregnancy:-

Few common oral problems associated with pregnancy are as follows. 

Pregnancy Gingivitis: 

Hormonal changes during pregnancy leads to pregnancy gingivitis. It is a condition of bleeding gums, redness. Moreover inflammation, swelling and tenderness are also observed. Ignorance of such problems may lead to serious gum disease.  Frequent cleaning can avoid such dental issues. Progesterone levels are elevated during pregnancy which stimulates the production of prostaglandins and leads to inflammation of blood vessels in gums.

Tooth Decay:

Diet during pregnancy is of high importance. It is designed to nourish both mother and unborn baby. Diet rich in carbohydrates actually increases the risk of tooth decay. The morning sickness increases acid secretion in the mouth which is harmful to enamel. 

Pregnancy tumors: 

Overgrowth of tissue on gums in pregnant women are called as pregnancy tumors. They are noncancerous and generally observed in the second trimester of the gestation period. It is nothing but swollen gums between teeth. The bleeding and reddening of gums observed in such conditions.  

Periodontal disease and preterm birth:

Untreated gum disease or periodontal problems can development periodontitis characterized by inflammation and infection of the gums and supporting structures of the teeth. 

This ultimately leads to loss of supporting material and finally the teeth themselves. 

A study result has revealed that there is a relation between preterm birth and presence of gum diseases in the pregnant women. The reason is not yet known. 

However, research study findings have evidence that oral bacteria or their byproducts can pass placental barrier. This initiate inflammatory response leading to preterm birth. 

Another correlation exists between the gum disease and the increased rate of pre-eclampsia, a condition of high blood pressure during pregnancy. The positive correlation suggests that gum disease may cause stress to the blood vessels of the mother, placenta and the fetus.

The basic periodontal therapies like oral hygiene instructions, scaling, root planing and prophylaxis can substantially decrease the level of prostaglandin responsible for inflammatory response. 

The regular treatment and follow-up in pregnant women can maintain good oral health and minimize the risk of preterm birth. 

Dental treatment during pregnancy: 

The dental procedures mostly include the use of X-rays and use of anesthesia. In the case of pregnancy, these procedures can be used with precaution. 

Therefore, before sitting on dental chair patient should declare her pregnancy to the physician. Untreated dental infections can harm fetus as well as the mother. 

The emergency situations request use of dental radiographs which can’t wait. The radiation of dental x-ray is generally very low and precaution is always taken for lowest radiation exposure. A leaded apron including collar, shields can be used to prevent abdominal exposure to the x-ray radiation. 

Use of dental X-ray is not at all contraindicated in pregnancy when used with precaution. 

The use of local anesthesia is also not contraindicated in the pregnancy. 

A clinical trial result showed that there is no harm in the use of local anesthetics during pregnancy. It is not linked with increased risk for major medical problems like cerebral palsy, cleft lip and heart defect in newborns. 

The medication prescribed during dental treatment mainly includes local anesthetics, analgesics, and antibiotics. Most of them can be used with safety precautions. Each drug has its own pregnancy category which should be considered during its use. 

The antibiotic like tetracycline can permanently cause discoloration of developing teeth.  

Prevention: 

Dentists can educate patients with following points to avoid problems associated with oral hygiene. 

  • The patient should declare the pregnancy to  the Dentist 
  • Follow routine oral checkup. 
  • Undergo all elective and emergency procedures. 
  • Follow good oral hygiene practice ask for a better toothpaste.
  • In the case of morning sickness avoid sweets and snacks which increase the risk of tooth decay. 
  • Have healthy and balanced diet. 

Awareness of oral health during pregnancy is a must to avoid complications.

Several researches and studies have proved that any decay or damage to a child's milk teeth leads to problems that affect the permanent teeth as well. Also, a mother's improper dental hygiene during pregnancy proves to be harmful for the developing baby. Here are some useful tips if you're planning a baby or if you already are a proud mother of one!

  • Your dental health also contributes to your overall health. Just like you pay regular visits to your Gynaecologist during pregnancy, make sure you also find yourself a friendly, neighbourhood dentist to take care of yours and your child's teeth.
  • The hormonal upsurge during pregnancy casts its effects on your gum health too. This leads to increased inflammation and plaque build-up in your oral cavity resulting in gazillions of bacteria in your mouth. What goes in your mouth, goes inside your body too. All these bacteria are transferred through the blood stream to your baby resulting in the following ill effects:
  1. Preterm babies with low birth weight
  2. Increased risk of premature delivery
  3. Gum bleeding
  • Do not defer your dental treatment. Most dental treatments can be safely done during the second trimester. Any emergency dental problem can be taken care of after obtaining a consent from your gynaecologist. 
  • First visit, first tooth. The first milk tooh appears at 6 months of age and that's when you need to schedule your baby's first appointment with the dentist. Keep your kid's gum pads clean by wiping them with a clean wet gauze pad or wash cloth after every feeding. Start brushing your child's teeth with a kid-sized toothbrush when milk teeth begin to erupt. Don't let your child sleep with sweetened milk bottle or pacifier in the mouth.
  • Be a role model. Overcome your own apprehensions related to dentistry and inspire your children to take care of their oral health. Good dental habits need to be taught at an early age. Monitor your kid's toothbrushing till 8-9 years of age. Teach them the importance of regular dental visits.

Follow these useful tips and pass on a healthy legacy to your child. A mother always wants to give the best of everything to her children and what's better than a healthy smile.

Use any toothpaste I repeat use any toothpaste other than used for teeth sensitivity as it doesn't have any major impact in adult permanent tooth structures. Instead a proper brushing technique brings a drastic change in your oral hygiene.

Quit habits strictly. Seems like you have oral submucous fibrosis (osmf). Kindly consult an oral & maxillo facial surgeon in person for further suggestion. We need more investigations with clinical examination to decide upon treatment. You may need conservative or surgical management for mouth opening depending upon the stage of osmf. Advise full mouth opening, ballooning exercise & side to side movement of the jaw. Wrap 25 to 30 ice-cream sticks, make a bundle, put it between upper & lower front teeth for 15 minutes thrice a day increase 1 sticks each day.

Using tooth picks

The worst habit you could possibly have when you have food lodgement in the mouth is using a tooth pick to get rid of it. We often try and use this as an acceptable habit even socially but few things are grosser than watching people pick out food bits after meals. If you have a cavity get it fixed. We have numerous cases where the toothpicks have either injured the gums and caused a larger infection or even worse broken in the cavity and triggered of even more pain. So stay safe from dental emergencies do not use tooth picks! while it's important to prevent food lodgement in the mouth and keep your mouth clean use things like brushes interdental brushes and floss sticks or dentists to help you do the same without harming you!

Using a hard or medium brush.

The oldest myth we have seen is brushing your teeth with a hard or medium brush. Please terminate this habit right away ensure you switch to a soft brush at the earliest.

We do not need more teeth ruined or worn off due to overzealous brushing. We see cases with the entire left side worn down and sensitive and I can tell the person is a right handed vigorous brusher. The softer the brush the better it bends and cleans between teeth as well. The gums also recede due to repeated trauma from the hard bristles so save your mouth from these constant attacks in the name of cleaning.

Scrubbing your teeth

The same principle applies when you switch from a hard to soft or extra soft brush don't apply so much pressure that the teeth and gums are still braving the injury.

You need to brush gently as though cleaning glass the force being minimum the reach and coverage being maximum!

So scrubbing is a habit we need to stop.

Not brushing at night

One of the best things we can do to avoid the dentist or any dental troubles is brushing at night. When it comes to cleaning your teeth frequency matters more than force. Ensuring you clean every 12 hours makes bacterial attacks that cause decay inactive so whenever we skip brushing at night we may not realise but it's definitely costing us.

Popping painkillers

The first sign of dental pain and we instinctively choose to ignore it hoping it will go away and if it doesn't we want to go ahead and pop a painkiller till it does. Masking the problem will actually make it bigger -while we all know this we prefer to accept the instant relief the medication provides over the tedious dental visit that may end with bad news. But imagine if we had gone at the very first sign of trouble most likely we would get away with a dental cleaning -the later we go the more often we need to go for extensive treatments.

Let's try and muster the courage to nip these problems in the bud -a habit to develop show up at the very first sign of trouble.

Do it yourself dentistry (diy) at home

We have all been our own dentists at some point!

From self medication to oral applications and home remedies when all else fails we make an appointment.

A few things are harmful and should be used with care when attempting our own dental care.

We recommend mild things like oral rinses or applications which relieve pain but do not use aspirin or any other pain killer to crush and put in the site, that can cause burns.

Also clove oil is a safe remedy but not cloves which can be sharp and hurt your cheek.

Do not try to remove shaking teeth or attempt any procedures the bleeding can be profuse and the damage severe.

In case of trauma ice to freeze the blows and water to clean the wound will work best.

In the age of technology and information let's not fall prey to easily avoidable dental habits that harm you more than they help you.
