B12 deficiency and unsteadiness

Vitamin b12 deficiency is common in the population over 80 (about 10%). It can be a cause of unsteady gait (ataxia), sometimes accompanied by anemia (macrocytic), and loss of position sense. When associated with spinal cord disease, it is sometimes termed" subacute combined degeneration. The ataxia is called a" sensory ataxia, because it appears to be related to loss of position sensation from the feet. A list of diseases that can present with a similar picture is given below:

Subacute combined degeneration (b12 deficiency)
Tabes dorsalis (neurosyphilis)
Fredreich' ataxia (a cerebellar disorder), with damage with sensory input to the cerebellum.
Peripheral neuropathy (common)
Thoracic spinal cord lesions (very rare)
Subacute sensory loss (a rare paraneoplastic syndrome).
The term subacute combined degeneration was coined by russell, batten and collier in 1900 to describe pathological changes in the spinal cord. While changes in the posterior columns are emphasized in clinical accounts, pathologically the lateral and anterior columns are also often affected. The peripheral nerves are generally unaffected. B12 deficiency is a risk factor for cognitive decline (i. E. Dementia)

Causes of b12 deficiency

Decreased intake (e. G. Vegetarians)
Malabsorption
Autoimmune (pernicious anemia)
Stomach and small bowel disorders (e. G. Gastritis, surgery)
Medication interaction (e. G. Metformin, medications that reduce stomach acidity)
Over consumption (blind loop)
B12 is available only from animal sources and thus strict vegetarians are at a risk of deficiency. B12 is bound to animal protein and released by gastric acid. When taken as part of food, b12 is released from food by a combination of gastric acid and pepsin (an enzyme).

A medication for diabetes, metformin, reduces absorption of b12.

Gastric conditions contributing to b12 deficiency include various stomach diseases that impair release of b12 from food

Pernicious anemia (pa) accounts for 15-70% of b12 deficiency. This an autoimmune disorder where antibodies are made to intrinsic factor. Oral supplements are less effective in pa than other modes of administration.

Diagnosis of b12 deficiency

B12 deficiency is usually detected through a b12 blood level. Typical lower limits of normal are 200 pg/ml. A cbc test may show macrocytic anemia.

Because b12 is stored in the liver, it takes about 2 years following a sudden cessation of intake for signs of deficiency to arise.

Treatment of b12 deficiency

There are presently a variety of options.

Oral therapy involves giving 1 to 2 mg (1000 mcg) of b12 daily. Oral therapy is usually as effective as the others noted below
Monthly injections of b12 are traditional. The first injection is generally 1000ug, and subsequent ones are 100.
Nasal b12 is also now available as an alternative to injection in persons who do not respond to oral therapy.

Hi When the temperature is too low, blood circulation reduces and causes paleness of skin and numbness too. Avoid exposure to cold, keep yourself warm ,do not take tobacco or coffee as nicotine and caffeine decrease blood circulation. Drink lots of water to prevent dehydration. Check your thyroid levels also.

Hi, 1) have frequent meals 5-6 meals per day 2) eat healthy like fresh vegetables and fruits and whole grain cereals, pulses, skimmed dairy products, nuts, egg white, chicken (2 times in a week if you are non veg), fish 2-3 times in a week (avoid fatty fish). 3) drink water before eat meals 4) do your house chores work or do physical activity 30-45 minutes every day. 5) avoid oily snacks, processed, refined and junk foods. Eat small but frequent meals. Avoid long gaps in between meals. Avoid junk as far as possible. Exercise whenever you get time. Lastly be committed. Remember it's a lifestyle change and not a one time thing. There is no short cut. It's a balanced intake that will last you longer. All the so called diets will only give you muscle and bone loss. The fats will remain as it is. So, try to first follow this seemingly simple tips. I am sure this suggestion will at least help you with initial fat loss.

Hye, thanks for the query. Firstly understand that muscle gain is very dependent on genetics and metabolism along with your daily diet and exercise routine. It is important to know that just eating just two heavy meals a day will not help. Muscle gain is all about taking more nutrients than you use up. Eating the right foods at the right time is very important. There is no one food that helps muscle gain. Eat meals at frequent intervals. You need to eat at least 6 meals a day. Eat a protein rich mid meal between each main meal. Pre/ post workout meals need a certain amount of carbs and proteins depending on your exercise routine. Avoid junk, sugary, oily foods. Include dairy, eggs, nuts, fruits, dry fruits, vegetables, whole grain cereals, smoothies, masala milk, healthy oils in your diet. Restrict eating sugary, sweet foods. Try to have healthy deserts like kheer, chikki or lassi. Avoid alcohol and smoking. Exercise at least 6 days a week. It helps development of muscle mass and improves appetite. Even if you were to take just medicines or supplements they will not work in isolation. You need to have a nutrient dense meal and good exercise routine, it's crucial.

Firstly weight gain is majorly dependent on your metabolism and genetics play a definite role too. You have to be patient and give your body time to gain a healthy body weight. Eat meals at frequent intervals. Eat a mid meal between each main meal. Avoid junk foods. Include dairy, eggs, sprouts, fruits in your meals. Avoid alcohol and smoking. Exercise at least thrice a week. Even if you were to take medicines or supplements. They will not work in isolation. You have to have a calorie and nutrient dense meal.

Hi lybrate-user Eat with your eyes and your mind Make sure your dinner table has a spread of colours like yellow, green, red, orange, white. Add a lot of different fresh seasonal vegetables, raw salads and herbs. This will not only be pleasing to your palate by providing a burst of flavour but will also give it an aesthetic appeal, so the meal doesn’t look boring and monotonous. While you sit to eat, do not just shove the food in your mouth and gulp it down. The idea is to go slow, and enjoy the entire experience, relish your food as you chew it properly to enjoy every flavour in every bite. Focus on what you are eating the texture, the visual appeal, the aroma of the dish. Eating mindfully will make it an enjoyable experience while assisting in better digestion, make you feel more fuller by avoiding overeating and prevent a lot of stomach distress like gas, bloating or acidity Take small portion Eating a smaller portion of food at a time helps in the breakdown of the food in an effective way so that the entire calories obtained from the food are used up by the body for all the bodily functions. When you eat a large meal, the excess energy which goes unused by the system gets stored as fat and causes weight gain. A good idea would be to use smaller plates so the plate looks fuller and psychologically makes you feel that you have eaten enough while eating from a larger plate makes you pile up your plate with more food which in turn makes you eat more calories. Fill up your stomach only up to 80% We have been raised to eat until we are absolutely full so we don’t feel hungry later. Either that or because traditionally Indian consider it a taboo to leave any food on the plates so we eat off the kids plates because he/she won’t eat anymore, also we have seen our moms finish the leftover paratha or grab an extra spoon rice even if they are not too hungry, just so that they don’t have to throw it away as they are usually the last ones to eat. Not realising how much damage we are doing to our bodies by eating more than what our body needs. However, it is better to stuff ourselves and only eat until we feel adequately full. Yes, you will feel hungry after few hours and that ok. It only means less pressure on your intestines and delaying the ageing process of the cells which can prevent heart attacks, diabetes and cancer. Eat small snacks every few hours. Having said that eating till you are 80% full will make you hungry for more, it is a good practice to eat 5-6 meals a day instead of the traditional 3 meal pattern of breakfast, lunch and dinner. Eat a healthy snack like a fresh whole fruit or nuts like almonds, walnuts, unsalted peanuts, pumpkin seeds, low fat curd or unsalted chaas, hard boiled eggs between main meals every 2-3 hours. This will help to keep you burning more calories throughout the day as the digestion process continuous to keep your metabolism up and running. Also, your body will thank you for feeling light and energetic throughout the day because of the regular supply of an array of nutrients and maintaining your blood sugar levels Have a light supper or dinner As they say, Eat breakfast like a King, Lunch like a commoner and dinner like a popper it is important to start the day with a filling wholesome breakfast to kick start your metabolism and taper down your meals as the sun goes down. Your last meal should happen atleast 3 hours before your bedtime so the body has enough time to digest the meal properly before you go to bed. If you eat a heavy meal too close to bedtime, the body cannot digest it properly leading to indigestion, acid reflux, disturbed sleep making you wake up next morning feeling sluggish and tired. As the day goes the body tends to slow down and to go into a repair and recovery mode to heal the body from the days stress and wear and tear. Also, since we are not physically active in the night time all the excess calories get stored as fat in the body Eat vegetables first Vegetables are full of fibre and when you consume them first, this fibre helps to cover whatever is eaten after and inhibits insulin spikes and the speed at which sugar is transported into the blood. Proteins such as fish, chicken, paneer or sprouts are harder to digest when they are eaten first and everything else eaten after would take time making you feel bloated. Raw vegetables contain a digestive enzyme that helps break down proteins. Eat more fiber Eating more fiber, makes feel fuller, can increase satiety and makes you lose weight. No its very wrong, you r not taking food in night. You should take low calorie diet in night and focus in high fiber in daily diets and don't take out side foods and take proper water. Avoid fried foods ans prefer fruits and fruits juice in daily diets.

Dear patient, calorie dense food should be incorporated in your diet like dried fruits, potato, sweets, nuts, jam jelly, banana or mango shake, porridge, badam kheer etc. leafy vegetables should be restricted in your diet as they reduce appetite. you should take 3 major meals a day and snacks in between. butter, margarine should be taken. Adequate amount of egg, meat, fish, milk should be taken as it help you for tissue building in your body.try to take 1 boiled egg at every morning. you should eat more food at each meal. Before taking meal you can walk for sometime as it help to you feel hungrier. Thanks...

  • Do give your child snacks that are low in sugar like vegetables, fruit cheese, or cereals. These do not cause tooth decay like sweet snacks do.
  • Do limit snacking between meals. When eating many snacks, the acid in the mouth is high. High acid levels can help make a cavity.
  • Do limit the eating of soft, sticky sweets that get on and in between the teeth, like chewy candy or raisins. Sticky foods stay in the mouth longer and the acid can decay the tooth.
  • Do brush your teeth after eating. It is important to prevent getting cavities. Wait 30 minutes after eating before you brush.
  • Do try to avoid sugary foods like gum, hard candies, and suckers that stay in the mouth for a long time.
  • Do reduce the damage that can be caused by sweets by serving sweets with other foods or drink. Saliva or water in the mouth helps reduce the damage sugar can cause to the teeth.

• Get adequate sleep and rest.
• Avoid rubbing your eyes too much or too often as it causes the blood vessels to dilate, thus making the under eye circles appear darker.
• Remove your make-up before going to sleep or else it may irritate your eyes.
