Dhat Syndrome - How It Affects You?

While some diseases and medical conditions affect people all over the world, there are some that are prevalent only amongst a certain cultural group or region. Dhat roga or Dhat syndrome is one such culturally bound syndrome that affects men in the Indian subcontinent region including India, Sri Lanka, Bangladesh and Nepal. In some cases, it may also affect the women of this region.

The word ‘Dhat’ originates from the Sanskrit word ‘Dhatu’ meaning ‘elixir of the body’. The condition was first described in 1960 by Dr Narendra Wig as a condition marked by psychosomatic symptoms such as fatigue, anxiety and sexual dysfunction caused by the passing of a whitish fluid or whitish particles that are believed to be semen in the urine. Other symptoms that person with this condition might express include weakness, lack of appetite, poor concentration and guilt.

How dhat is different from sperm leakage and premature ejaculation?
Dhat is the name given to the collective symptoms, such as sexual, psychological, and physical related to the passing of a whitish fluid, believed to be semen in urine.  This is usually followed by psychological distress and anxiety due to semen-loss in patients. The word ‘dhat’ is derived from the Sanskrit word ‘dhatu’ which means ‘metal’ and/or ‘elixir’. Whereas, sperm leakage is a sexual dysfunction which happens due to weak parasympathetic nerves. What happens here is that seminal fluid oozes out of a flaccid penis. This is said to happen due to over masturbation. Dhat, on the other hand, is passing of seminal fluid in urine. And, premature ejaculation is something that happens at the time of coitus. In this, the male is unable to sustain an erection long enough for penetration and ‘comes’ prematurely. In this condition, penetrative sex doesn’t happen or happens for too short a period of time to satisfy the partner.

In the dhat syndrome, males usually believe that they have premature ejaculation and suffer from impotence apart from just leaking the white seminal fluid during urination. This loss frightens them into developing a sense of depression as the fear of semen loss is very strong in the subcontinent. Young men from a rural background or of a lower socioeconomic status typically complain of this syndrome. It may be further categorised under three heads:

  • Only Dhat: In this case, the patients present with hypochondriacally oriented symptoms attributed to the loss of semen
  • Dhat accompanied by anxiety or depression: In this case depression or anxiety is the core condition that may be accompanied by Dhat
  • Dhat accompanied by sexual dysfunction: In such cases, the patient may complain of erectile dysfunction, premature ejaculation or other psychosexual dysfunction conditions such as depressive neurosis, somatoform/ hypochondriasis or anxiety neurosis.

Symptoms reported

  • Fatigue and listlessness
  • Appetite loss
  • Lack of physical strength
  • Poor concentration and forgetfulness
  • Guilt
  • Sexual dysfunction

Common misconceptions about the disease
There are a number of misconceptions about dhat. Most patients think that the semen loss during urination will make them impotent or someway weaker sexually. This anxiety can be traced back thousands of years to certain Ayurvedic treatises in which the loss of a single drop of semen, the most precious body fluid, was enough to weaken the whole body. This cultural belief leads to a lot of stigma, guilt and depression related to dhat.

The morbid hypochondriacal fear that is associated with dhat syndrome is also stuff that myths are made of. Patients believe that dhat will irreversibly harm their bodies and they will be unable to produce male offspring, and that dhat will cause a death of offspring at an early age, lead to the birth of a malformed fetus; or cause anaemia, leprosy, tuberculosis, permanent impotency, and shrinking of the penis.

Factors apart from reproductive health which results in Dhat roga

  • Nerves are responsible for keeping the semen locked  in during a normal state in men. When nerves get weak, semen is discharged during urination or while doing normal activities in the form of a white discharge, called dhat.
  • This problem further strains other organs like liver and a poorly functioning liver increases weakness related to muscles and also reduces energy levels, fat metabolism, and contaminates blood.
  • Prostatitis, a disease of the prostate gland which secretes seminal fluids can also aggravate dhat.
  • A weak digestive system, prone to constipation and piles also causes dhat. This is seen as a white discharge while defecation.

Modern science has not been able to understand an organic evolution of this condition and hence has no form of medication to treat it. Hence, it is often considered a neurotic condition. According to Ayurveda, apart from excessive masturbation and a weak reproductive health, conditions such as weak digestion, constipation and prostatitis can also trigger the symptoms of Dhat roga.

Ayurveda believes that the body always functions as a complete entity and hence no condition can be isolated and treated. Hence when treating diseases, it approaches it through changes in the person’s diet, exercise and lifestyle, mental health and overall physical health. In the case of treatment for Dhat roga, it is important for the doctor to first correct the patient’s erroneous beliefs and listen to the person before medicating him.

  • Charaka samhita postulates that semen is all pervasive within the body like "oil in the sesamic seed" and suggests one ejaculation per week in summer and 168 total yearly ejaculations as the optimal sexual frequency for males.
  • Over-indulgence in sex and especially masturbation can cause an imbalance in the dhatus with a resultant physical illness called dhat.

Postpartum Hemorrhage - Know Why It Actually Happens!

Postpartum hemorrhage is a condition where a woman may lose a lot of blood (approximately 500ml to 1000ml) from her vulva shortly after childbirth. This usually occurs within the first 24 hrs of childbirth and can even result in severe blood loss threatening the mother’s life in case the blood loss is over 2000 ml. Secondary postpartum hemorrhage can also occur until 12 weeks after childbirth and this extra care should be taken, if there is any such possibility.

Symptoms of postpartum hemorrhage:
1. Rapid increase in heart rate.
2. Feeling dizzy while standing up.
3. Increase breathing rate.
4. Feeling cold and chilly even if the room temperature is normal.
5. Sudden fall in blood pressure levels.
6. Fainting or becoming unconscious.

Causes of postpartum hemorrhage:

  1. Uterine atony: This is a condition in which the uterus contracts and may lead to excessive bleeding. Infection in the placental tissue can also lead to postpartum hemorrhage.
  2. Trauma: This is a very common cause for postpartum hemorrhage. Sometimes, the uterus (womb), vagina, cervix (passage forming the lower end of the womb) and the perineum (area between the vulva and the anus) can get injured. These areas become vascular during the course of pregnancy and may rupture in the process of childbirth causing excessive bleeding.
  3. Tissue: At times, the whole placenta or fetus tissue does not come out of the body after childbirth. This can lead to profound blood loss resulting in postpartum hemorrhage.
  4. ThrombinThis is a disorder which occurs when there is excessive bleeding when the blood fails to clot inside the body.

How can you prevent postpartum haemorrage?

  1. Oxytocin is a substance which is used after the delivery of the baby. This prevents postpartum hemorrhage. It can be administered in the following ways :
    • As an injection which is directly injected into the blood stream.
    • After delivery, breastfeeding your baby can also trigger natural oxytocin. This occurs as encouraging the baby to suckle also stimulates the nipples which release oxytocin, thus preventing postpartum hemorrhage.
    • In the form of intravenous drips mixed in with important medications. IV drips can administer oxytocin along with other important fluids along with it.
  2. Uterine massage is also recommended to prevent postpartum hemorrhage. After delivery, massaging the uterus makes the muscles relax and contract. This reduces the risks of excessive bleeding.

The cornea is the transparent layer of the eye that acts as a protective shield. The cornea, in combination with the sclera, protects the eyes from various harmful microorganisms and particulate matter, including the harmful UV radiations (to some extent). The cornea is made up of three important layers, the Endothelium, Stroma, and the Epithelium.

  1. Endothelium: The inner layer of the cornea, the endothelium helps to maintain a clear vision by removing the excess fluid absorbed by the stroma. The mitochondria-rich single celled layer functions more like a pump.
  2. Stroma: The middle corneal layer is extremely thick (~90% of the total corneal thickness) and composed mainly of proteins and fluids. Like the endothelium, the stroma, too, plays a pivotal role in the normal eye vision.
  3. Epithelium: Being the outermost layer, the epithelium functions as a defensive shield, protecting the eyes from harmful germs, bacteria and any matter that can harm the eye. Most importantly, the epithelium absorbs the essential nutrients and oxygen (present in the tears). The cornea, thus, plays a pivotal role in the normal vision of the eye. However, an injury or an infection can interfere with the corneal functioning, affecting a person's vision as well as giving rise to other complications (the eyes may appear itchy, watery and red. There may also be light sensitivity).

Some of the common cornea problems include

  1. Keratitis: Injury or microbial (bacteria, virus or fungi) infiltration of the cornea can trigger this condition resulting in corneal infection and inflammation. Some of the symptoms characteristic of keratitis include extreme light sensitivity, blurred vision. The inflammation can also result in redness, pain (moderate to severe) and watery eyes. Antifungal, antiviral or steroidal eye drops may provide relief from the associated symptoms.
  2. Shingles (Herpes Zoster): This condition mainly affects people who have suffered from chickenpox. In some cases, the virus though inactive may still be present in the body (in the nerves). However, at a later period, certain factors may trigger its activation. Once active, the virus can affect different parts of the body, including the cornea. Shingles can cause corneal soreness and inflammation. Old people and those with a weak immunity are more likely to suffer from shingles. People with shingles may benefit from the administration of steroidal (tropical) and antiviral eye drops.
  3. Ocular Herpes: This is a viral infection triggered by Herpes Simplex Virus I (HSV I) or Herpes Simplex Virus II (HSV II) resulting in herpes of the cornea. The condition gives rise to corneal inflammation and sores. Here too, the patient can get some relief by using antiviral or steroidal eye drops.
  4. Keratoconus: This is a degenerative disorder of the cornea that results in thinning of the cornea. The condition can also alter the corneal shape, resulting in swelling, pain, and loss of vision (partial to complete, particularly the night vision).
  5. Fuchs' Dystrophy: This is a corneal dystrophy whereby the corneal endothelial cells break down at a slower rate than usual. As a result, the stroma may be waterlogged resulting in swelling and blurred vision.
