The Glycemic Index and Diabetes

The glycemic index is a measure of food containing carbohydrates and how it can raise the level of glucose in blood. This is essentially a process where every food is ranked based on a reference food such as white bread. A food with a high GI value increases the level of glucose in the blood compared to a food with a GI that is medium or low. Some of the common examples of food with a low GI include legumes and dried beans. Fats and meats are not included in the index due to the non-availability of carbohydrate in them.

Some common foods with a GI of 55 or less include bread made up of pumpernickel and whole wheat, muesli, oatmeal, bulgur, barley, pasta, yam, lentils, sweet potato, converted rice, corn, non-starchy fruits, and vegetables. A food set with medium GI in the range of 56 to 69 includes quick oats, basmati rice, pita bread and couscous. Some example of foods with high GI value in the range of 70 includes pineapple, russet potato, white bread, macaroni, puffed rice, corn flakes, rice cakes and melons.

What influences GI?

Since GI has nothing to do with fibre and fat, some general findings of the GI count are as follows:

  1. When a food is processed or cooked, the GI tends to increase

  2. More the storage time of the food, the higher is the GI. It is applicable for ready to cook food and frozen food as well.

  3. The higher is the GI count, more the ripeness of a fruit or vegetable.

  4. Converted food items tend to have a lower GI as compared to the original version of the food

What are the other considerations?

While the GI value gives first-hand knowledge about the type of carbohydrate a person is consuming, it is hard of any help when it comes to the amount of carbohydrate intake. Portion size, therefore, still plays an important role for patients suffering from diabetes. The GI count of a food item greatly varies when combined with a food which has higher GI or lower GI for that matter. Nutritious food that is extremely beneficial for the body, tends to have a higher GI count. For instance, the GI count of oatmeal is greater than that of a chocolate.

Which one is more effective?

While there is no hard and fast rule for maintaining the carbohydrate count, an approach that is equally balanced between a GI count and carbohydrate count works best for a patient. Both the type of carbohydrate as well as the count of carbohydrate plays a crucial role in keeping the blood sugar level under control. If you wish to discuss about any specific problem, you can consult an Endocrinologist.

Hi, Dengue is caused by the bite of female aedes mosquito which carries the dengue virus. Symptoms of dengue are high grade fever, headache, bodyache, retro orbital pain, skin rashes, vomitting, abdominal pain, fatigue and restlessness. You can go for a blood test called Dengue screening to rule out Dengue. Continue taking tab. Paracetamol 3-4 times a day till the fever comes down.

What Is Heartburn - सीने में जलन क्या है

हार्ट बर्न या सीने में जलन का कारण है एसिड रिफ्लक्स या अम्लपित्त. हार्ट बर्न या सीने में जलन की बिमारी को समझने के लिए हमें भोजन के पचने की प्रक्रिया को समझना होगा. जब हम भोजन को मुंह में लेते हैं और उसे चबाना शुरू करते हैं तो हमारे मुंह में उपस्थित लार भोजन में से स्टार्च को छोटे-छोटे अणुओं में तोड़ने लगती है. इसके बाद भोजन, आहार नाल से होता हुआ पेट में प्रवेश करता है. यहाँ पर पेट की अंदरूनी परत भोजन को पचाने के लिए स्टमक एसिड जैसे पाचक रसों का निर्माण करती है.

क्यों होती है सीने में जलन?
अब यहाँ दरअसल होता ये है कि कई लोगों में लोवर इसोफैगियल स्फिंक्टर (एलईएस) ठीक से बंद नहीं हो पाने के कारण पेट का एसिड वापस बहकर इसोफैगस में चला जाता है. जब एसिड इसोफैगास की दीवारों से टकराता है तो छाती में दर्द और तेज जलन महसूस होता है.

क्या है हार्ट बर्न के लिए जिम्मेदार एसिड रिफ्लक्स?
एसिड रिफ्लक्स या उस प्रक्रिया का नाम है जिसमें एसोफेगस से स्टमक एसिड वापस पेट में लौट जाता है. अक्सर ऐसा तब होता है जब आपने काफी समय से कुछ खाया न हो यानी कि आ खाली पेट हों. जब आप बहुत देर तक खाली पेट रहते हैं तो एन्ज़ाइम और एसिड पेट के खाने को पचने नहीं देते हैं जिसके परिणामस्वरूप एसिड बनने लगता है. इसकी वजह से लोअर इसोफेगल स्पिंचर (एलईएस) ठीक से काम नहीं करता है. फिर ग्रासनली, एसिड को पेट से ऊपर की ओर धकेलती है.
आप ये भी कह सकते हैं कि एसिड रिफ्लक्स का एक कारण लोवर इसोफैगियल स्फिंक्टर का खराब होना या ठीक से काम न कर पाना भी है. इसकी वजह से कई बार ऐसा भी होता है कि पेट में भोजन को पचाने वाला एसिड गले तक वापस चला आता है. ये इतना खतरनाक है कि इससे गले में घाव पैदा हो जाता है. कई बार इससे सांस लेने में दिक्कत, आवाज भारी हो जाना, और मुंह में छाले जैसी समस्याएं भी हो जाती हैं.

क्या अन्तर है एसिड रिफ्लक्स और हार्ट बर्न में
अब यहाँ ये सवाल उठना तो लाजिमी है कि आखिर एसिड रिफ्लक्स और साइन में जलन में अंतर क्या है? तो यहाँ हम आपको ये बताना चाहेंगे कि एसिड का इसोफैगस में पहुंचना एसिड रिफ्लक्स है. ऐसा होने पर दर्द नहीं होता है. लेकिन जब इसोफैगस की अंदरूनी परत नष्ट हो जाती है और इसकी वजह से छाती के बीच में दर्द, जकड़न और बेचैनी होती है. इसे हार्ट बर्न कहते हैं.
यहाँ ये बात समझना भी जरुरी है कि एसिड रिफ्लक्स बिना हार्ट बर्न के भी हो सकता है. लेकिन हार्ट बर्न बिना एसिड रिफ्लक्स के नहीं हो सकता है. इसे यूँ समझें कि एसिड रिफ्लक्स कारण है और हार्ट बर्न उसका प्रभाव है. आपको बता दें कि जब सामान्य से अधिक मात्र में एसिड का स्राव होता है तो उसे जोलिंगर एलिसन सिंड्रोम कहते हैं.

इन लक्षणों से हार्ट बर्न को पहचान सकते हैं
देखा जाए तो इसके नाम में ही इसका लक्षण है. लेकिन इसके साथ ही हर बिमारी के कुछ प्रारंभिक लक्षणों के साथ ही कई और लक्षण भी होते हैं. तो आइए देखें इसके और लक्षण-

1. गले में इन्फेक्शन
अब आप कहेंगे कि पाचनतंत्र की समस्याओं का भला गले में खराश से क्या लेना-देना. तो हम कहेंगे कि ये अजीब भले लगे लेकिन है लॉजिकल. यदि आपको सर्दी जुकाम जैसी कोई समस्या नहीं लेकिन गले में दर्द हो रहा है तो इसका कारण हार्ट बर्न हो सकता है.
2. चक्कर आना
चक्कर आने का कारण आमतौर पर हम कमजोरी को मानते हैं. लेकिन जरुरी नहीं कि हर बार ऐसा ही हो. चक्कर आना भी हार्ट बर्न या एसिड फ्लक्स का एक कारण हो सकता है.
3. मुंह से लार का अधिक स्राव
मुंह में लार का बनना या स्त्रावित होना एक स्वाभाविक प्रक्रिया है. यदि ये सामन्य रूप से बन रहा है तो कोई दिक्कत नहीं है. लेकिन यदि मुंह में अचानक लार का स्राव बढ़ने लगे तो ये एसिड रिफ्लक्स हो सकता है.
4. नॉन कार्डिएक चेस्ट पेन
हार्ट अटैक के बाद होने वाला छाती में दर्द जिसको कार्डिएक चेस्ट पेन कहा जाता है. इसके लक्षण हैं - पसीना आना, सांस फूलना आदि. लेकिन जब इन लक्षणों के बीना छाती में दर्द हो तो यह कई बार यह दर्द फूड पाइप की वजह से भी हो सकता है. इसको नॉन  कार्डिएक चेस्ट पेन कहा जाता है. हलांकि दोनों में ज्यादा फर्क करना मुश्किल है इसलिए बेहतर है कि डॉक्टर को दिखाएँ.

No. I recommend the following 1. Find out how much of protein you are losing in the urine. Protein in urine rises the creatinine. tablets like ace/arb can reduce protein in urine 2. Avoid red meat 3. What you have is called dysphagia. Patients usually complain of dysphagia to both solids and liquids. Dysphagia to liquids, in particular, is a characteristic of achalasia. Other symptoms of achalasia include regurgitation, night coughing, chest pain, weight loss, and heartburn. But there are many other things which can cause dysphagia. But yes the problem is in your food pipe. Which needs to be checked. You need to get a upper GI endoscopy done and consult a Gastroenterologist asap.
