Dear Doctor, I am a gross diabetic and hypertensive and have undergone cabg heart bypass ten years back. I am a professional working in a nationalized bank in supervisor cadre as officer. Since three months my feet and upper feet are burning and I feel that blood may shootout any time in that intensity of pain. There are frequent swellings in the ankles and feet, and unbearable pains in the entire day and more in the nights. I am using mixtard40 insulin 30am 16pm units, metformin, and silica-t and metyxyl for Blood pressure and also one nurokind one in the morn I in the evening. Yet no relief. Please advice for correction.

I am suffering with back pain from long time. I have been done of MRI my L5 S1area. In MRI report came my spine disc L3 L4, L4 L5, L5 S2 dehydration. I have been consult my doctor with MRI report they suggest to me for regularly exercise of some special type of exercise step and giving one injection in my spine. I don't know name of the injection but I am feeling well with that injection for some time. Now again slowly start same problem. So I want needful solution of my problem. Degenerative disc disease is not really a disease but a term used to describe the normal changes in your spinal discs camera.gif as you age. Spinal discs are soft, compressible discs that separate the interlocking bones (vertebrae) that make up the spine. The discs act as shock absorbers for the spine, allowing it to flex, bend, and twist. Degenerative disc disease can take place throughout the spine, but it most often occurs in the discs in the lower back (lumbar region) and the neck (cervical region). The changes in the discs can result in back or neck pain and/or: Osteoarthritis, the breakdown of the tissue (cartilage) that protects and cushions joints. Herniated disc, an abnormal bulge or breaking open of a spinal disc. Spinal stenosis, the narrowing of the spinal canal camera.gif, the open space in the spine that holds the spinal cord camera.gif. These conditions may put pressure on the spinal cord and nerves, leading to pain and possibly affecting nerve function. What causes degenerative disc disease? As we age, our spinal discs break down, or degenerate, which may result in degenerative disc disease in some people. These age-related changes include: The loss of fluid in your discs. This reduces the ability of the discs to act as shock absorbers and makes them less flexible. Loss of fluid also makes the disc thinner and narrows the distance between the vertebrae. Tiny tears or cracks in the outer layer (annulus or capsule) of the disc. The jellylike material inside the disc (nucleus) may be forced out through the tears or cracks in the capsule, which causes the disc to bulge, break open (rupture), or break into fragments. These changes are more likely to occur in people who smoke cigarettes and those who do heavy physical work (such as repeated heavy lifting). People who are obese are also more likely to have symptoms of degenerative disc disease. A sudden (acute) injury leading to a herniated disc (such as a fall) may also begin the degeneration process. As the space between the vertebrae gets smaller, there is less padding between them, and the spine becomes less stable. The body reacts to this by constructing bony growths called bone spurs (osteophytes). Bone spurs can put pressure on the spinal nerve roots or spinal cord, resulting in pain and affecting nerve function. To relieve pain, put ice or heat (whichever feels better) on the affected area and use acetaminophen (such as Tylenol) or nonsteroidal anti-inflammatory drugs, such as ibuprofen or naproxen. Your doctor can prescribe stronger medicines if needed. If you develop health problems such as osteoarthritis, a herniated disc, or spinal stenosis, you may need other treatments. These include physical therapy and exercises for strengthening and stretching the back. In some cases, surgery may be recommended. Surgery usually involves removing the damaged disc. In some cases, the bone is then permanently joined (fused) to protect the spinal cord. In rare cases, an artificial disc may be used to replace the disc that is removed. My MRI INDICATE 1. Lumbosacral transitional Vertebra with complete socialization of L5 2. Disc desiccation with diffuse disc bulge and broad based posterior central disc protrusion at L4-L5 level causing ventral thecal sac indentation and significant compromise of bilateral neural forminal (right>left). Mild ligamentum flavum thickening is also seen at this level with maintained spinal canal dimensions. 3. Cervical spondylitis changes with disc osteophyte complexes at C4-C5 and C5-C6 levels. Broad based left paracentral disc protrusion at C4-C5 level causing ventral thecal sac indentation and moderate compromise of left sided neural forminal. 4. Mild disc bulge at d5-d6 (screening of rest of spine revealed) I am confused. Dr. Said physio will fix it. I read too many damages. Please guide and suggest sustainable solutions. I am willing to take prolonged treatment. What does this report mean?

Hello Mr. lybrate-user. The report says that there is significant compression of nerves at L4-5, C4-5 and C5-6 vertebral levels. You need to wear lumbar hot belt on regular basis, have to take medicines, you have to undergo physiotherapy and take precautions like no forward bending, no lifting weight, using ortho matress to keep spine straight. I would advise you to meet an orthopedician and discuss your symptoms in detail. Clinical examination and some further tests are required before prescribing medicines.

I am having ankle pain for the last 6 months mostly on my left foot. It increases after I get up from sleep. But when I do something reduces. Actually the pain increases after my foot gets rest. One thing I want to inform that once I saw a article based on ankle problem. And if I am not wrong they specified my problem as" Calcaneal Spur"

When you take rest the supply of blood in the heel is reduced so when you move from the position of rest to standing the area hurts, once the supply of blood is restored the pain goes away. The issue is because of stress of your plantar fascia. This if not cured can lead to a calcium deposit which looks like a bonny outgrowth called calcaneal spur. The exact line of treatment can be determined only after a physical examination. I suggest you 1. Apply cold pack on the heal and the surrounding areas 2-3 times a day 2. Do stretching exercise for calf and ankle. 3. Have you gained weight off late? 4. If the pain does not go with cold pack and exercise or keeps reoccurring then you should consider using an orthotic in your foot wear. I deal with plantar fasciitis/calcaneal spur patients every day so feel free to ask any question you have on this subject.

Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. Hamstring Stretch Lie on your back with your left leg flat on the floor. Loop a towel or rope around your right foot and pull your leg as far as comfortable toward your chest, while keeping a slight bend at the knee. Keep your back pressed to the floor throughout the stretch. Hold for 10 to 30 seconds and then release. Repeat 3 or 4 times with each leg. Do this stretch 5 or 6 times a week. Short-Arc Knee Extensions In the same starting position as the straight-leg raises, put a ball (about the size of a basketball) under your left knee so that your leg is bent. Slowly straighten your leg. Hold, then slowly lower. Repeat with your right leg. Exercises To Avoid A few of the following exercises can be done safely if you have chronic knee problems; they're on this list because they're more likely to be done improperly. The exercises above are safer, while still giving you similar results. •Full-arc knee extensions •Lunges •Deep squats •Hurdler's stretches.
