Dear doctors, even underweight person can get type 2 diabetes? If yes, what are the factors?

Dear doctors, even underweight person can get type 2 diabetes? If yes, what are the factors?
Mr Raghu, Thanks for the query. T2 diabetes is usually said to be common in overweight or obese persons. But it can be seen in persons who are under weight or thin or having normal weight. The reasons are there could be insulin resistance, lack of insulin secretion plus other factors which can contribute are inactivity (no exercise) and intake of junk foods and colas. Excess body fat causes a severe insulin resistance leading to development of diabetes. This can be prevented by adapting lifestyle changes, like diet restriction, exercise and weight control. Thanks.

My Hb1Ac is 6% and Average Blood Glucose (ABG ) is 125 mg/dl. Is it diabetic or impaired glucose tolerance.

Mr. K r lybrate-user, it is impaired glucose tolerance. Also considered as pre-diabetes state. It requires you to adapt a modified lifestyle, meaning - maintain normal weight, (ideal body weight 65 kgs), take healthy diet, exercise regularly and refrain from alcohol and smoking. Then you will be able to prevent getting diabetes or may be able to postpone it. Thanks.

Dear , though your fbs and ppbs are normal your hba1c is high. That means that your average sugar control is poor and that could be one of the reasons for weakness. Are you taking any medications for diabetes?

Will diabetes have any impact on our sexual life especially when it is under control.

Will diabetes have any impact on our sexual life especially when it is under control.
Dear lybrate-user, ideally it should not. Since you are already on insulin it means that you are absolutely normal in all other aspects like a non diabetic individual, provided your sugar levels are maintained well. Please start or continue self glucose monitoring and a 3 monthly hba1c record and do not worry.

Fasting blood sugar - 129 and PPBS - 179, is it diabetes if so what is the diet plan to control.

BHMS
Homeopath,
Fasting blood sugar - 129 and PPBS - 179, is it diabetes if so what is the diet plan to control.
Dear Lybrate User, One of the major nutritional substance required for human body is carbohydrate. We take carbohydrate in the form of rice, wheat, sugar etc. The complex molecules of carbohydrate are broken down into simple glucose molecules during digestion & released in the blood stream to be absorbed by the muscle cells. Muscle cells utilise the glucose molecules to release energy by breaking down those glucose molecules. Insulin hormone plays a vital role in the absorption of the glucose molecules by the muscle cells. In abnormal conditions, either due to less production of insulin or due to resistance to insulin, the process of absorption of glucose by the muscles comes to a stand still. The glucose molecules remains in the bloodstream which results in the increase in the blood sugar level. The failure of utilisation of the glucose molecules by the muscle cells results in production of less energy than normal condition. This is externally manifested as the fatigue & tiredness of the patient. This excess amount of blood sugar compels the kidney to function more than normal level in order to excrete the blood sugar molecules through urine. As a result the patient suffers from frequent urination. This abnormal condition is clinically known as DIABETES MELLITUS. Once a patient is diagnosed with Diabetes he has to take regular medicine to keep his blood sugar level within normal limits. However, the patient does not need to take medicine unless his/her Post. Prandial Blood Sugar level reaches 200. If Post. Prandial Blood Sugar level reaches 200 then patient should take the following Homoeopathic medications:- 1) Homeopathic URANIUM NIT 3X TABLETS (R.S. BHARGAVA company). Take one tablet thrice daily, in empty stomach 2) 30 drops of Homeopathic Mother Tincture GYMNEMA SYLVESTRA Q, thrice daily, after meals, in a cup of water Along with intake of medicines the patient need to walk at least 1 hour daily without any interval. In order to decrease the intake of carbohydrate in diet the patient should avoid rice, sweet dishes, fruits, cold drinks, fast foods, alcoholic beverages, potato etc.

I am a diabetic patient for last 13 years I am taking regular medicine for that not insulin my fasting report is 229 one week before suggest what remedies I have to take?

PGDD, RD, Bachelor of Home Science
Dietitian/Nutritionist, Mumbai
For a diabetic it is best advised to have a diet planned as per the sugar levels, lifestyle and history. Though you can follow some guidelines - have small frequent meals. Do not fast for more than 2-3 hours. Avoid a very heavy meal at one time. Avoid fruit juices, aerated drinks or sugarcane juice, honey, jaggery or sugar. Fruits should be had as a mid meal. Avoid junk foods. Exercise and stay active. Do not attempt to try out too many remedies as you could go into hypoglycaemia. Sleep well. It's all a matter of discipline.

Mr Manickam, Looking at the details provided your BMI is 28 kg/M2, which means you are over weight, your ideal body weight should be 60 kg, so there is 18 kg extra weight. This is a major contributor for diabetes. You have not mentioned about blood sugar levels, HbA1c% and whether you do any exercise. It is important in the treatment of diabetes apart from taking drugs exercise and restricted diet giving exact calories matching the needs based on Ideal Body Weight, blood sugar level and extent of physical activity. Plus one should avoid sweets, deep fried foods, junk foods, pastries, alcohol, red meat and smoking. If you want any specific guidelines please come in a private conversation with details which I have asked, where I will be able to givve specific advice.

Mr Balakrishna, If your blood sugar is in good control (fasting close to 100 mg and PP around 160 mg/100 ml and HbA1c <7%), then consume good deal of green leafy vegetables, salads and fruits about 100 g a day. Avoid sweets, deep fried foods, junk foods and pastries. Avoid red meat and egg yolk. Always use whole grain flour for making roti and chapati, no maida. Avoid high fat cheese, butter, ghee and other saturated fats. In addition exercise regularly for 1 hour a day or more. Avoid smoking and alcohol completely.

lybrate-user, If hypothyroidism is present then weight increase is bound to occur unless TSH level is brought down around 3 to 4 mU/L. So first thing is to see that TSH is at desired level. Then efforts to reduce weight are likely to be fruitful. Most important is to have a restricted diet and exercise daily for at least 1 hour. Thanks.

There is a possibility that at least in part your irregular periods are due to thyroid dysfunction. Therefore if you take the right dose of thyroxine it should get better .Regesterone or primulut-n do not cause any issues with getting pregnant in the future

My Uric Acid is high and Calcium is low. I need suggestions or advice fort some medicine and precautions.

C.S.C, D.C.H, M.B.B.S
General Physician,
My Uric Acid is high and Calcium is low. I need suggestions or advice fort some medicine and precautions.
Uric acid is produced from the natural breakdown of your body's cells and from the foods you eat. Most of the uric acid is filtered out by the kidneys and passes out of the body in urine. ... High levels of uric acid in the blood can cause solid crystals to form within joints. This causes a painful condition called gout.Things that may cause this slow-down in the removal of uric acid include rich foods, being overweight, having diabetes, taking certain diuretics (sometimes called water pills) and drinking too much alcohol. Other less common causes are a diet high in purine-containing items or your body producing too much uric acid.To reduce the uric acid levels in blood, you may need to limit intake of alcohol, sugary foods and reduce purine rich foods like meat, poultry, seafood and pulses. Vegetables such as cauliflower, asparagus, spinach, peas and mushroom also contain a lot of purines.A healthy diet based on lots of fruits and vegetables can include high-purine vegetables, such as asparagus, spinach, peas, cauliflower or mushrooms. You can also eat beans or lentils, which are moderately high in purines but are also a good source of protein. Organ and glandular meats.

Causes And Treatment Of Infertility!

Causes And Treatment Of Infertility!

Infertility is a condition where a couple, trying to conceive in a natural way is not able to do so even after one year of trying. It is also a matter of worry for patients who are over the age of 35 years and those who are not able to conceive naturally even after 12 months of trying constantly.

Causes: There are many causes of infertility. It may be down to a condition present in one or both partners. Ovulation and fertilisation are the main elements of the conception process and any condition in the female or male can upset either function, which can make it difficult to conceive.

While abnormal sperm production and transport may be a common condition that leads to infertility in males, other factors like side effects of cancer treatments and too much exposure to toxins and chemicals can also play an important role in this regard. For females, the causes of infertility may range from ovulation disorders, uterine and fibroid tumours, uterine and cervical anomalies, damage to or blockage of the fallopian tubes, endometriosis, primary ovarian insufficiency and pelvic adhesions.

Treatment: Female patients can turn to medication that will stimulate ovulation, intrauterine insemination, and surgical processes to correct a uterine septum and remove endometrial polyps, or even a hysteroscopy surgery. Male patients can go through a treatment for any existing infections, hormonal injections and medication, surgical options like vasectomy reversal and art or assistive reproductive technology.

If these methods fail, the patient or the couple can always turn to methods like in-vitro fertilisation. For other mild reasons, lifestyle changes along with a stress free environment can help in increasing the chances of conception. If you wish to discuss about any specific problem, you can consult a Gynaecologist.
