I have thyroid since 8 months and I have very irregular periods and I am trying to conceive what to do?
Lybrate-user, hypothyroidism causes irregularity of menstrual periods. But if is well controlled, then all the symptoms disappear including menstrual irregularities. To achieve good control tsh levels should be between 3 to 4 milli units when on treatment. Further, if conception is desired then tsh levels have to be brought down to 2 to 3 milli units/ml. Otherwise conception may not occur, if occurs, fetal development may be affected and pregnancy outcome may not be good. So please see that tsh levels are brought down to 2 to 3 milli units even before you conceive. If you have any doubts please come back. Best of luck.

Hi Mr. S. Lybrate-user, with medicines, diet is also the most important thing to maintain you sugar level under control. You have to take proper balanced diet for sugar control take small and frequent meals. Avoid simple carbs like maida and it's product, potatoes, sweet potatoes, rice etc. Avoid fried foods. Include more fiber to your diet. Take more protein. You need a well planned diet chart to follow and you cholesterol levels and sugar level will definitely be under control with in 10-20 days. *** for proper diet chart you can consult privately.

Why Diabetic Patients Develop Eating Disorders?

Diabulimia is a type of eating disorder in which patients of Type 1 diabetes intake less amount of insulin purposely, in order to lose weight. This disorder, however, is not a formal diagnosis. The disorder may be defined as disturbed eating behavior. 

Our body requires insulin for moving glucose from the blood to the cells. In type 1 diabetes patients, the pancreas stop producing insulin and glucose gets stored in our bloodstream. This causes extra stress on the kidney in getting rid of excess of sugar. The body gets devoid of glucose and the calories associated with it and a rapid loss in weight occurs. Patients develop binge eating, with more emphasis on sugar and carbohydrate enriched foods. The excess sugar gets out of the body by urinating.

The symptoms of this disease are:

  1. Occurrence of hyperglycemia
  2. Developing a habit of eating more, but still experiencing weight loss
  3. The shift in weight may be dramatic
  4. The energy level is low
  5. Eating patterns turn unusual
  6. Binge eating
  7. Developing an obsession for food
  8. Enhanced awareness about body image
  9. Feeling anxious about the weight
  10. A delay occurs in sexual maturity or attaining puberty
  11. Occurrence of stress
  12. Hospitalization is required frequently for diabetic patients
  13. Exercising amount increases
  14. The patient tends to hide food
  15. There is a smell of ketones from the breath and urine of the patient
  16. Frequency of urination gets enhanced
  17. Malnutrition occurs signalled by hair loss or drying of skin

Health hazards of Diabulimia

  1. Diabulimia can cause a great amount of permanent effects on the body, irrespective of age.
  2. Patients with diabulimia have a risk of getting early comorbidities.
  3. Health issues similar to issues arising from diabetes are observed, but are more adverse in nature.
  4. Dehydration happens along with excess urination.
  5. The patient experiences fatigue and the level of concentration falls down.
  6. An electrolyte disbalance is caused.
  7. There may be risk of heart attack, stroke, retinopathy, neuropathy, gastroparesis.
  8. Vascular disorders, gum infections and infertility in women may also occur.
  9. In the most extreme cases, death occurs.

Prevention
It has been proven by research that almost 30% of type 1 diabetes patients are the ones who abstain from or avoid using insulin in order to lose weight. This habit leads to the risk of many other fatal diseases which may be permanent in nature. Dibulimia is more common among teenage girls who suffer from type 1 diabetes and are generally obsessed with body image. Diabulimia is an eating disorder which leaves a patient at risk of acquiring many other severe diseases. Complex carbohydrates: Complex carbohydrates raise your blood sugar but not like its cousin, simple carbohydrates. Simple carbohydrates cause an immediate spike in your blood sugar levels where as complex carbs provide a steady stream of energy. So drop that packet of refined flour and get that whole wheat variant.

Cherries: Cherries, especially tart cherries and dark sweet cherries, are full of anthrocyanins. Insulin resistance prevents insulin from working properly to control blood sugar in normal ranges. Eating a diet high in anthrocyanins can mean improved use of insulin and lower blood sugars.

What are the precautions that a patient should keep in mind while having thyroid?

In general, thyroid means it either be hypo or hyper. But mostly we see hypo only in people, people who have thyroid dysfunctioning are need to take care of diet, they should consume proper levels of iodine in their diet. They should follow particular exercises. Consumption of coffee, tea should be minimised& regular exercises should be followed to maintain the proper wt/ht ratio. Regular tests for tsh is mandatory to follow up the progress. Intake of calcium& d vitamin supplements {preferably in food source] is mandatory.

Mr Pravin, If she has hypothyroidism, then treatment with thyroxine tablets should be started. Dose of the table will depend on body weight and TSH levels. Plus she should avoid soy beans and soy containing products, cabbage, cauliflower, broccoli, radish, mustard, peanuts and coffee. Daily 1 hours exercise is also equally important to maintain normal weight.

Lybrate-user, I think you are talking about" hot fomentation" which means applying heat to the affected part by using a rubber bag filled with hot water or using a cloth dipped in hot water. It is definitely useful in relieving stiffness and pain. But it will depend on the cause of low back problem. By and large it is useful.

Dear lybrate-user you must know that hlab27 is a diagnostic test of ankylosing spondylitis. A short course of anti-tnf alpha therapy can cure severe hla-b27 associated enthesitis.

Hypothyroid is not considered to be under control till the tsh report is within range. Our objective is to bring tsh within range which is usually less than 5.5. So currently your dose needs to be escalated for sure.

Sir, I am a diabetic female since 17 years, 5 year back i have developed microalbuminaria, 3 months back dr told me to start lisinopryl 5 mg, but my BP become very low. My BP was normal. Microalbumin level is 170 mg/ at present sometimes at night my systolic is 142,144 . Should i continue AC inhibitors please suggest

Whether your ACE inhibitor was started for hypertension or microalbuminuria? If your systolicis 142 with lisinopril then it is ok. You can continue.

Mr. Lybrate-user, paracetamol is an antipyretic and analgesic, meaning it reduces fever and pain like headache, body ache etc. Diabetes usually does not produce any symptoms, that is why many people do not come to know about its presence. When fully developed may cause excessive urination, excessive hunger, excessive thirst, unexplained weight loss and tiredness. For controlling headache painkillers can be used.
