Your daily habits or age may be the root of your knee problems. Some common home care techniques for knee pain that control inflammation and help to break the inflammatory cycle are protection, rest, ice, compression, and elevation, glucosamine supplement also may help. It requires a detailed treatment plan.

A detailed history of the site of pain and history of pain can help me come to a perfect diagnosis and treatment. Still if you feel swelling is present on knee, do ice massage 3-5 times a day for 5-7mineach. If you don't feel any relief in 2-3 days. Consult me privately for a better diagnosis and treatment. I am facing pain in the back-side of right-hand scapula. I used to take smoke. How shall I minimize these two bad activities ?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. This type of periscapular pain is very common and could be fibromyalgia. If there is no swelling or any other s/s of inflammation, I would advise you to undergo: - intensive physical therapy for strengthening of your periscapular muscles - adequate calcium and vitamin d intake which is important - a short course of gentle anti inflammatory medication like tab etoshine or tab paracetamol can also be initiated along with physical therapy - hot water fomentation at home and application of analgesic spray like dynapar qps spray for local application can be done after fomentation. But your mainstay of management will remain intensive physical therapy, so try to find out a good rehab & physical therapy center at your place. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You will have to let me know what injury did you sustain earlier, was that at back, what was the diagnosis at that time and what management was done for it? you have mentioned that you are having back pain, can I ask you on a scale of 1-10 what is the intensity of this pain? I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias (electrical shock like abnormal sensations) or any weakness in limbs. Kindly provide a detailed reply to my question and attach all relevant investigations so that I can understand and advise accordingly. Do not hesitate to contact me if you need any further assistance.

I have problem with my back pain. What should I do?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist,
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance.

I have pain from past 6 days in my legs. what should I do more over it happens suddenly and cures itself?
Requires detailed examination & investigations. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid, vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful. Use no pillow under the head. Do hot fomentation paracetamol 250mg od & sos x 5days. Bio d3 max 1 tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that the patient is not allergic to any of the medicines that patient is going to take. If it does not give relief in 4-5 days, inform.

Hi back pain usually occurs due to prolonged sitting in a bad posture hence the pressure on the back increases and you get back pain as your muscle don't support any undue pressure so for this now you can start with a hot packs for ten minutes twice a day and also start strengthening your core muscles.

This is a case of negligence and really bad management, the nerve is compressed, either you visit a physiotherapist who would guide you with nerve release technique, what we call as neural mobilisation or visit a neurologist or vascular surgeon to remove the pressure, even an orthopedic would be helpful.

Low Back Pain !

MPTh/MPT, BPTh/BPT
Physiotherapist, Mumbai
Low Back Pain !
Allow me to quote the legendary Michael Jackson here, “YOU ARE NOT ALONE”. Low backache is one of the commonest ailments globally. Although it varies in intensity, almost all individuals at some point in their life or the other undergo low backache.

A very common question we face at our centre is, “Will I be able to resume weight training ever?” Justifiable apprehension as the last thing they (and we) want is a relapse.

The following are some precautions which can allow one to exercise safely post severe backache.

1) Doctor: Consult a medical doctor / orthopaedic surgeon before commencing your exercise programme.

2) Physiotherapist: Sit down for a detailed counselling and understanding of the problem with your Gym – Physio and set goals accordingly.

3) Start slow: Gradual loading is the key to a successful exercise programme.

4) Rehabilitative exercises first: Your trainer is advised to integrate rehabilitative exercises in your programme at the offset.

5) Begin with stabilization exercises: At Inch By Inch – The Body Temple, we have a specialized programme called the “Rebalance” programme which is a strategically modified core programme that ensures low back safety.

6) Train accessory muscles unrelated to the back: Although all the muscles of the body are affected with backache, post rehabilitation weight training for accessory muscles such as biceps, triceps etc can be commenced safely while ensuring position providing sufficient back rest during all exercises.

7) Ensure good form: Good form is crucial for all individuals with or without back pain. With previous episode of low backache, this becomes even more crucial.

8) Listen to your body: Respect the signs your body delivers to you. If you wake up in the morning with increased uneasiness, local muscle fatigue or slight pain, take the day off! But do not make it a habit!
