When to Seek Surgical  Care for Back Pain of Disc origin?

Avoiding unnecessary spinal surgery is the intent of this course, but unfortunately not all spine surgery is avoidable, and not every reader will avoid surgery. Some disc injuries require surgery or at least a consultation with a spine surgeon. Before reading the rest of this book you are urged to carefully review the next pages to determine if you need a surgical consultation. There are varying degrees of urgency in seeking a surgical consultation. On one end of the spectrum is a medical emergency which requires immediate surgical intervention to avoid or minimize a permanent impairment. On the other end of the continuum is the elective surgical consultation. An elective surgical consultation occurs when a patient elects to be seen for a surgical consultation due to a chronic progressive condition that, while not emergent, constitutes a sustained and un resolved spinal condition which may be addressed with surgery .Medical Emergencies Certain spinal symptoms require an immediate surgical consultation: Loss of bowel or bladder control, urinary retention, neurological sexual dysfunction, muscle weakness or flaccidity,neurological deficit, numbness of the genitalia, or numbness in the distribution of a saddle.Less urgent symptoms that may need a surgical consultation include severe and disabling back pain or shooting pain into the legs that is refractory (does not respond) to other treatment sand lasts more than four to eight weeks.

These conditions need immediate medical attention

Spinal Fractures Trauma (falling, car accident…)

  • Osteoporosis: Even minor trauma can cause fractures in people who have osteoporosis. Risk for osteoporosis increases with age or with the use of steroids or transplant immune-suppressing drugs.

*take 10 gms powder of carom seeds (ajwain, 10 gm powder of cumin (jeera), 10 gm of rock salt. Mix them well and take 1/2 teaspoon (3-5 gm) of mixture daily after meal. *take 1/2 teaspoon of black pepper (kali mirch), 1/ 2 tea spoon of jeera (cumin), 2-3 slice of ginger. Boil it in one glass of water for 2-3 minutes. Drink 5-10 teaspoons (15-20 ml) in morning and evening. * walk for atleast 15-20 minutes daily and avoid long duration continuos sitting. * avoid taking nonveg at night & in day time after taking food, do not sleep in day. * avoid taking alcohol for atleast 15 days & do not eat spicy foods surely.

This is a sign of stress and your mind will wander frequently because of this. Meet with a counselor and sort out that stress. In the meantime do some of these recommendations very faithfully: Daily exercise of at least half an hour is a must. Even if you go to a gym, ask for aerobic and/or callisthenic exercises with whatever else you are doing. A healthy body harbors a healthy mind. With regard to memory, it is very important that your brain and body is ideally rested to be able to recall whatever is required, rather comfortably. Puzzles pose problems to the brain that help it to use new pathways and neurons, which give the brain considerable exercise. It taxes the left brain to use logic to solve the myriad possibilities which other activities do not stimulate. Crosswords are excellent for vocabulary learning and use. Jigsaws and Rubik cube stimulate different permutations to finally settle on the most likely one. Picture completion and anagrams help approach problem-solving from several angles. Do Sudoku, and memory co-relation activities and skills. Have a good night’s sleep, have a good breakfast of more proteins, meditate often, remain free of stress, eat a lot of fiber (whole grains, fruits and vegetables), nuts, avocado, eat dark chocolate, consume less of fat and use olive oil instead, do Yoga meditation exercises, etc. You need to check out if you are stronger visual or auditory. The visual is a better mode than the auditory. However, if you combine the two modes, you will get the best concentration. Have a special place of learning, which should be well lit, with soft painted walls, well-ventilated, with no distractions. When you get bored, study by writing. If you repeat learning, at least, five to seven times, you will apparently remember for a longer time. Sit comfortably but do not slouch. The reading material should be of a fairly large print. Study at small intervals of about 40 minutes and then take a break or change the subject. Short-term memory is a faculty of the left brain, and long-term memory is a feature of the right brain. When people are stressed, they tend to favor the right brain and abandon the left brain, where short-term memory resides. So, it is really very simple: deal with the stress and activate left brain functions. Here are a few suggestions to activate left brain function: shut your left nostril and breathe, move your eyes from right to left and vice versa for at least half a minute at a time, and do callisthenic exercises with some form of counting, regularly. There is a new exercise called Super Brain Yoga, which is done by holding the right earlobe with your left thumb and index finger, and the left earlobe with your right hand’s thumb and index finger. In this position, you must squat down and rise up and do this for five minutes every day. There is some memory enhancing techniques and study methods that your teacher will be able to guide you with. If your home life is full of distractions and stress, it is likely to affect your memory, adversely. In that case, I suggest that the family goes for counseling too. The following foods do help too: Blueberries, walnuts, turmeric, Spinach, tomatoes, broccoli, acorn squash, green tea, oily fish, boiled egg, turkey, apples, oatmeal, leafy greens, lentils, pumpkin seeds, avocado, cinnamon, thyme, sunflower seeds, and red wine. Avoid sugar and junk food. You may also include these noted foods that are good for your brain health: Brain-Friendly Foods MIND stands for Mediterranean-DASH Intervention for Neurodegenerative Delay. But the MIND approach “specifically includes foods and nutrients that medical literature and data show to be good for the brain, such as berries,” says Martha Clare Morris, ScD, director of nutrition and nutritional epidemiology at Rush University Medical Center. You eat things from these 10 food groups: •Green leafy vegetables (like spinach and salad greens): At least six servings a week •Other vegetables: At least one a day •Nuts: Five servings a week •Berries: Two or more servings a week •Beans: At least three servings a week •Whole grains: Three or more servings a day •Fish: Once a week •Poultry (like chicken or turkey): Two times a week •Olive oil: Use it as your main cooking oil. •Wine: One glass a day You avoid: •Red meat: Less than four servings a week •Butter and margarine: Less than a tablespoon daily •Cheese: Less than one serving a week •Pastries and sweets: Less than five servings a week •Fried or fast food: Less than one serving a week.

Gums bleeding is due to infection in gums. Get scalling done by dentist. Take vit c tab for one week. Use proper brushing techniques. Brush, floss n use mouthwash twice daily. Knee surgery-TOTAL KNEE REPLACEMENT (TKR) is quite safe & gives uniformly good results, in our hands. We have well equipped center in DELHI. Do ask for detailed treatment plan. Surgery should be done when patient is still active & will be able to take benefit of surgery. Do not ignore .It could be beginning of a serious problem. Mr. lybrate-user, Thanks for the query. The tablets prescribed are for T2 Diabetes, high lipids and high blood pressure respectively. So whether yo you take alcohol or wine, these medications need to be taken regularly. If one decides not to have alcohol or wine even with regular parties it is possible. Looking at your weight and height it is clear that with a BMI of 30.14 kgs/M2, you are clearly obese. Obesity is one of the main precipitating factors for diabetes, dyslipidemia and hypertension. Your ideal body weight (IBW) should be <65 kgs. If you take some specific steps to reduce weight that will be beneficial to you. Since there is no mention about blood glucose levels, HbA1c%, lipid levels and BP readings there is no way to know whether things are in control or not. Thanks.

Doing wrist extensions is the best way to gradually start getting bigger wrists. Rest the back of your forearm on a table or on your leg. Your palm should be facing up, and your hand should be aligned with your arm. Place a light weight in that hand, then slowly lower the weight toward the floor. The movement should take you five seconds, then bring the wrist back up to starting position. Do two sets of 10 or 15 reps with each wrist four or five times a week.
