Peripheral Artery Disease - How Can It Be Diagnosed & Treated?

Cardiologist, Delhi
Peripheral Artery Disease - How Can It Be Diagnosed & Treated?

Peripheral artery disease is a condition brought about by the partial blocking of the main arteries that carry pure blood from the heart to the different parts of the body. When the block occurs or when the flow of blood is restricted, then the functions are directly affected since they may not receive sufficient blood at regular intervals. The most frequently observed form of the peripheral artery disease is the feeling of pain in the legs while walking.

Though it is in the legs that most people seem to find the pain initially, there are other parts of the body like the arms, stomach, head and even the kidneys. As mentioned, the trouble starts when the body is not able to burn enough fat and it deposits itself in the inside walls of the blood vessels leading to these cramp like conditions. These deposits are also known as plaques. Within the medical field, the peripheral artery disease can also be identified or called by other names such as Atherosclerosis or peripheral vascular disease and so on.

Diagnosis of the disease
As with any health condition, the specialist will do a physical examination of the part which you complain is giving you the pain and will ask you a series of questions regarding your personal details, family history and any previous complaints of this sort or of different nature. The best way the doctors would confirm if it is a peripheral artery disease is by a process called ankle-brachial index. This involves finding the differential between the pressure of the blood in your body, in the arms which are closer to the heart, and the legs the farthest. This could reveal to the doctor if the rate of flow of blood to the farthest part like the feet is the same as to the arms. If the doctor finds a substantial difference in the blood circulation, then the doubt increases. The advanced diagnostic tests needed to confirm a case of peripheral artery disease include the Doppler and Ultrasound tests, Angiography and Magnetic resonance Angiography (MRA). These tests will give conclusive evidence to the doctor that it is indeed a case of peripheral artery disease.

Treatment for Peripheral Artery Disease
There are different ways in which the peripheral artery disease can be countered. One way is to change the lifestyle and food habits of the affected person. Since it is obvious that the deposition of plaque from the fatty foods consumed by the person is responsible for the condition, merely stopping the consumption of such items can have a salutary effect. Other steps like quit smoking, if you were into this bad habit, exercising regularly and taking suitable medication as prescribed can treat the condition. In severe cases, procedures like Angioplasty might have to be performed.

Go for a complete medical check up with a physician. If you think that you will find it very difficult, then admit yourself to a hospital and go for de-addiction: which normally lasts for about two weeks. Then follow that up with addiction counseling from a professional for at least three years to completely get rid of the habit. If this is also not enough, then admit yourself into a de-addiction center and stay there for at least 6 months and after that attend counseling with a professional. There are medicines that help with the drinking just to ease the initial craving, called antabuse. It has to be taken regularly and even a minor consumption of alcohol (like in cough syrups), accidentally or otherwise, will lead to reactions, sometimes requiring hospitalization. Ultimately, it is your will power and the support that you receive from the medical fraternity and your close and dear ones. You must also learn to substitute and deal with the oral need, a rigid value system, the script issue, and of course look at all the genetic factors to plan a strategy not to get into what is called ‘cross addictions’ i.e. another form of addiction that may appear alright but is in fact as bad as the primary addiction. The center or hospital and the counselor will advise and guide y ou on several measures and precautions you will need to take to stay with your resolve. Even after the rehabilitation you must attend AA meetings and continue this support for a long time. Your family will also need to attend some sessions and go for Al-anon meetings for their co-dependency issues. You cannot be treated in isolation because the family has gotten used to your drinking and have made some unhealthy adaptations to somehow cope. Your children will also have to attend meetings to work out their issues because of your habit. In fact, they are all suffering from the Adult Children Of Alcoholics Syndrome (ACOAs), which in effect means that they are genetically predisposed to alcoholism or can have cross addiction problems and they will have similar traits of the abusing alcoholic but in a milder form. There are special support groups for them all over the world. Should they touch or indulge in alcohol or any addictive substances or behaviors, they could also become full-fledged addicts themselves. Make a serious plan with the wife and children and whoever else’s support you can get and act on it fast.

Sir what are the symptoms for heart failure. How can we identify. How to cure that disease. Why and how do we get.

PGD IN ULTRAASONOGRAPHY, Non invasive cardiology course, MD - Medicine, MBBS
General Physician, Narnaul
Sir what are the symptoms for heart failure. How can we identify. How to cure that disease. Why and how do we get.
Heart failure does not mean the heart has stopped working. Rather, it means that the heart's pumping power is weaker than normal. With heart failure, blood moves through the heart and body at a slower rate, and pressure in the heart increases. As a result, the heart cannot pump enough oxygen and nutrients to meet the body's needs. The chambers of the heart may respond by stretching to hold more blood to pump through the body or by becoming stiff and thickened. This helps to keep the blood moving, but the heart muscle walls may eventually weaken and become unable to pump as efficiently. As a result, the kidneys may respond by causing the body to retain fluid (water) and salt. If fluid builds up in the arms, legs, ankles, feet, lungs, or other organs, the body becomes congested, and congestive heart failure is the term used to describe the condition. What Causes Heart Failure? Heart failure is caused by many conditions that damage the heart muscle, including: Coronary artery disease. Coronary artery disease (CAD), a disease of the arteries that supply blood and oxygen to the heart, causes decreased blood flow to the heart muscle. If the arteries become blocked or severely narrowed, the heart becomes starved for oxygen and nutrients. Heart attack. A heart attack occurs when a coronary artery becomes suddenly blocked, stopping the flow of blood to the heart muscle. A heart attack damages the heart muscle, resulting in a scarred area that does not function properly. Cardiomyopathy. Damage to the heart muscle from causes other than artery or blood flow problems, such as from infections or alcohol or drug abuse. Conditions that overwork the heart. Conditions including high blood pressure, valve disease, thyroid disease, kidney disease, diabetes, or heart defects present at birth can all cause heart failure. In addition, heart failure can occur when several diseases or conditions are present at once. You may not have any symptoms of heart failure, or the symptoms may be mild to severe. Symptoms can be constant or can come and go. The symptoms can include: Congested lungs. Fluid backup in the lungs can cause shortness of breath with exercise or difficulty breathing at rest or when lying flat in bed. Lung congestion can also cause a dry, hacking cough or wheezing. Fluid and water retention. Less blood to your kidneys causes fluid and water retention, resulting in swollen ankles, legs, abdomen (called edema), and weight gain. Symptoms may cause an increased need to urinate during the night. Bloating in your stomach may cause a loss of appetite or nausea. Dizziness, fatigue, and weakness. Less blood to your major organs and muscles makes you feel tired and weak. Less blood to the brain can cause dizziness or confusion. Rapid or irregular heartbeats. The heart beats faster to pump enough blood to the body. This can cause a rapid or irregular heartbeat. If you have heart failure, you may have one or all of these symptoms or you may have none of them. They may or may not indicate a weakened heart. Tests to determine the cause and severity of your heart failure. These include: Blood tests. Blood tests are used to evaluate kidney and thyroid function as well as to check cholesterol levels and the presence of anemia. Anemia is a blood condition that occurs when there is not enough hemoglobin (the substance in red blood cells that enables the blood to transport oxygen through the body) in a person's blood. B-type Natriuretic Peptide (BNP) blood test. BNP is a substance secreted from the heart in response to changes in blood pressure that occur when heart failure develops or worsens. BNP blood levels increase when heart failure symptoms worsen, and decrease when the heart failure condition is stable. The BNP level in a person with heart failure -- even someone whose condition is stable -- is higher than in a person with normal heart function. BNP levels do not necessarily correlate with the severity of heart failure. Chest X-ray. A chest X-ray shows the size of your heart and whether there is fluid build-up around the heart and lungs. Echocardiogram. This test is an ultrasound which shows the heart's movement, structure, and function. The Ejection Fraction (EF) is used to measure how well your heart pumps with each beat to determine if systolic dysfunction or heart failure with preserved left ventricular function is present. Your doctor can discuss which condition is present in your heart. Electrocardiogram (EKG or ECG. An EKG records the electrical impulses traveling through the heart. Cardiac catheterization. This invasive procedure helps determine whether coronary artery disease is a cause of congestive heart failure. Stress Test. Noninvasive stress tests provide information about the likelihood of coronary artery disease. Other tests may be ordered, depending on your condition.

The initial choice of treatment is a steroid ointment that is locally applied. (This can be suggested by a doctor after you are examined in person.)The ointment softens the foreskin and is applied for up to 6 weeks. When full retraction is possible the ointment is stopped. If this treatment is unsuccessful, a circumcision (surgical removal of foreskin) may be advised.

Dear lybrate-user, improve your diet, timing of diet. Avoid smoking and alcohol. Avoid fried items. Don't lie down soon after meals. Have omez only when you get acidity. Long term use of omez may lead to bone thinning or osteoporosis. Check the list of medicines and remove offending agent with help of your doctor. Long standard diabetes leads to gastroparesis or slow move of stomach which leads to acid reflux. Discuss this with your doctor.

How should I change my diet to reduce high uric acid level in blood without taking medicine.

MBBS, M.MED, DFM, FID, CCEBDM, ACMDC, CCMTD
Diabetologist, Hyderabad
How should I change my diet to reduce high uric acid level in blood without taking medicine.
Lower fat intake as excess fat reduces normal excretion of uric acid Liberal fluid intake to keep the urine dilute Restriction or elimination of alcohol What to avoid Shell fishes, red meat, mince meat, meat extracts, organ meats, fishes (sardine, roe, makeral). Whole pulses (masur, rajmah, chana, chole, etc.). Other dals are allowed Vegetables and fruits - green peas, spinach, french beans, brinjal, cauliflower, mushroom, chickoo, custard apple Diet rules Use less salt while cooking. Avoid adding salt while cooking rice or in chapatti dough Use adjuncts like tomato, vinegar, cocum, amchur, lime juice, curd, pepper powder to make the food cooked with less salt palatable Remove the salt shaker from the table Avoid salted nuts/ biscuits, papads, pickles, chutneys Avoid excess consumption of processed and preserved foods; eg. Bakery products Caution - Do not overdo on salt substitutes as that might cause an electrolyte imbalance Recommended foods Fresh cherries, strawberries, blueberries and other red-blue berries Bananas Celery Tomatoes Vegetables including cabbage and parsley Foods high in bromelain (pineapple) Foods high in vitamin C (red cabbage, red bell peppers, tangerines, mandarins, oranges, potatoes) Low-fat dairy products Complex carbohydrates (breads, cereals) Chocolate, cocoa Coffee, tea Although Purines (organic compounds, which create uric acid breakdown in the body) are found in vegetable sources, they don't cause the same amount of uric acid buildup in the body that meat sources do. In fact, fresh fruits and vegetables reduce acid levels in the body and it is often advised to avoid red meats, organ meats and highly processed foods. Fresh fruits and vegetables are encouraged so the patient can increase alkalinity in the blood. Therefore, rather than attempting to find foods that are free of purines and uric acid, it's important to know how to increase the body's alkalinity, which naturally reduces the body's acidity. Fresh, non-processed foods, such as fruits and vegetables, are the way to go.
