I have frozen shoulder. Also I feel pain at different places of my my arm. During night, it starts hurting at some random location on my arm, in whichever posture I try to sleep. The point of pain goes on shifting with changing the posture. My Vitamin D report says it is 8.6 ng/ml. My doctor prescribed me a dose of 60,000 IU of Vitamin D3 (1 gm cachet) every alternate day for 2 months. I searched the Google for the dose of this medicine. On almost all the sites, the recommended dose is 60,000 IU once per week or per month. Please advise me the correct dose.

As per your report your D3 is very low. Take it weekly twice. And apart from that, Early morning sun light exposure for 30mt. Daily one egg. Fish is better than chicken or mutton. Green veg and fruits would be help. Recheck after 3 months.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Actually pain at calf/leg region can be due to many reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you - serum vitamin d and serum potassium levels, esr and crp once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance. Thanks & regards

Hi, Apply Bakson’s Rheum Aid oil gently (2-3 times a day) and take orally Bakson’s Rheum-Aid syrup/ 1 tablespoon thrice daily. Also, increase your dietary intake of calcium which is present in green leafy vegetables, milk, milk products, sea food, white legumes, dried fig, black-eyed peas, almonds, oranges, seasame seeds. And vitamin D which is essential for absorption of calcium in our body and is synthesized naturally after getting exposed to sunlight, so early morning sunlight for 10-15min. 2-3 times a day is mandatory. Apart from this, do walk daily along with physiotherapy will also help you.

Spondylitis - Can Non Surgical Methods Help You?

FIPP ( UK ), DA, MBBS
Pain Management Specialist, Navi Mumbai
Spondylitis - Can Non Surgical Methods Help You?

Spondylitis includes swelling of the vertebra. It happens because of wear and tear of the ligament and bones found in your cervical spine, which is in your neck. While it is to a great extent because of age, it can be brought on by other reasons too. Side effects incorporate pain and stiffness starting from the neck to the lower back. The spine's bones (vertebrae) get fused, bringing about an unbending spine. These changes might be mellow or extreme, and may prompt a stooped-over posture. Some of the non-surgical methods to treat spondylitis are as follows-

  1. Exercise based recovery/physiotherapy: Your specialist may send you to a physiotherapist for treatment. Non-intrusive treatment helps you extend your neck and shoulder muscles. This makes them more grounded and at last, relieves pain. You may neck traction, which includes using weights to build the space between the cervical joints and decreasing pressure on the cervical disc and nerve roots.
  2. Medications: Your specialist may prescribe you certain medicines if over-the-counter medications do not work. These include:
    1. Muscle relaxants, for example, cyclobenzaprine, to treat muscle fits
    2. Opiates, for example, hydrocodone, for pain relief
    3. Epileptic medications, for example, gabapentin, to ease pain created by nerve damage
    4. Steroid infusions, for example, prednisone, to decrease tissue irritation and diminish pain
  3. Home treatment: In case your condition is less severe, you can attempt a couple of things at home to treat it:
    1. Take an over-the-counter pain reliever, for example, acetaminophen or a calming medication, for example, Advil or Aleve.
    2. Use a warming cushion or an ice pack on your neck to give pain alleviation to sore muscles.
    3. Exercise routinely to help you recover quickly.
    4. Wear a delicate neck prop or neckline to get transitory help. In any case, you shouldn't wear a neck brace for temporary pain relief.
  4. Acupuncture: Acupuncture is a highly effective treatment used to mitigate back and neck pain. Little needles, about the extent of a human hair, are embedded into particular points on the back. Every needle might be whirled electrically or warmed to improve the impact of the treatment. Acupuncture works by prompting the body to deliver chemicals that decrease pain.
  5. Bed Rest: Severe instances of spondylitis may require bed rest for close to 1-3 days. Long-term bed rest is avoided as it puts the patient at danger for profound vein thrombosis (DVT, blood clots in the legs).
  6. Support/brace use: Temporary bracing (1 week) may help get rid of the symptoms, however, long-term use is not encouraged. Supports worn for a long time weaken the spinal muscles and can increase pain if not continually worn. Exercise based recovery is more beneficial as it reinforces the muscles.
  7. Lifestyle: Losing weight and eating nutritious food with consistent workouts can help. Quitting smoking is essential healthy habits to help the spine function properly at any age. If you wish to discuss about any specific problem, you can consult a pain management specialist.

In a free diagnosis camp, I got to know that I have osteoporosis (T score -3.5). Post this I got my full checkup done, all other tests are good except (Vit D 11.98) & B12 88) which are towards insufficiency end. Though my lifestyle was sedentary but I have picked up running in last 5 years and have run quite a few half marathons. I have left all this as I have developed foot pain and weight again increased post quitting running. After a gap of a year, I have again started workouts and I run 2 km 3-4 times a week and do 30 min walks daily, followed by workout. I do these exercises in park so get some sun exposure as well. I am 39 year old (Wt 85 kgs), my legs pains whenever I run fast now. Pls advise what should be done so that I can continue running long.

Dear lybrate-user, namaskar, you seem to do very well with your workout. Keep it up. Avoid running fast continuously. You can do that intermittently and increase the speed gradually. Secondly. Bmd test done in a free camp is a very crude sort of screening. If you wish, get bone densitometry done at a reputed radiology centre for exact assessment. You may take dv k-60 once a week and becosule twice a day for 3 months to compensate the deficiency reported. Take care.

Legs burning below the knee from many years. He visit many Dr. But still same problem.
Lybrate-user ji first of all you should chek your bloid sugar to know if you have diabetes. If you have diabetes than you should take treatment for that. And if that is normal you should chek your vit b12 level if that is less than also can have such burning. If it is so you should take treatment to correct it. In that case you can consult me privately. Thanks.

Your calf muscle may b tight. do stretching exercise. For that u have to learn from good physiotherapist. wear comfortable footwear, I mean which have comfortable sole. For example sports shoes. Avoid prolong standing. Try to do your majority work in sitting position. This problem start with continues pressure on sole muscles. so u have to take couple of more physiotherapy sessions. Thank you.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. It is to be expected that at your age, the knees would have some degenerative changes by now due to which you are feeling this pain. Actually if you can send me your latest knee x rays, I can have a look at them and depending upon the arthritis stage, treatment can be initiated. Also clinically how much distance can you walk and what is the requirement of analgesics per day? what exercises do you do on a daily basis and what management has been done till now for the existing problem? do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.
