I have diabetes since last 15 years and it is largely controlled but time and again it goes beyond the regulated limits. I am experiencing pain in my feet and maybe because of neuropathy, and there is also a sense of numbness. Lately is is seen that the third toe on my right leg has curved downward. Can you suggest treatment for the above and who should be consulted. Thanks/regards

M B B S, M. D (Internal Medicine)
General Physician, Nellore
If your glycated hemoglobin is within fair control, then you need not bother about occasional rises in the sugar levels. Neuropathic pains are treated symptomatically. They are expected to happen after so many years. Do not worry about your toe. I think it is only age-related changes in the joints.

It seems you are suffering from a depressive disorder. Solopose or escitalopram is an antidepressant and chills and shivering are two of its less common side effects. I would advise you to consult a psychiatrist as soon as possible and change the drug. Take care.

MBBS, MD Psychiatry, DNB Psychiatry

Headaches can impair daily routine adversely. Particularly if the headache appear daily and last longer than 1-2 hours. If the problem is for last 10 years, I hope you must have investigated yourself in detail and not resorted to just painkillers. If the problem of headache is so troublesome I would recommend you to visit a neurologist and a psychiatrist for detailed evaluation and management.

Epilepsy does has treatment. It may take some time. Also the thing to note that since the disease have a very long history in your mother - if complete omission of the disease is virtually not possible the delay in the fits episode may be a huge success. We can definitely improve the quality of life of your mother better to an extent. Start avle ka murabba in your diet.

Iam 30years old male and have writing on paper hands shivering for last one year what should I do.

MBBS,J.R. Neurology
General Physician,
This is probably due to neurodegeneration in the cerebellum. But at your age the possibility is not much. It can also be due to drugs or alcohol. I would advise you to consult a neurologist asap and abstain from alcohol and driving for the time being. Take care.

I am 20 year old male and have been having kind of migraine since past 2 years. What should I do to completely terminate my migraine. I haven't taken any sort of tablets so faar but whenever I miss my meal and get in contact with Sun my migraine triggers. Please help.

PDDM, MHA, MBBS
General Physician, Nashik
Migraine cannot be cured completely. Avoid triggers. If certain foods or odors seem to have triggered your migraines in the past, avoid them. Reduce your caffeine and alcohol intake and avoid tobacco. In general, establish a daily routine with regular sleep patterns and regular meals. In addition, try to control stress. Exercise regularly. Regular aerobic exercise reduces tension and can help prevent migraines. If your doctor agrees, choose any aerobic exercise you enjoy, including walking, swimming and cycling. Warm up slowly, however, because sudden, intense exercise can cause headaches. Obesity is also thought to be a factor in migraine headaches, and regular exercise can help you maintain a healthy weight or lose weight.

I am suffering from migraine. What I do?

Certified Diabetes Educator, Registered Dietitian (RD), PGDD, Bachelor of Unani Medicine and Surgery (B.U.M.S), General Physician
Dietitian/Nutritionist,
migrane can be managed with lifestyle changes and medications, try muscle relaxation exercises. Relaxation may help ease the pain of a migraine headache. Get enough sleep, but don't oversleep. Get an adequate amount of sleep each night. It's best to go to bed and wake up at regular times, as well. Rest and relax. If possible, rest in a dark, quiet room when you feel a headache coming on. Place an ice pack wrapped in a cloth on the back of your neck and apply gentle pressure to painful areas on your scalp. It is very important keep a headache diary. Continue keeping your headache diary even after you see doctor. It will help you learn more about what triggers your migraines and what treatment is most effective. A dose of riboflavin in appropriate dose as prescribed by me, also may prevent migraines or reduce the frequency of headaches. Coenzyme q10 supplements may decrease the frequency of migraines, magnesium supplements may also be used to treat migraines do reply back for medication prescriptions. Being also a general physician and registered dietitian, I prescribe both evidence based herbal as well as allopathic medicines complementing with dietary guidelines and home remedies carefully personalized for each individual patient.

If you have recurrent attack of headache1. It could be a tension headache due to anxiety/stress, depression inadequate sleep, low bp/high bp, migraine, prolonged use of cell phone/computer, chronic anaemia, refractive error, chronic sinusitis, organic brain lesion 2. Avoid stress, physical and mental exertion, have adequate sound sleep for 7-8 hours daily in the night 3. Go for regular exercise 4. Practice yoga, meditation and deep breathing exercise to calm your mind, control your emotion and relieve stress 5. Check for refractive error, sinusitis hemoglobin, bp 6.
