Sex is a problem to me, I am unable to satisfy wife.
Masturbation is harmless and need not stop. It is only anxiety that you are not satisfying wife and frankly ask her and change positions or the way you stimulate her.

i am 48 year old having sleepless problem could you suggest a good and available pills to resolve ?

MD Psychiatry
Psychiatrist, Chennai
As you might have known previously sleeping pills cannot be prescribed without meeting the person. However read the following simple steps to see if it helps. Many times when the sleep is disturbed it leads to many problems in the body and the mind. In today?s fast moving world sleep is disturbed by our surroundings and our activities. Here are some simple steps to improve your sleep. 1. Keep a fixed time to wake up every morning regardless of what time you go to sleep. 2. Use your bed/mattress only for sleeping. Better don?t use it for watching TV/browsing the net/chatting on phone. In case you haven?t noticed separate furniture are available for those activities.! 3. Avoid taking tea/coffee after 5 pm. No question of smoking or alcohol ? these are sure fire ways to have a disturbed sleep. 4. If you don?t fall asleep within 20 minutes of going to bed, walk out of the bed to a different room. Engage in some simple activity like reading a pleasant book/listening to some melody. 5. Don?t keep staring at the watch/clock while trying to fall asleep. 6. Try to unwind with a warm bath and gentle music before going to bed. Grandma would say drink a cup of milk. Listen to her.! 7. Sleep in a place which is not noisy/bright ? pretty obvious isn?t it.?! Yet we ignore it too often. 8. Consult a psychiatrist for help if above methods fail. Please don?t self-medicate. We are here to help.!! Palpitations - Everything You Need to Know

MBBS, MD - Internal Medicine, DM - Cardiology, Cardiac Device Specialist (CCDS - Physician )
Cardiologist, Delhi
Palpitations - Everything You Need to Know

What is Palpitation?
Palpitation is a feeling of awareness of your own heart beat. It is usually described as heart rate being either too fast (racing), too slow or a sensation of missing a beat.

Types of palpitations?

Normal (Benign)

  1. These palpitations occur as a response to physical or mental stress like exercise, fever, pain, fear, anxiety etc.
  2. They are harmless and settle on their own once the precipitating factors disappear. They do not require any medical treatment.

Abnormal

  1. These palpitations occur due to some underlying abnormality in either the structure of heart or the beating of heart.
  2. These can be dangerous and at times life threatening. They may or may not settle on their own and require some medical treatment.

Warning signs that suggest palpitations are abnormal?
Palpitations are abnormal, if they are associated with chest heaviness, chest pain, uneasiness, sweating, weakness, giddiness, feeling of black out, fainting, nausea, vomiting, seizures. Palpitations are more likely to be abnormal in people who have some existing heart disease and these patients should report to their doctor at the earliest.

What to do during palpitations?

  1. Stay calm.
  2. Call for help. If u are alone avoid driving, use a taxi or auto, go to your nearest hospital/doctor and try to get an ECG during the palpitations. If you are helping someone having palpitations, if they faint, call for help, start CPR if they remain unresponsive and take them to the nearest hospital.

Which doctor can treat palpitations?
Doctors who specialise in the treatment of palpitations are called Electrophysiologists. They specialise in heart rhythm and are capable of performing a variety of complex tests to identify and treat different types of palpitations.

Which investigations are used to diagnose palpitations?

  • ECG: Taken during palpitations and when the patient is normal is a very helpful tool.
  • Holter: Externally applied recorder, which continuously records heart rhythm for 24 hrs.
  • ELR: Extended loop recorder, is like holter, but it records rhythm for longer durations.
  • ILR:  Internal loop recorder is attached within the body for recording rhythm for long duration.
  • EPS: Electrophysiology study, is the most sure shot test to diagnose, identify and treat Palpitations.

What is EPS?
EPS stands for Electrophysiology study. By this test a trained Electrophysiologist studies the conduction and formation system of heart beat, to understand the source, cause and type of palpitation. It is a simple and safe procedure of 2 to 3 hours and can be done as a day care procedure (by admitting the patient for a few hours in hospital, with discharge on same day).

It requires fasting for 4 hrs, some standard blood investigations and is done with local anaesthesia and if required it can be combined with treatment like ablation in the same sitting. In the procedure Electrophysiologist take catheters into your heart to study and stimulate the palpitations and understand them. Once found they can use various techniques to stop the palpitations, the techniques are called Ablation.

What are the treatment options available for palpitations?
There are many options depending upon the nature of palpitations and condition of the patient:

  1. Drug therapy
  2. Cardioversion where either a drug or electric shock is given to stop palpitations immediately.

Ablations using many sophisticated computer softwares
Device Therapy like ICD (Internal cardiac defibrillator)
Combination of all the above therapies.

Your Electrophysiologist and you as a team can make a choice about the options that will be best for you.

You tell me two problems. First is size. Vagina is a flexible organ and take the size of your penis. Secondly analysis sex. It is liking and not liking of both of you. No one can guide you in that. But if do sex vaginally then it is more satisfying than analysis sex. Try the right way and you would like it. ☺

Hi, sexuality will be pleasurable and cherishing if enjoyed at right time and with right partner. Don't just engage in sex, try to concentrate on life and lead a loyal and healthy life. Have a goal. Be loyal to your partner. If this is done then there will be no fear of diseases. Try to calm down yet times with practice of masturbation.

Sir I want to ask about symptoms of diabetes and blood sugar. Are they too much dangerous?

MBBS, MD - Medicine

Sir I want to ask about symptoms of diabetes and blood sugar. Are they too much dangerous?
Increase thirst, increased frequency of urination are some initial symptoms of diabetes but it can vary from being completely asymptomatic to non specific symptoms like weakness, loss of weight, tingling in feet, dry mouth, blurry vision, burning during urination. You need to follow a healthy lifestyle to prevent such metabolic diseases. Type 1 usually occurs at younger age due to insulin deficiency and treated with insulin injections. Type 2 in adults and treated wid oral tablets. It can't be cured completely but you can keep it controlled by taking these precautions.
