Dear, low back pain are often referred for physical therapy for four weeks as an initial conservative (nonsurgical) treatment option before considering other more aggressive treatments, including back surgery. The goals of physical therapy are to decrease back pain, increase function, and teach the patient a maintenance program to prevent future back problems.
Common forms of physical therapy include:
1.Passive physical therapy (modalities), which includes things done to the patient, such as heat application, ice packs and electrical stimulation. For example, a heating pad may be applied to warm up the muscles prior to doing exercising and stretching, and an ice pack may be used afterward to sooth the muscles and soft tissues.

2.Active physical therapy, which focuses on specific exercises and stretching. For most low back pain treatments, active exercise is the focus of the physical therapy program.
This article focuses on active physical therapy and exercise as a means to help recover from back problems and prevent or minimize future flare-ups of low back pain
Low back pain has lasted between two and six weeks, or if there are frequent recurrences of low back pain, physical therapy is often recommended. Some spine specialists consider physical therapy sooner, particularly if the pain is severe.

In general, the goals of physical therapy are to decrease pain, increase function, and provide education on a maintenance program to prevent further recurrences.
A physical therapy program for back pain usually has two components:
1.Passive physical therapy to help reduce the patient's pain to a more manageable level
2.Active exercises
Passive Physical Therapy - Modalities
Acutely, the physical therapist may focus on decreasing pain with passive physical therapy (modalities). These therapies are considered passive because they are done to the patient. Examples of modalities include:
•Heat/ice packs
•TENS units
•Iontophoresis
•Ultrasound.

Hi lybrate-user, it's ab arthritis with many management options. Pain killers and steroids ate given fir few days only to suppress the acute inflammation or swelling, there have been very good studies saying that if taken judiciously under supervision they cause no harm. 20 percent of AS don't respond to basic treatment line and exercises. They require slightly more expensive drugs. It depends on which way you want to l9ok at it 10 years back we had almost nothibgvto treat, not much evidence and a dismal outlook. Many patients spines used to get fused with dismal outlook. Now a days good evidence, good medicines, gives AS patients very good quality of life, which is very important as most of AS patients are young working males. Please see the glass half full, be positive and follow your rheumatologist.

Multiple Joint Pains / Early morning pain
It is called as Rheumatic arthritis. If your pain is more in the distal joints, ie. In the upper limbs if the pain is present in the fingers/wrist and in the lower limbs it the pain is present in the toes/ ankle, then we shall definitely say it is Rheumatic arthritis. Wear elbow brace and wrist brace which will make her to feel warm and that will make the joints become firm. Hot water fermentation will help.
Knee cap will also help to prevent the damaged cartilages
If your pain is more in the distal joints, ie. In the upper limbs if the pain is present in the fingers/wrist and in the lower limbs it the pain is present in the toes/ ankle, then we shall definitely say it is Rheumatic arthritis. Which joints you have pain? If your proximal joints (ie. Shoulder, hip & knee has pain) then you can pour hot (warm) water in that area to reduce the inflammation. If you have pain in the distal joints ie. Wrist, fingers, ankle, toes then you can wear either elbow brace or wrist brace which will help you to feel warm and very protective.
And also immerse the distal joints in the hot water tub which will help you to reduce the pain.
Consult the near by physiotherapy clinic and also consult a general physician to check with your ESR levels to check whether you have inflammation.
