The mainstay of treatment is timely supportive therapy to tackle shock due to hemoconcentration and bleeding. Close monitoring of vital signs in critical period (between day 2 to day 7 of fever) is critical. Increased oral fluid intake is recommended to prevent dehydration. Supplementation with intravenous fluids may be necessary to prevent dehydration and significant concentration of the blood if the patient is unable to maintain oral intake. A platelet transfusion is indicated in rare cases if the platelet level drops significantly (below 20, 000) or if there is significant bleeding. The presence of melena may indicate internal gastrointestinal bleeding requiring platelet and/or red blood cell transfusion.

Fever is not considered medically significant until body temperature is above 100.4 F (38 C). Fever serves as one of the body's natural defences against bacteria and viruses which cannot live at a higher temperatures. Also, the body's defence mechanisms seem to work more efficiently at a higher temperature -Fevers of 104 F (40 C) or higher may be dangerous and demand immediate treatments like: -Acetaminophen (dolo-650 mg) twice or thrice can be used to lower a fever . Aspirin should not be used for fever in children/ adolescents or during a viral illness. -Wear loose fitting clothes and don’t overdress. Overdressing can cause temperature to rise further. -Tepid water (85 F [30 C]) bathsmay help bring down a fever. -The body should be cooled with a wet sponge. -A fever can cause severe dehydration. Drink plenty of fluids.. Have citrus fruits, particularly Vitamin C rich ones like mausami,Amla juice etc. -Take light and easily digestible home cooked meals .avoid outside food -Wash hands frequently with soap and water especially after returning from public places or public transport. -Further therapy depends on the cause of fever like urinary infection/typhoid or malaria/dengue infection and the accompanying symptoms that need to be investigated and treated accordingly

As per details mentioned by 3 minutes is considered to be normal and will not come under premature ejaculation as in PE there is inability to delay ejaculation for more then one minute after penetration.There is no medicine which can increase size of penis after a certain age,hence not recommended.

Problems Related To Digestion!

The human digestive system is a very complicated and sizeable component of the body. It starts right at the beginning of the mouth, ending at the rectum. Some of the major responsibilities of the digestive system include absorption of essential nutrients and elimination of wastes.

Not only do digestive problems have embarrassing and unwanted symptoms, serious, chronic diseases can be caused if the minor problems are not treated.

Some digestive problems are listed below:

  1. GERD: This is a burning sensation that results from acids in the stomach being squirted into the oesophagus. A burning sensation is felt, right from the chest to the end of the throat. Frequent heartburns can lead to GERD (Gastroesophageal Reflux Disease), symptoms of which include dry cough, sore throat, chest discomfort and difficulty in swallowing.
  2. Bloating: This is caused due to the presence of excess gas in the colon, small intestines or the stomach.
  3. Dyspepsia: This is an uncomfortable feeling or pain which occurs in the upper abdominal regions, mostly a result of excess food intake. It is commonly referred to as indigestion. Dyspepsia can be discomforting and if prolonged, medical assistance should be opted for.
  4. Chronic Constipation: Persistent constipation (chronic) is a sign that there is a problem with the elimination of wastes. This usually happens when the colon is unable to move or pass stools. Other major causes of chronic constipation can be changes in your usual diet, various eating disorders, Irritable Bowel Syndrome and excessive consumption of dairy products among others.
  5. Food Intolerance: This occurs when the digestive system is unable to tolerate certain food items. This condition is different from food allergies as it only affects the digestive system. Some of the common symptoms include diarrhoea, headache, gas and vomiting.

Possible Serious Conditions-

A gastroenterologist is a type of doctor that specializes in diagnosing and treating diseases involving the digestive system. If you continue to experience digestion problems, it’s time to make an appointment.

More serious signs could mean an emergency medical problem. Those signs include:

  • bloody stools
  • continuous vomiting
  • severe abdominal cramps
  • sweating
  • sudden, unintentional weight loss

These symptoms could be an indication of an infection, gallstones, hepatitis, internal bleeding, or cancer.

Medications-

Please follow the advice given below. 1. Take a break-use 20-20 rule, take a break for 20 second after working in computer for 20 minutes and look at some thing 20 feet away. 2. Blink frequently to prevent dryness of eyes 3. Use of computer glass- it optimises your eye sight when you are looking at digital screen and helps to decrease glare 4. Keep your monitor bright- this decreases flicker 5. Use proper lighting like incandescent light 6. Check position of monitor- monitor should be placed 4-5inches below eye level and 20-28 inches from eyes 7. Adjust, screen resolution 8. Minimise glare- clean monitor regularly to remove dust and place an anti glare screen 9. Try massage or eye cupping- massaging the area around the eye will relax eye muscle. Rub your hands together to create friction and warmth, then gently cup your palm over the closed eye and rest them 10. Take multivitamin tablet containing vitamin a, c, e, b complex, zinc and antioxidant (one capsule of revital daily after breakfast) 11. Go for yearly eye examination.

You should change your working pattern. If you have to sit longer in office then stand-up intermittently and take a short walk come back then engage to your work again. It means don't fed with work with some change of method in doing your work. Do exercise in morning and in evening even if you arrange 15 minutes but do work out daily take healthy diet full of salads, juices, milk, fruits etc. And ayurvedic medicines. You will see a sea change in you strength.

get a xray done to exclude any injury to the bone or cartilages of the joint must have taken a TT injection?the transparent fluid may be some body fluid coming outbpfrom the injury so getvthecwound attended to regularly. do not wetbthe knee joint or injury site . if necessary btakeva cour s ebof antibiotics in consultation with a doctor near your locality.

Mr. lybrate-user, Thanks for the query. To avoid developing hemorrhoids (piles), one has to take following precautions: Take a great deal of green leafy vegetables, salads and fruits. Use whole grain products to get adequate fiber content. Avoid highly spicy foods, foods with too much masala. Drink plenty of water, other fluids also. Develop good bowel habits, do not postpone visit to toilet when there is an urge. Prevent developing constipation. Some times in spite of all precautions the condition can still occur. Secondly joint pains or arthritis can be due to multiple reasons. Depending upon the cause treatment may differ. But mostly disease like Gout, rheumatoid arthritis and osteoarthritis are not curable. Hello- Masturbation is an addiction. Any addiction if stopped suddenly might cause health hazards. So it is advisable to stop masturbation gradually and finally you will over come from that habit without doing harm to your health in a safe manner. And yes masturbation is not healthy , this is the reason why you can see lots of men asking questions in Lybrate about erectile dysfunction which they faced after the habit of masturbation.

It could be due to a narrow opening in foreskin or due to adhesions underneath foreskin or simply due to smegma (white cheesy substance) accumulated under the foreskin that stick to the skin and prevent retracting it. Please visit your local doctor/surgeon, who - depending on the situation - will give a small incision on the foreskin or circumcize it or just give suggestions on how to carefully pull the skin back. As such, sex in pregnancy is not forbidden medically. Yes. If she had a healthy pregnancy, there’s no reason why you and your partner can’t have sex. The thick mucus plug that seals your cervix helps to guard your baby against infection. The amniotic sac and the strong muscles of your uterus (womb) also keep your baby safe. Your penis won’t damage your baby in any way while you're having sex. You may find your baby moves around a bit after you've had an orgasm. It's nothing to worry about. Your baby's reacting to the pounding of your heart. He doesn't know what's happening or feel any discomfort. You may have heard that having sex can bring on labour before your baby is ready to be born. This is not true, as long as you have had a straightforward pregnancy. If your wife's body is not ready to go into labour, having sex will not cause her to have a premature birth.
