Hi I am 30 year old female. I constantly have low BP varying form 90/60 to 18/128. What could be the probable reason. How to control this.

PDDM, MHA, MBBS
General Physician, Nashik
It could be because of dehydration. When dealing with low blood pressure, it is usually suggested to increase your salt and fluid intake. Before increasing yoursalt consumption, do check with your doctor. Also, cut down on alcoholic beverages and high-carbohydrate foods such asbread, rice, pasta, and potatoes.

Dear lybrate-user 60% EF is considered to be normal provided there is no segmental weakness. Furtehermore sudden symptoms may be a cause of sudden increase in BP or sudden abnormal heart rhythm & both of them should be investigated. 24 hr ECG Monitor (Holter) or 24 hr ambulatory BP may give you the answer if it happen frequently. As of now you should consult a cardiologist & evaluate the likly complication of high BP.

Dear, To control & maintan BP normal these are best methods to follow:.• Do physical exercise like jogging, running or brisk walk for atleast 20-30 minutes. • Maintain your cholesterol level to normal & eat least amount of butter, cream or fatty things. • Eat less amount of pickle, salt and fast food as they contain high sodium levels. • Avoid drinking alcohol and smoking. • Take less amount of caffine containing coffee, tea or cold drinks because caffine causes increase BP. • Drink 8 to 10 glass of water daily • Take 6 to 8 hrs of sleep daily. Do not take too much stress. • Do long breathing pranayams (Anulom- Vilom, Bhramari) daily for 10 - 15 minutes. It will reduce your stress level in your body. • Eat healthy diet such as green leafy vegetables, fruits. • Take 5 ml of GULAB Jal added in a glass of water daily once a day.

How can arteriosclerosis be diagnosed? Is there any link between arteriosclerosis and cholesterol levels?

How can arteriosclerosis be diagnosed?
Is there any link between arteriosclerosis and cholesterol levels?
Dear lybrate-user Arteriosclerosis is a disease of the blood vessel walls; typically when they get brazed or hardened. Cholesterol is wrongly blamed as being a culprit of depositions causing blockages. Cholesterol gets deposited only in the presence of toxic free radicals in the blood. And this condition is also called Atherosclerosis. You can diagnose blood vessel condition with a Carotid Dopplar.

Respected Doctors, I am seeking for a great support from all of you. My husband age about 31 yrs, Height 5.6 inches, Weight 86 kgs, he consumes alcohol every day around 360 ml and meet (Chicken, beef and other oily junk foods like bhaji, Gobi Manchurians etc.) as well. Tried advising him by me and family members and taken him to de-addiction center still he is very determined not to stop. And tried giving him de-addiction powder of 3 tins without knowing still no development in his habits. Some more health issues he is facing details given below: # He is having BP – 140/105 pulse & cholesterol around 400, no sugar. # His legs are getting swollen by evening (His working style is sitting on chair at one place around 8 hrs), Lots of leg, neck, back pain is too much every day massage is required to legs at least to sleep. # His tummy also very big like a pot. Face and other body parts are also looks bulky even though he wont do any exercise and sit’s at one place after reaching home immediately starts drinking and sitting in one place he wont move little as well. There will no movement at all to the body. # he will not like to go to hospitals as well if we try to ask him, then we are unable to face his scoldings and not able to take as well to hospital. My request is to advise any medicine which will stop carving to consume alcohol, since he is very determined not to stop so that I can mix in his food consumption or any other source. Due to his addiction family happiness is drained out and relations between family members are also slowly vanishing. Request you to kindly advice the best solution I will be very thankful to you.

Respected Doctors,

I am seeking for a great support from all of you.
My husband age about 31 yrs, Height 5.6 inches,...
You have all my sympathies and I am very concerned that your husband has not reached what we call, ‘Hitting Rock Bottom’ as yet. To me his Rock Bottom seems to be death i.e. only death can stop his addiction! Anyway do not despair as yet and you may attempt the following and see how it works: I know you have tried a de-addiction center but he should not have been released if he was not ready. So in this attempt, he may be taken to another de-addiction center that are stricter and will also have follow up treatment and AA meetings for him to compulsorily attend. There is a medicine called disulfiram, which only the doctor can prescribe and it is very powerful that even if he attempts to drink a small quantity, he can have serious reactions that may sometimes entail hospitalization. You have to monitor his intake of the medicine and keep a close watch on him. He also has to go for regular counseling for at least three years! Apart from his health he will also affect the family and all his children will carry genetic factors for alcoholism. If he is into business or working, he will soon not be able to continue and he will go into financial ruin. He will lose all his friends and become ostracized by his society. He will lose his dignity, self-esteem, self-confidence, and become a misfit. He will also lose his sexual capacity, liver function, heart function, kidney etc. Now it is difficult to give up because he has become addicted to alcohol and has a lot to do. Under such circumstances it is best that when you admit him into a de-addiction center that you let him stay there for at least 6 months and after that attend counseling with a professional. The consumption of antabuse has to be carefully monitored to see that he not only consumes the medication but is also avoiding alcohol. Ultimately, it is his will power and the support that he receives from the medical fraternity and his close and dear ones. He must also learn to substitute and deal with the oral need, a rigid value system, the script issue, and of course take a look at all the genetic factors to plan a strategy not to get into what is called ‘cross addictions’ i.e. another form of addiction that may appear alright but is in fact as bad as the primary addiction. The center or hospital and the counselor will advise and guide him on several measures and precautions he will need to take to stay with his resolve. Even after the rehabilitation he must attend AA meetings and continue this support for a long time. The family will also need to attend some sessions and go for Al-anon meetings for their co-dependency issues. He cannot be treated in isolation because the family has gotten used to his drinking and have made some unhealthy adaptations to somehow cope. The children will also have to attend meetings to work out their issues because of the father’s habit. In fact, they are all suffering from the Adult Children Of Alcoholics Syndrome (ACOAs), which in effect means that they are genetically predisposed to alcoholism or can have cross addiction problems and that they will have similar traits of the abusing alcoholic but in a milder form. There are special support groups for them all over the world. Should they touch or indulge in alcohol or any addictive substances or behaviors, they could also become full-fledged addicts themselves. Now you should not give up on him but neither should you harass him. Be continuously after him and in the end you will succeed. His medical condition warrants a very strict course of action. In case he is not cooperative, then there are some centers that will come and pick him up when he is totally drunk and take him by force. They will however need to get written permission from you and the family.

Yes, food and exercise will definitely improve your condition. But continue your medication and slowly taper it off. The following building blocks can help you get a good start on improving your physical fitness: - Be active every day. Move instead of watching TV, surfing the Internet, or vegging on the couch. Exercise at least 30 minutes a day—at least 60 minutes a day if you have a weight problem. - Exercise vigorously—greater than 60 percent of maximum effort—if you want to improve fitness. Engage in a variety of activities, including weight-training, cardio, stretching, and muscle endurance exercises. - Eat a healthy diet. Eat a variety of foods, emphasizing fruits, vegetables, whole-grain products, and lean meats. Avoid simple sugars, saturated and trans fats, excessive amounts of alcohol, and desserts. - Take care of obvious health problems. See your physician regularly for checkups. Get treated for known problems, such as high blood pressure and high blood fats. Stop smoking. Do back-pain-prevention exercises if you have a back problem. - Enjoy life and reduce stress. Stress is the root cause of many health problems. Indians work hard trying to make it in a competitive society. Take time to enjoy your friends and family. Hope this helps.

Complications of High Blood Pressure Persistent high blood pressure can result in multiple complications.

C.S.C, D.C.H, M.B.B.S
General Physician,
Some people with hypertension may react with alarm when they check their blood pressure at home or at the drugstore and the reading is elevated. It's not unusual for people with an isolated finding of elevated blood pressure to show up at the local emergency department with sudden concern, but in many cases, the real concern is not a single high reading, but the cumulative effect of persistent uncontrolled hypertension. Hypertension is a risk factor for many negative health outcomes, including heart failure, coronary artery disease, stroke, kidney disease, peripheral artery disease and hypertensive retinopathy, a disease of the eye. Hypertension is also associated with type 2 diabetes mellitus, which is why the united states preventive services task force has issued a recommendation for diabetic screening in people with high blood pressure. Good control of blood pressure results in a rapid reduction of risk for cardiovascular disease.

Hypertensive heart disease results from changes in both the structure of the heart and blood vessels and their function as the body adapts to abnormally high blood pressure. If you can imagine the heart as a pump operating against a fluid circuit, picture the increased work of that pump if pressure within the circuit is increased. Some of the changes caused by persistent high blood pressure can be partially reversed with treatment using ace inhibitor medications.

Hypertension results in wear and tear of vital organs and vessels as a result of mechanical stress.

Blood vessels have mechanisms to regulate blood flow throughout the body, and when high blood pressure occurs, the blood vessels constrict in initial response. Eventually, the walls of the blood vessels thicken and tissue damage occurs, resulting in hardening of the vessel walls.

Hypertensive disease of the eye is known as hypertensive retinopathy. In the retina of the eye, changes in the blood vessels eventually result in disruption of the integrity of the vessel, and blood or fatty acids may leak onto the delicate retinal tissue. This results in permanent vision change, and it's very preventable.

Complications of stroke can be due to hemorrhage of the blood vessels that supply the brain, or, more frequently, can result from poor blood flow through arteries damaged by persistent high blood pressure. Approximately 85% of strokes are ischemic, or due to poor blood flow. Another manifestation of hypertensive disease that affects the brain is hypertensive encephalopathy. This condition refers to a change in mental status, frequently accompanied by headache and nausea, that results from swelling in the brain caused by alterations in blood flow at very high blood pressure. This condition is associated with malignant hypertension, or hypertensive emergency.

Persistent high blood pressure is one of the leading causes of chronic kidney disease, frequently requiring dialysis. Blood is filtered through the kidneys, and when the small blood vessels of the kidney are damaged by chronic uncontrolled hypertension, the body becomes unable to filter waste. People with kidney damage eventually spend hours hooked to dialysis machines that can remove waste products from their bloodstream, or they may require a kidney transplant. In either case, prevention of the complication is preferable and less complex.

Unfortunately, it is easy to underestimate the cumulative effects of high blood pressure, since the damage is asymptomatic initially. It's important to recognize the inevitable consequences of ignoring persistent high blood pressure. If you have high blood pressure, it's important to realize the silent damage occurring over time if your blood pressure is not adequately treated. If you have a high blood pressure reading, you should repeat measurement on two more occasions. If your blood pressure remains above the level your doctor has suggested, it is essential to follow up. There are many excellent and affordable medications available for treatment of hypertension. Some people require a combination of drugs to maintain control. Whatever the case, the inconvenience of treatment is far exceeded by the benefits to your health over a lifetime.
