Yes, this is normal in diabetic patient because this is cause by no up take of glucose by tissues so to have 1/2 teaspoon methi dana seed powder empty stomach daily it will increase your glucose into body tissue and than your body uptake glucose and utilize in proper way.

Mabashasji, Treatment of hyperthyroidism (Neomercazole 10 mg), has to continue for at least 12 to 18 months. If thyroid function returns to normal with TSH levels increasing, then drugs are stopped. In some cases, patients remain free of any symptoms for a fairly long time. In a few symptoms return in some moths, then other options of treatment are considered.

The only other way to fight diabetes is through weight loss. Worldwide, bariatric surgery has proven that weight loss is vital to diabetic reversal (a state where you are euglycemic without the need of any oral medication or insulin. A systematic review and meta-analysis of the english literature reported complete resolution of type 2 diabetes (defined as discontinuation of all diabetes-related medications and blood glucose levels within the normal range) in 78.1% of cases. But, there are two problems in this, first of all bariatric surgery is still very expensive in india (starts from rs 3-4 lakhs and above) secondly, it is not known clearly that all the benefits of post - bariatric surgery on diabetes reversal is entirely due to weight loss alone. On the other hand, dietary restriction and exercise (also known as lifestyle modification) can also theoretically cause weight loss and lead to diabetic reversal and has been tried world over. But, the fact remains that around 95% of weight loss achieved through lifestyle changes is short lived and does not translate into any benefit in the long run. Then there are stem cell therapies

Your blood sugar is not under control. You shoud avoid sugar, jagary,honey, sweets,seet fruits like banana, mangoe,grapes, cheeku etc. You shoud do exercise rehularly. And take medicines regularly if advised by yourr doctor.

Hello To reduce your weight I would advice you the following: 1.Cut down the excess carbs. If you want to lose weight you should start by avoiding sugar and starch (like bread). A low carb diet reduces your hunger and makes it easier to eat less. And it might even increase your fat burning at rest. 2.Don’t skip meals. Skipping meals will not make you lose weight faster. That strategy backfires because your body thinks food is in short supply, so it slows your metabolism in order to conserve energy. Over time, the result is that when you do eat — even if you consume the same foods as always — your body will be slower to use the calories as fuel, thus creating a backlog of unwanted pounds. 3.Stay hydrated. You've heard it before, and you'll hear it again. That's because drinking 64 ounces of water daily is one of the easiest ways to speed up weight loss. Your body needs water in order to efficiently metabolize stored fat. When you shortchange your supply, you're likely to slow down that process, meaning it's more difficult for you to burn calories. An easy eight glasses a day (at least!) will keep your system running smoothly. 4.Start exercising. Strength training builds lean muscle tissue, which burns more calories — at work or at rest — 24 hours a day, seven days a week. The more lean muscle you have, the faster you'll slim down. How do you start strength training? Try some push-ups or a few squats or lunges. Use your free weights to perform simple biceps curls or triceps pulls right in your home or office. Do these exercises three to four times per week, and you'll soon see a rapid improvement in your physique. 5.Go for an evening walk. An evening activity may be particularly beneficial because many people's metabolism slows down toward the end of the day. Thirty minutes of aerobic activity before dinner increases your metabolic rate and may keep it elevated for another two or three hours, even after you've stopped moving. What that means for you: Those dinner calories have less of a chance to take up permanent residence on your hips. 6.Take adequate rest. You should also make an effort to get enough good sleep, preferably every night. Strive to wake up refreshed of your own accord, independently of the alarm clock. If you’re the kind of person who always gets brutally woken up by the alarm ringing, you might never be giving your body adequate rest. 7.Supplement vitamins and minerals. Nutrient-dense, good food is certainly the foundation of weight loss. But an adequate amount of vitamin D can be difficult to ingest via food. In the case of a lack of sun (such as during the darker months of autumn and winter), it’s wise to supplement for multiple health reasons – and perhaps even for your weight. Sudden gain or loss in weight, rapid growth, heredity factors, stress and changes in physical conditions leads to stretch marks. Stretch marks happen after delivery too. Stretch marks can make you feel self-conscious and uncomfortable about your appearance. But, don't worry, there are several natural ways to reduce the appearance of stretch marks. The stretch marks fade away with time. 1. Try to scrub the area daily and put vitamin e oil 2. You can also apply some castor oil to the stretch marks and gently massage the area in circular motions for 5 to 10 minutes 3. Use aloe vera gel on the area once a day 4. Use rich cocoa butter before going.

lybrate-user, Thanks for the query. When on treatment with thyroxine it is necessary to maintain TSH < 3mU/L to be free from all the symptoms of hypothyroidism. So please check TSH value and if it is higher than that mentioned above, then dosage may have to be increased and TSH has to be checked again after 6 weeks. It is essential to mainntain TSH at that level. Thanks.

Does dark chocolate affect the sugar level content of the body or for diabetic patients?
Dark chocolate without sugar is very bitter, but has benefits as it contains flavonoids and other anti oxidants. About 20 g a day does help insulin sensitivity. But eating large quantity adds to blood sugar level.

Hello friends. I am Dr. Surekha Jain practicing as a gynecologist and obstetrician since last 40 years. I own my own hospital at Shalimar Bagh with the name of Jain Child and Maternity Hopsital Pvt. Ltd. I am also attached with the Fortis Hospital, Shalimar Bagh as a Senior Gynecologist and Obstetrician. Today, I am planning to tell you about Thyroid Diseases during Pregnancy. The reason for choosing this topic is that it is very common nowadays to find thyroid disease during pregnancy and the smallest variation in the thyroid organ can really affect your pregnancy. The thyroid disease is present in two forms: Hyperthyroidism and Hypothyroidism. Hyperthyroidism means over active thyroid and Hypothyroidism means which is under active thyroid.

Thyroid is an organ which is located in the front of your organ like a butterfly and it releases hormones which control metabolism of your body it controls even single cell of your body. Thyroid affects the whole body. During pregnancy, why we are worried about the thyroid disease that if you already know that you have hyperthyroidism or hypothyroidism at the very first consultation of your pregnancy, you should tell your obstetrician that you are either Hyperthyroid or hypothyroid and what drugs you are already taking. So that the doctor can help you adjust those medications. In case you do not know the status of your thyroid at very first visit the doctor will you investigations to be done and thyroid status is one of them.

Why is it so important to do early? Because the thyroid functions are more important in the first trimester of pregnancy to control the development of the baby. You very well know that during the very first three months of your pregnancy, the child being made, all the organs, all the systems are developed and after that the child only grows inside so the first three months are very crucial. And thyroid controls the activities in the mother and the baby. So, you should know the status of thyroid. Now, let us know what are the most important irregularities you can have in thyroid? Hyperthyroidism, even though it is rare, in early pregnancy signs and symptoms they quite mimic hyperthyroidism like excessive vomiting, nausea, irritability, giddiness and irregular heartbeat, even nervousness. So, these are very common in pregnancy as well as in hyperthyroidism.

So, if they are in a moderate amount, you can think of having these symptoms because of pregnancy. But, in case there is excessive, your doctor will definitely test you for hyperthyroid by doing certain blood tests and manage you accordingly on anti-thyroid drugs. The second most important is hypothyroidism. Hypothyroidism being more common than hyperthyroidism, therefore, it is very important to manage hypothyroidism in early pregnancy. In first, seven to nine weeks, the fetus doesn’t have any thyroid function in itself. So whatever it is getting it is from the mother. If the mother is deficient, the child will also become deficient and later on it can have multiple problems of nervous system, skeletal system, and developmental problems. Now, how do you diagnose the thyroid disease? It is very simple. Only by blood test you can diagnose thyroid. Three blood tests are there, TSH (Thyroid Stimulating Hormone), T3 and T4. These are the hormones which are released by thyroid and thyroid stimulating hormone is the controlling hormone which keeps adjusting itself according to the hormone. Only by test you can come to know whether you are youth thyroid that is normal thyroid or you are hypothyroid or hyperthyroid.

If thyroid is not controlled properly during pregnancy then there can be multiple things which can happen to the mother and the baby. Now, if it not controlled in the first trimester, then developmental problems can be there. Baby can have certain congenital birth defects but later on it can lead to prematurity, and high blood pressure. Now, how can you come to know you are suffering from symptoms only? Hyperthyroidism can lead to severe nausea, vomiting, and slight tremors in your body, trouble sleeping, weight loss, or low weight for pregnant women. How can you know about hypothyroidism? Symptoms of hypothyroidism are such as extreme tiredness, weight gain, confusion, constipation, concentration and memory problems.

Mr Rajesh, Hyperthyroidism is treated with anti thyroid drugs like Methimazole, carbimazole and propylthiouracil for about a few months, that will control excess thyroxine secretion. Some time radio active iodine is used to reduce excess thyroxine production or some times thyroid surgery may be used.

lybrate-user, Thanks for the query. A person in pre-diabetes state has to adapt a modified lifestyle, if he wants to postpone developing diabetes or preventing it. That means he should refrain from sweets, pastries, bakery items, foods made from maida, fruits like mango, banana, chikkoo, grapes. Dark chocolate is supposed to be good but without sugar in it. In your case with a BMI of 28.4 kgs/M2 (normal <23 kgs/M2), with a good deal of excess weight, you need to be more careful. Reason being one of the main cause for precipitating diabetes is excess weight second is inactivity. Thanks.

I had done my T3/T4/TSH test 4 days back My T3 is 102 ng/dl T4 is 9.2 & TSH is 6.05 I had been taking eltroxin 50 mcg for over 1 year. I had not taken the medicine 2 days before the test. What should be my new dose? I had to do the test since I was not at all feeling well when I was taking the dose. What should I do?

You need to increase thyroxine dose. Normally increament is done with 12.5 to 25 mcg dose. TSH takes around 6-8 weeks to reflect change after thyroxine dose increament. It is advisable to consult your diabetologist or endocrinologist before dose increament.

Your bmi is 31.54 kg/m2 showing grade 1 obesity. I am not aware what all the measures you have done to reduce weight. Please give details of thyroid test report, let me see that. Secondly your diet has to be based on (for caloric value) on ideal body weight that is 60 kgs, since you want to cut weight your total intake has to be very well regulated. That means daily calories should be restricted to a maximum of 1200 to 1300 kcal/day. Plus you will have to exercise for at least 1 hour. Brisk walking, aerobic exercise, jogging, skipping and swimming are to be undertaken. This has to continue for at least over a period of 3 to 4 years with same intensity. You should aim at a reduction of 5 to 8 % of present weight, initially.

Infertility: Causes and Treatment

I am Dr. K.D. Nayar. I’m a senior consultant and head of the department at Akanksha IVF center, Mata Chanan Devi Hospital. This center is there since 2002, more than 15 years. Right now we’re doing almost 700 to 800 IVF cycle every year with the success rate of 40 to 45 percent. Basically, infertility is affecting nearly 15 percent of the population that means one in every six couples is infertile and when we look at the causes of infertility 30 percent is reason because is in the wife, 30 percent is in the husband and 30 percent causes are in both the husband and wife and 10 percent are unexplained. Whenever we get a patient with infertility we have to first investigate them and find out what is the cause of infertility.

Generally speaking 5 percent of the patients land up in the IVF cycle. In a IVF cycle, before starting IVF cycle we have to check the husband’s profile, husband’s seminal traces, whether the husband got any problem in ejaculation or erection. He must be able to produce a normal semen sample. Once we have the husband’s profile then we will investigate along with that we investigate the wife and then we start the IVF treatment.
IVF treatment, basically we give injection to the wife, around 10 to 12 days to produce more number of eggs. We monitor her with ultrasound; see how the follicles are growing. Once the eggs are ready we take out this eggs through a very small procedure through vagina only, no cuts. We take out the eggs at the same time we take the husband’s semen, put them together in a dish, fertilize it. All this process is being done in our IVF labs.

If the husband’s semen count or motility is poor then we can catch hold of one sperm individually and inject into egg itself this is called ICSI. Once we have mixed these sperms and eggs, we leave it in the incubator and observe the further process of fertilization. We check next day they become two cell, day two they should become four cell and so on, they’re growing.

Generally speaking, we like to take them to day five when they become blastocyst and then we have the transfer. Embryo transfer is also a very simple procedure, done without anesthesia. It hardly take 3 to 4 minutes. So, that will be the most vital step in the procedure and patient has to be reassured that there is no pain limitation. There is no obligation to the patient, there is no side effect to the patients general health. Once we do embryo transfer there after 15 days we check whether this embryo is implanted or not. That is a brief summary of the ideal procedure IVF or ICSI which we are doing generally in our center.

Please do not panic. Please understand that diabetes will stay with you life long, so it's more pragmatic to accept it gracefully and focus on managing it very thoughtfully by adopting a very healthy lifestyle. I hope you appreciate the role of the doctor and medicines are limited and so its all the more important that you take good care of your health by following a disciplined life consistently. Over the last 10 years you could manage it, so in future also it can be managed efficiently provided you have the will power and right knowledge. It is acceptable normally that the medicines which had worked for your earlier may not be efficient now. What is required is to change the dosage or the medicines or line of treatment depending upon the present situation. Your diabetologist would do that for you. Just have patience and follow up with the doctor regularly. Please do some yoga and pranayama as it has the potential to transform your internal mechanism and bring harmony within the body. Please see that you exercise everyday (go slow if you are not habituated to it ). Relax your mind as worrying can disturb the equilibrium and indirectly affect the sugar control. Your blood sugar levels are not so bad, it can be fine-tuned. Trying to bring it to perfect control levels like non - diabetics is dangerous and irrational.
