You are asking about PE. Ejaculation of semen during sexual intercourse before or immediately after penetration. Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common sexual complaint. Estimates vary, but as many as 1 out of 3 men say they experience this problem at some time. As long as it happens infrequently, it's not cause for concern. The primary symptom of premature ejaculation is the inability to delay ejaculation for more than one minute after penetration. However, the problem may occur in all sexual situations, even during masturbation. Premature ejaculation can be classified as lifelong (primary) or acquired (secondary). Lifelong premature ejaculation occurs all or nearly all of the time beginning with your first sexual encounters. Acquired premature ejaculation has the same symptoms but develops after you've had previous sexual experiences without ejaculatory problems. Many men feel that they have symptoms of premature ejaculation, but the symptoms do not meet the diagnostic criteria for premature ejaculation. Instead these may have natural variable premature ejaculation, which is characterized by periods of rapid ejaculation as well as periods of normal ejaculation. Psychological causes Some doctors believe that early sexual experiences may establish a pattern that can be difficult to change later in life, such as: •Situations in which you may have hurried to reach climax in order to avoid being discovered •Guilty feelings that increase your tendency to rush through sexual encounters Other factors that can play a role in causing premature ejaculation include: •Erectile dysfunction. Men who are anxious about obtaining or maintaining an erection during sexual intercourse may form a pattern of rushing to ejaculate, which can be difficult to change. •Anxiety. Many men with premature ejaculation also have problems with anxiety — either specifically about sexual performance or related to other issues. •Relationship problems. If you have had satisfying sexual relationships with other partners in which premature ejaculation happened infrequently or not at all, it's possible that interpersonal issues between you and your current partner are contributing to the problem. Biological causes A number of biological factors may contribute to premature ejaculation, including: •Abnormal hormone levels •Abnormal levels of brain chemicals called neurotransmitters •Abnormal reflex activity of the ejaculatory system •Certain thyroid problems •Inflammation and infection of the prostate or urethra •Inherited traits •Nerve damage from surgery or trauma (rare) Common treatment options for premature ejaculation include behavioural techniques, topical anaesthetics, oral medications and counselling. Keep in mind that it may take a little time to find the treatment or combination of treatments that will work for you. Behavioural techniques In some cases, therapy for premature ejaculation may involve taking simple steps, such as masturbating an hour or two before intercourse so that you're able to delay ejaculation during sex. You may try avoiding intercourse for a period of time and focusing on other types of sexual play so that pressure is removed from your sexual encounters. You can also try diverting your mind before ejaculation is ready to come and proceeding after a brief break, thereby prolonging duration of intercourse / action The pause-squeeze technique You and your partner can use of a method called the pause-squeeze technique. This method works as follows: 1.Begin sexual activity as usual, including stimulation of the penis, until you feel almost ready to ejaculate. 2.Have your partner squeeze the end of your penis, at the point where the head (glans) joins the shaft, and maintain the squeeze for several seconds, until the urge to ejaculate passes. 3.After the squeeze is released, wait for about 30 seconds, then go back to foreplay. You may notice that squeezing the penis causes it to become less erect, but when sexual stimulation is resumed, it soon regains full erection. 4.If you again feel you're about to ejaculate, have your partner repeat the squeeze process. By repeating this as many times as necessary, you can reach the point of entering your partner without ejaculating. After a few practice sessions, the feeling of knowing how to delay ejaculation may become a habit that no longer requires the pause-squeeze technique. Topical anaesthetics Anaesthetic creams and sprays that contain a numbing agent, such as lidocaine or procaine, are sometimes used to treat premature ejaculation. These products are applied to the penis a short time before sex to reduce sensation and thus help delay ejaculation. A lidocaine spray for premature ejaculation is available Although topical anaesthetic agents are effective and well-tolerated, they have potential side effects. For example, some men report temporary loss of sensitivity and decreased sexual pleasure. In some cases, female partners also have reported these effects. In rare cases, lidocaine or procaine can cause an allergic reaction. Oral medications Many medications may delay orgasm. Although none of these drugs is specifically worthwhile as people believe, approved medications may be prescribed for either on-demand or daily use, and may be prescribed alone or in combination with other treatments to treat premature ejaculation, some are used for this purpose, including antidepressants, analgesics and phosphodiesterase-5 inhibitors You can ask me personally if you want these tablets with your full health details.

Three doses of hib vaccine is usually given in combination with dpt and/or hepatitis b. Cost depends on brand used and doctor's fees and could be around one thousand rupees. Other vaccine for meningococcal meningitis is used in high risk babies or those who demand and who can afford. All require 3 doses for primary immunisation at one month interval and a booster dose after one year

Depression In Adults - 5 Signs To Help You Recognise!

Depression In Adults - 5 Signs To Help You Recognise!

Alternating between being happy and sad as you go through life's ups and downs is normal, but if you're sad most of the time, it could be a sign of being depressed.  Depression takes the joy out of all your activities, and can make merely getting through a day feel overwhelming. The first step to overcome this problem is to understand its symptoms and causes.  Not everyone experiences depression the same way, but here are a few questions that can help tell you if you are depressed or not.

Do you feel helpless?

It is common for depressed people to feel helpless and that everything around them is out of their control. As a result, the person may either stop participating in activities all together or try to control things they can't. Both situations are cyclic and lead to further depression.

Have you lost interest in daily activities?

The feeling of helplessness is one of the main reasons depressed people lose interest in activities. Another reason is that activities that were once enjoyed now do not seem interesting enough. In most cases, this leads to the person isolating himself and withdrawing from all his family and friends.

Do you often feel angry or irritated?

Depression is almost always accompanied by irritability and anger. Small annoyances that would usually be overlooked quickly, take on an inappropriate significance. This can lead to poor impulse control, substance abuse and anxiety disorder. In most cases, anger and depression are related by the person trying to take accountability for things that are out of his control and hence feeling helpless.

Is there a change in your sleep pattern?

Your sleep quality is very closely related to the way you feel. Depressed people often suffer either from insomnia or oversleeping. Depressed people can have trouble going to sleep as well as staying asleep. This can cause drowsiness and lethargy during the day. Sleep apnea is also linked with depression. As you can imagine, if this is left untreated, it will only worsen your depression.

Do you feel tired?

When the body does not get adequate sleep, you begin the day feeling tired. Thus depressed people often feel fatigued and complain of a number of muscle and joint pains.  This is because the body has not had enough time to regenerate cells during the night. This is usually complemented by complains of persistent headaches and stomach aches, both of which are emotionally connected to the way you feel.

The nature of depression is such that it lowers a person's self esteem to the point of interfering with his ability to seek help. Hence if you have a positive response to the above questions, it is time to address your problem before it worsens. If you wish to discuss about any specific problem, you can consult a Psychologist.

Piles are basically caused when you are constipated. Don't eat oily or fried foods. Have healthy foods like fruits, green leafy vegetables, plenty of salads with every meals. You should not be constipated. Make sure you do brisk walk daily for 45 mins. Take plenty of water. Never exert during motion. Consult a surgeon locally if it's not helping.

Dear, follow methods by which you can gain your desired height. 1. Follow these exercises regularly and you get the goal of height gain: * first stand straight bare foot on floor, then raise your arms upwards then raise your heels of foot with breathing in, try to stand on heels for 10-12 seconds and slowlylower down your heels back on the floor and breath outthe air. Repeat this exercise 5-8 times in morning. *sit on the floor, stretch you legs together so that both foot touch each other. Now raise your arms upwards together with back straight, now move your hands towardstoes of feet and touch them. Your forehead touching your knees. Slowly revert back the arms and move back arms in normal position. Repeat this exercise 2-4 times daily. You can privately consult me for understanding more exercises. 2. Take 300 ml of rich cream milk, 200 gm of cheese and 2-3 eggs regularly. It will definitely give boost to your bone growth. 3. Please note you can wait for 3-4 days, because as your hpt is negative so you might get your periods. If after 3-4 days you did not get your periods then do a blood pregnancy test which is more confirmative.

I am 23 weeks and 4 days pregnant. My USG reports are all normal and baby measurements are also as per the time frame. Though I can't still distinguish the kicks, roll ups etc, I can feel the baby movement as well. However I don't have any baby bump. I'm still my normal self. How is that possible? When will I show up? Also, I have to travel almost 32 km each side to and from my office. The road conditions is not so good like rest of the part of Kolkata. Can this harmful movement everyday affect the baby? When would it be ideal to the break from office?

Your pregnancy bump will gradually increase in size from 6 months onwards. Traveling is generally okay if your pregnancy is progressing normally. There is nothing absolute; however as your pregnancy advances you may start feeling tired and may not be able to travel such a long distance. You will have to then decide to take leave say from 32-36 weeks onwards.
