My mother's age is 52 and she is nearing menopause. Her periods stopped. She is complaining of severe vaginal dryness. What ointment or tablet can I use. She is hypertensive and on treatment. She also had thyroidectomy for papillary ca last year and on thyroxine.

Dear lybrate-user. Thanks for the question. The hormone estrogen helps maintain that fluid and keeps the lining of the vagina healthy, thick, and elastic. During menopause, the drop in estrogen levels reduces the amount of moisture available. It also makes the vagina thinner and less elastic. Vaginal estrogen cream (Estrace, Premarin). You use an applicator to insert the cream into your vagina. You will typically apply the cream daily for one to two weeks then reduce the frequency to one to three times a week as directed by your doctor.

Angioplasty - Know More About It!

Multi Speciality
Cardiologist, Delhi
Angioplasty - Know More About It!

Angioplasty is referred to as a cardiac procedure, which involves widening the narrowed section of a coronary artery. Usually performed by an interventional cardiologist, this procedure is performed in a cardiac catheterization lab, short for a cath lab. An interventional cardiologist takes extra care to review the risks, in addition to obtaining your counsel prior to preparing you for the procedure. The physician also refers to your medical history in order to determine the amount of anesthesia that should be used.

What happens during the procedure?
Although mild sedatives will be administered to help you relax, the process requires you to remain awake throughout the procedure. The procedure of angioplasty involves cardiac catheterization. The doctor numbs the intended site through medications and inserts the catheter. Following such the doctor inserts a thin sheath in your artery, oftentimes in your groin or arm. A long, narrow, empty tube known as a catheter, is then carefully guided through the sheath, through the blood vessels and into the arteries near the heart.

A little amount of liquid is then injected through the catheter, which is moved through the chambers, valves and vessels of the heart. This movement is then pictured with the help of X-rays, with which the doctors can diagnose the condition and working of the coronary arteries and the heart valve.

The surgical procedure lasts for around 1.5 - 2.5 hours, but the preparation and post - surgical procedures may take several hours. A patient is usually made to stay overnight to be monitored by medical personnel.

What to expect before an Angioplasty?
Most people are made to undergo a blood test and electrocardiogram before having an angioplasty. Usually scheduled a day before the procedure, such tests usually require separate appointments. Usually patients are restricted to eat since 12 hours before the exam.

What happens post-procedure?
After the procedure, you will be instructed to remain flat for 5-6 hours without bending your legs, so as to prevent bleeding. The nurse will instruct you as to when you can get up from your bed. You may not be permitted to drink or eat until the groin sheath is removed as such can cause nausea. If you wish to discuss about any specific problem, you can consult a Cardiologist.

Hello there, for cough, take 2-2 teaspoonful of pranacharya kasantak by mixing it in hot water twice daily. Sitopaladi, praval and tankan bhasm churna taken with honey can also bring relief. Consume this 3 to 4 times a day for relief from dry cough; you can also drink tea with basil leaves or add ginger and black pepper in it. Salted hot water gargling also helps. And stay away from cold drinks and don't take milk when having cough.

Worrying about anything will not solve. You alredy have done the right thing to do all the important tests and as they are normal now the only thing to do is to follow a healthy life style. One thing you can do is take a multivitamin and multimineral tablet. You can also consult for homoeopathic medicine which has a very good effect both physically and psychologically.

I want know about tight foreskin in penis please suggest me better way to solve it.
Dear Lybrate user. Phimosis is a congenital narrowing of the opening of the foreskin so that it cannot be retracted. Phimosis is a very common problem. In most of the cases you can clear the problem yourself. Apply oil on your foreskin and on the glans. Move the foreskin forward and back slowly without hurting you much. Because the oil is lubricant, it will be easier than without oil. Do this forward and back movement 20 times several times in a day. Repeat the same every day for around one month. You will get the elasticity and you will be able to retract the foreskin completely. In case if there is no result from the above massage, you can always go for a minor surgical procedure called circumcision. Take care.

Hi, there are two types of insomnia: primary insomnia and secondary insomnia. Primary insomnia: primary insomnia means that a person is having sleep problems that are not directly associated with any other health condition or problem. Secondary insomnia: secondary insomnia means that a person is having sleep problems because of something else, such as a health condition (like asthma, depression, arthritis, cancer, or heartburn); pain; medication they are taking; or a substance they are using (like alcohol). Sleep plays a vital role in good health and well-being throughout your life. Getting enough quality sleep at the right times can help protect your mental health, physical health, quality of life, and safety. In children and teens, sleep also helps support growth and development. The damage from sleep deficiency can occur in an instant (such as a car crash), or it can harm you over time. For example, ongoing sleep deficiency can raise your risk for some chronic health problems. It also can affect how well you think, react, work, learn, and get along with others. Management: here are five tips everyone can use to help improve the quality of their sleep: keep your bedroom cool and dark put away/turn off all electronic devices while preparing for bedtime stick to a regular bedtime and wake time every day, even on weekends stop drinking caffeine by the early afternoon and avoid large late-night meals skip the late-afternoon nap, as it can make it harder to sleep at bedtime avoid taking unnecessary stress/ tension. Do regular exercise. Go for a brisk walk in the morning. Medication: take schwabe's bacopa monierri 1x/ thrice daily and kali phos./ once daily at night for 1month.

It is often said that the key to successful asthma treatment lies in the identification of its triggers and symptoms. It is important to know as much as possible about this chronic respiratory condition so that you can take appropriate action when required. The inflammatory disorder causes the airways to narrow and swell leading to difficulty in breathing and chest tightness. However, all asthma patients do not have the same set of symptoms. Apart from the classical signs of asthma, patients may also suffer from many other physiological changes which are difficult to track at times. But, for successful management, one should learn to identify the pattern of occurrence and chalk out a proper rating system. Apart from this, one should also be able to spot the triggering agents in order to avoid them.

Asthma – An Overview:

Asthma is defined as a chronic inflammatory condition of the lungs that may affect people of all age-groups. There is no complete cure for the condition and treatment is mainly aimed at correcting the symptoms and avoiding the triggering agents.

Symptoms: The symptoms of asthma can be broadly segregated under three sub-heads namely, classic, emergency and additional symptoms.

Classic Symptoms:

• Shortness of breath that worsens with activity or exercise.
• Cough – with or without sputum.
• Chest tightness experienced as something squeezing or sitting on your chest.
• Wheezing – characterized by a squeaky sound that occurs while breathing. Wheezing may come in episodes and may worsen during activity and cold weather. They may also exacerbate early in the morning or at night.

Emergency Symptoms:

• Extreme difficulty in breathing.
• Sharp – rapid pulses.
• Sweating
• Bluish coloration of the lips and face.
• Feelings of anxiety or panic
• Difficulty talking and inability to catch your breath

Additional Symptoms:

• Temporary stoppage of breathing
• Abnormal pattern of breathing associated with severe wheezing.
• Coughing that won’t stop
• Nasal flaring and chest pain

Factors that trigger asthma:

Cigarette Smoke: Smoking of any form – direct or passive - places a person at a greater risk for developing asthma. If you are pregnant, it is advisable to avoid smoking as this increases the likelihood of your baby developing the condition.

Dust and Mites: If you are allergic to dust mites, you should take proper measures to limit exposure. This is because they can trigger allergic reactions like asthma. Although, it is impossible to get rid of all the mites, you can probably cut down the number by taking certain precautionary measures. This involves reducing the dampness in your homes, preventing the accumulation of dust, frequently washing pillows, blankets and stuffed toys and using high efficiency vacuum cleaners.

Pets: Another major source of allergen that can trigger asthma are household pets, cats and rodents. An ideal way to control animal allergens is by preventing their entry inside the living space and thoroughly cleaning household furniture, carpets and walls at periodic intervals.

Pollens: A major form of airborne allergen – pollens can acts as a triggering factor during certain specific times of the year. Hence, it is seen that in some people, asthma gets worse during the pollen seasons (i.e. during the spring and early summer seasons). To reduce exposure to them, remain indoors during these seasons as much as possible and avoid any form of direct contact with these allergens. Also, make it a point to shower every time you have any kind of high exposure to pollens.

Occupational asthma: If you have asthma, it is advisable to refrain from working in any type of “high risk” industry that involves dust, grains or animals. For different people, the triggering agent may be different. Hence, always consult a doctor to help you identify the exact cause of the condition.

Infections: Asthma can also be triggered by certain respiratory infections and common cold. When asthma is caused by such health conditions, prompt treatment should be taken to limit the duration of illness and prevent further attacks.

Physical Exercise: A particular form of asthma, termed as exercise-induced asthma usually occurs after a few minutes of exercise in certain people. This is particularly noticeable in cold weather conditions and should be avoided by using appropriate medications a few minutes before exercising.

He is suffering with diabetes and don't want to leave sweets. How can I manage the things? Please help me.

MBBS, CCEBDM, Diploma in Diabetology

He is suffering with diabetes and don't want to leave sweets. How can I manage the things? Please help me.
the reason sweets should be avoided in diabetes is intake will increase blood sugar straight away. High blood sugar causes multiple problems affecting eyes (retina), small and large blood vessels, kidneys, heart, legs and feet (ulcers not healing, leading to amputation), severe neuropathic pain etc. Plus sweets also contain high fat content, increases caloric intake and may lead to weight increase. So if one has diabetes then there is no option but has to stop eating sweets. You have to convince him and impress that sweets will cause serious problem in the course of the disease. Another possibility is to use sucralose in stead of sugar while making sweets. But still gheee will be plenty.

What are the symptoms of dengue? My uncle has high fever from a week. Can it be dengue?
The principal symptoms of dengue are: High fever and at least two of the following: Severe headache/Severe eye pain (behind eyes) Joint pain/Muscle and/or bone pain Rash or Mild bleeding manifestation (e. G, nose or gum bleed, petechiae, or easy bruising) Watch for warning signs as temperature declines 3 to 5 days after symptoms/fever began, as this is the most dangerous period when platelet counts decline significantly. Report immediately to hospital if any of the following warning signs appear: Severe abdominal pain or persistent vomiting Red spots or patches on the skin Bleeding from nose or gums Vomiting blood Black, tarry stools (feces, excrement) Drowsiness or irritability Pale, cold, or clammy skin Difficulty breathing There is no specific medication for treatment of a dengue infection. Persons who think they have dengue should use analgesics (pain relievers) with acetaminophen (crocin or dolo -650 mg) and avoid those containing ibuprofen, naproxen, aspirin or aspirin containing drugs. Take rest, drink plenty of fluids like coconut water, lime water etc. To prevent dehydration. Preventing dengue fever: the best way to prevent the disease is to prevent bites by infected mosquitoes.Avoid visiting heavily populated residential areas, if possible. Use mosquito repellents, even indoors. When outdoors, wear long-sleeved shirts and long pants tucked into socks.
