Hello doctor am 25 years old. I have been married since two years and now we are getting ready for the family planing, so for that we had made a pre-conceptional visit with our doctor and she had checked my BP and had given few tests like Hb, Thyroid profile, FBS/PPBS and Rubella Ag Test. So the other test results were normal except the Bp (132/88 for the first visit and 130/92 for the second visit) & Rubella Ag Test that gave a non reactive result within a range of 9.30 and advised me to take the vaccination (R-Vac) on my second day of the menstrual cycle and reminded me to Not to get pregnant within 1 month. For the HT my doctor suggested me to meet a physician so for that I met a consultant physician so he checked my BP and it was normal. But for clearance of vaccination I asked him about the rubella ag test so he advised me that after getting the vaccination, I should Not get pregnant for minimum 6 months, since its a live attenuated vaccine so it may take time. So Doctor my question is how long should I not plan for the pregnancy after vaccination? Is it 6 months or 1 month? Is there any specific date for the vaccination as mentioned above (the second day of menstrual cycle)? Or any time?

No there is no spesfic date but if you get just aftre periods you are sure that you are not already pregnant next it said ideally one should nor get pregnant for three months after r vsc vaccine.

My wife is 55 years old. her echo cardiogram report is normal. Her ecg report speaks of STT changes, for which the doctor here told me to get a street Test, modified done. The stress test has come positive - it speaks of "significant ST segment depression over and above basal changes in exercise and early recovery, reverting to basal at the end of recovery" and concludes saying "stress test positive for inducible ischaemia by ST criteria, this could represent a false positive in view of baseline abnormality. Suggested clinical correlation and further evaluation as clinically appropriate" my doctor says that she is otherwise asymptomatic, with normal bp, triglycerides, non diabetic, no chest pain, no breathlessness, no palpitation, and may be because of baseline issues. But he has suggested for a ct scan coronary angiogram. I would like to know whether is angio is necessary, as I would not otherwise have liked to expose her to 64 radiation hazards, if avoidable. Please advise. Please tell me the charges and the digital payment path.

HYPOTHYROIDISM:--- where there is deficiency of thyroxine hormone leading to sluggishness, tired feeling, weight increase, slow heart beats, intolerance to cold, swelling of legs, feet and face, difficulty in concentrating and at times in speech. Treatment for hypothyroidism is giving thyroxine tablets. And dose are according to level of thyroid for life time. Avoid bad habits . Avoid stress. Some Relax during work. Go for walk, Take plenty of water, high-carbohydrate foods such as potatoes, rice, pasta and bread. Nutrient-rich foods that improve your health may also benefit your thyroid gland, antioxidant-rich fruits and vegetables more plenty of water: Blueberries, tomatoes, bell peppers, and other foods rich in antioxidants can improve overall health and benefit the thyroid gland. Take high fiber low fat diet. Drink lot of water everyday. Take small and frequent meals at regular intervals of 2-3 hrs. Avoid outside food completely. Take almonds and flaxseeds daily Ask me privately for detailed diet plan.

My mother in law is suffering from high blood pressure since many years. Her Age - 48 yrs. Weight - 65 kg. Her uterus was removed 10 years back. Her gall bladder was removed many years back. She had undergone many medical tests. Everything came out normal. She is taking so many medicines. But still sometimes her blood pressure shoots up. Since last couple of days its 180/100. She is feeling uneasiness, lethargic, weakness, neck pain all most all the time. She is self employed. Lives alone. Husband passed away long time back. Daughter married. She is taking Amtas 5 mg, Telday H 80 mg, Zabesta 2.5 mg, Aztolet (10+75), Minipress XL 10 mg, Rablet IT, Thyrox 75 mcg, CCM tab every day. Her Blood Urea & Creatinine Serum Creatinine 0.9 mg/dL, Urea Serum 20 mg/dL. FREE T4 - Serum 16.01 pmol/L. TSH Serum 4.38. Metanephirines 633.3 (24 hrs urine sample). Potassium- Serum 4.4. Sodium- Serum 131. Glucose-Plasma (Fasting) 91. Vitamin D Total- Serum 16. Her CT 128 Cardiac Angio was done. Findings- Normal. Aldosterone-Serum 13.90. CT Scan of whole abdomen (specially adrenal gland) was done. Fundings- No Adenoma, No Pheochromocytoma. Patchy calcification of hepatic parenchyma seen-likely, healed granuloma. Bilateral non obstructing renal calculi (at lower pole). No adrenal SOL identified. Stone found in left kidney.

I understand your concern and would explain that low blood pressure could be related to different causes: - chronic anemia - dehydration - electrolytes imbalances from malnutrition - orthostatic hypotension, etc. I would recommend performing some tests to investigate for the possible cause: - a resting ECG (as you refer to have a fast heart rate) - blood lab tests (complete blood count, blood electrolytes, kidney and liver function tests) A head up tilt test would be necessary to exclude possible orthostatic hypotension. If all these tests result normal there is nothing to worry about. Meanwhile, I would recommend avoiding prolonged standing up position and take plenty of fluids (water, juices) to stay well hydrates. An increase in the salt and caffeine intake could help in raising blood pressure. Hope to have been helpful! Greetings!

I am a 40 years old male having Hypertension for the past 5 years I was taking Telma 40 morning and amlong 5 mg evening for the last three years a week ago my BP suddenly shot up to 190/100 I was feeling dizzy and my heart was pounding very fast this lasted for half and hour. The next day I got and ECTG and 2D Echo done which was normal but the TMT test showed positive for induced ischemia and my Total cholesterol was 237. On the Doctor's advice I got a coronary angiogram done which showed mild coronary artery disease but no blocks there was slight plaque build up in the arteries the doctor changed my medicine to cilamet xl 10/50 to be taken morning and evening and ecosprin av 20/75 to be taken in afternoon. It has been 5 days now that I am taking this medicine and I notice that after taking cilamet I veel dizzy and lightheaded for about 3 hours my BP also goes down to 95/55. Please let me know if I should reduce the strength of Cilamet or change the tablet.

I am suffering from chest pain I think oall is due to my habit of smoking how I can get rid of this habit.

You have to quit cold turkey because the body and mind will torture you endlessly while attempting a reduction in smoking. The tobacco tar in cigarette smoke lines your lung with huge patches that make that portion dysfunctional. Your oxygen carrying capacity is jeopardized, and breathing is impinged on. But don’t be discouraged because it is not too late even now to reverse all the ill-effects of smoking. I don’t know what other organs could be damaged but do go for a thorough medical check-up to assess the effects of your abuse. Regarding improving your lungs, do a lot of aerobic exercise breathing through your nostrils only. Then there are a lot of deep breathing exercises in Yoga which you can learn too. Deep breathing cleanses your system and lungs and restores it to good health. Deep breathing uses the 80% of the lungs which are not normally used in normal breathing and even when exerting yourself. The greatest effect is in the lungs because this substance is processed through the respiratory system directly through the lung surface. When quitting, you must take one day at a time; though you must give up all smoking at once, and promptly. If you look too far ahead you will get discouraged and will give up. If you sit and count the number of days you have given up, that will also focus too much on achievement. You must focus on the here and now, on that day to be precise. After all quitting is not that easy due to many factors. So even if you fail one day, you must try and try again. Very few people succeed in their first attempt. Your persistence will eventually pay off. The urge to discontinue is hampered by many factors but the worst of them is the conditioning. In fact, these conditions can exist all through the day, unlike other addictions. But your desire to want to stop is the most important factor. It is important to find out about the ill-effects of smoking to have a reason that alarms you about continuing the habit. The information is really scary but true and you must remind yourself about this every day. The nicotine in tobacco smoke stimulates the heartbeat to raise the blood pressure in no time at all. So you must avoid it like the plague: it is just very bad medicine for you. In fact, it is highly toxic too. You will need to stop smoking or consuming any tobacco related items with immediate effect. You should start vigorous exercises, gradually increasing it over a period of time. Do deep breathing exercises every time you feel like smoking (try Yoga), and replace the oral urge with some healthy food substitute to satiate the need. You may also take hard candy or chew gum. If the urge is too strong, then use nicotine gums and nicotine patches, for a little while until the smoking drive reduces, substantially. You may also join Smokers Anonymous in your city where the support is really extraordinary for like-minded people who are also in the same situation. Keep yourself occupied or engaged with interesting activities during the times when it is most tempting. Keep the company of non-smokers for some time i.e. At least for the first 21 days. Above all announce it to everyone that you have quit smoking. Here’s a piece of very good advice: even if you accidentally/willfully take a cigarette, it is not the end of the world – you can start the cessation all over again. You must persevere with the best support until you defeat this addiction. Counseling is very useful in the initial stages and when temptations are at their highest. You could also join twitter2quit program and enjoy its success to give up smoking.

You need to reduce salt in your diet. Cut down on pickles, chutneys & sauces. For weight, you need to exercise daily. Do yoga also.

Diabetes mellitus is a condition affecting the way one’s body utilizes blood sugar (glucose). Glucose is essential to your health since it is a vital energy source for the cells that make up the tissues and muscles. Diabetes is characterized by excess blood sugar levels. The complications might get aggravated and turn fatal if it is left untreated over a period of time.

Diabetes mellitus can be of two types - Type 1 and Type 2.

In Type 1 diabetes, the pancreas is unable to produce enough insulin to metabolize sugar and starch from the foods that you consume while Type 2 diabetes affects the way the body processes the blood glucose.

Diabetes substantially increases the risks of numerous cardiovascular disorders (a condition that affects the heart’s blood vessels due to fatty deposits within the arteries) such as atherosclerosis (narrowing of arteries), stroke, heart attack (wherein flow of blood to the heart muscle gets blocked all of a sudden), heart failure (inability of the heart to pump blood throughout the body) and angina (chest pain).

Why are diabetic people at a higher risk of developing cardiovascular diseases?

The following factors are associated (could be the common reason as well) with both diabetes and cardiovascular diseases-

  1. High blood pressure and hypertension.

  2. High triglyceride level (a kind of fat present in the blood) and abnormal cholesterol level.

  3. Obesity.

  4. Lack of exercises and physical activities.

  5. Uncontrolled blood sugar level.

  6. Smoking.

I am a boy of 18 years. After ejaculation I feel several problems like, faster heart beat, backache and dizziness. Recently I saw that my blood pressure is 100-80. I can not concentrate in my job. Without ejaculating for several weeks I feel lethargic. So I am getting dependent on masturbation.

Sir my bp charting for two days is like this 137-86 and 150-90 and having pain on left side of the chest and back and also headache .feeling of dizziness is also there.

Hi, Welcome. Considering your age of 20 yrs, your bp of 150/90 is definitely high. But a single value of raised bp is not important & I'll recommend you to monitor it daily & if it is still high, you will need medication. Their are specific causes of high bp at your age. So you need to check them which can also rule out cause of your dizziness. I'll recommend for a Thyroid function test, Vit D level, hemoglobin, Kidney function test. Any abnormality in these will be treated accordingly. Thanks.

I want to get rid of belly fat. But am unable to as I am already a patient of Scleroderma with Raynaud and also am having high blood pressure. What should I do to reduce my weight.

Pay close attention to your food choice. ·        Eat less carbohydrate food sources like chapatti, rice, noodles, pasta, bread etc as they help in gaining weight and storing fat. ·        While having lunch or dinner eat vegetables and dal to fill your stomach rather eating lots of chapattis or rice. ·        Have a healthy breakfast like poha, idli, upma, plain dosa, oats, dalia or khichdi with vegetables. ·        Eat a balance diet including good quality proteins like: fish, lean chicken and eggs. If you are a vegetarian then include skimmed milk, low fat paneer, butter milk, soy bean or soy milk in your diet. ·        Avoid ghee, butter or vanaspati. Prepare your diet with vegetable oil like sunflower, safflower soybean or olive oil. ·        Eat plenty of salads. ·        Eat fresh fruits including apple, orange, pineapple, papaya, sweet lime, berries, guava, watermelon, mask melon etc. ·        Avoid eating outside junk and fried foods like pizza, pasta, roll, burgers, cakes, chocolates and cookies. ·        Have an early dinner. The earlier the better, giving your system more time to digest the food. ·        Avoid cold drinks/alcohol as they add empty calories to your diet. ·        Avoid fat enriched milk products like cheese, sweets, full fat cream etc. ·        Drink 3 liters of water in a day. ·        Try to have less sugar in food and beverages. ·        Try to drink 2 cups of green tea in a day as green tea helps in weight loss by aiding digestion. ·        Avoid red meat like mutton, beef, lamb or pork. ·        Eat sprouts or roasted chana, whenever you feel hungry.
