Dear Sir, I am 42 years old got oral cancer surgery twice, first surgery done in Aug 2014 at ONCOLOGY HOSPITAL at Hyderabad, due to radiation not given for first time surgery, 1 lump re-occurred under the neck with in 4 months of surgery. Undergone MRI Scan and diagnosed lump done 2nd surgery at Secunderabad on 14th of Jan 2016, by a reputed Oncology Surgeon & Radiation Oncologist Dr. Reputed chemotherapies they removed total left side Jaws (Up & down) I got radiation treatment and also Chemotherapy, and still suffering with radiation effect, my radiation therapy completed in july 2015. But still saliva secretion is not happening unable to eat chilli mixed curries my mouth is becoming dry, as secretion of saliva is not happening at one side. How long shall this effect will be there in my life. Aside I am not able to open my total mouth and take food properly. Kindly suggest me if any possibilities of fast cure of this problem Sir.

Brush your teeth after each meal and at bedtime with a soft-bristle toothbrush and fluoride toothpaste. Soak the brush in warm water to make the bristles even softer.Floss gently once a day.Rinse your mouth four to six times a day, especially after meals, with salt and baking soda. Dear sir I am facing premature ejaculation. My penis doesn't erect easily. I was ok before some time. But now I'm facing this problem before one week. So please give me suggestion as much as possible.

Ejaculation of semen during sexual intercourse before or immediately after penetration. Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common sexual complaint. Estimates vary, but as many as 1 out of 3 men say they experience this problem at some time. As long as it happens infrequently, it's not cause for concern. The primary symptom of premature ejaculation is the inability to delay ejaculation for more than one minute after penetration. However, the problem may occur in all sexual situations, even during masturbation. Premature ejaculation can be classified as lifelong (primary) or acquired (secondary). Lifelong premature ejaculation occurs all or nearly all of the time beginning with your first sexual encounters. Acquired premature ejaculation has the same symptoms but develops after you've had previous sexual experiences without ejaculatory problems. Many men feel that they have symptoms of premature ejaculation, but the symptoms do not meet the diagnostic criteria for premature ejaculation. Instead these may have natural variable premature ejaculation, which is characterized by periods of rapid ejaculation as well as periods of normal ejaculation. Psychological causes Some doctors believe that early sexual experiences may establish a pattern that can be difficult to change later in life, such as: •Situations in which you may have hurried to reach climax in order to avoid being discovered •Guilty feelings that increase your tendency to rush through sexual encounters Other factors that can play a role in causing premature ejaculation include: •Erectile dysfunction. Men who are anxious about obtaining or maintaining an erection during sexual intercourse may form a pattern of rushing to ejaculate, which can be difficult to change. •Anxiety. Many men with premature ejaculation also have problems with anxiety — either specifically about sexual performance or related to other issues. •Relationship problems. If you have had satisfying sexual relationships with other partners in which premature ejaculation happened infrequently or not at all, it's possible that interpersonal issues between you and your current partner are contributing to the problem. Biological causes A number of biological factors may contribute to premature ejaculation, including: •Abnormal hormone levels •Abnormal levels of brain chemicals called neurotransmitters •Abnormal reflex activity of the ejaculatory system •Certain thyroid problems •Inflammation and infection of the prostate or urethra •Inherited traits •Nerve damage from surgery or trauma (rare) Common treatment options for premature ejaculation include behavioural techniques, topical anaesthetics, oral medications and counselling. Keep in mind that it may take a little time to find the treatment or combination of treatments that will work for you. Behavioural techniques In some cases, therapy for premature ejaculation may involve taking simple steps, such as masturbating an hour or two before intercourse so that you're able to delay ejaculation during sex. You may try avoiding intercourse for a period of time and focusing on other types of sexual play so that pressure is removed from your sexual encounters. You can also try diverting your mind before ejaculation is ready to come and proceeding after a brief break, thereby prolonging duration of intercourse / action The pause-squeeze technique You and your partner can use of a method called the pause-squeeze technique. This method works as follows: 1.Begin sexual activity as usual, including stimulation of the penis, until you feel almost ready to ejaculate. 2.Have your partner squeeze the end of your penis, at the point where the head (glans) joins the shaft, and maintain the squeeze for several seconds, until the urge to ejaculate passes. 3.After the squeeze is released, wait for about 30 seconds, then go back to foreplay. You may notice that squeezing the penis causes it to become less erect, but when sexual stimulation is resumed, it soon regains full erection. 4.If you again feel you're about to ejaculate, have your partner repeat the squeeze process. By repeating this as many times as necessary, you can reach the point of entering your partner without ejaculating. After a few practice sessions, the feeling of knowing how to delay ejaculation may become a habit that no longer requires the pause-squeeze technique. Topical anaesthetics Anaesthetic creams and sprays that contain a numbing agent, such as lidocaine or procaine, are sometimes used to treat premature ejaculation. These products are applied to the penis a short time before sex to reduce sensation and thus help delay ejaculation. A lidocaine spray for premature ejaculation is available Although topical anaesthetic agents are effective and well-tolerated, they have potential side effects. For example, some men report temporary loss of sensitivity and decreased sexual pleasure. In some cases, female partners also have reported these effects. In rare cases, lidocaine or procaine can cause an allergic reaction. Oral medications Many medications may delay orgasm. Although none of these drugs is specifically worthwhile as people believe, approved medications may be prescribed for either on-demand or daily use, and may be prescribed alone or in combination with other treatments to treat premature ejaculation, some are used for this purpose, including antidepressants, analgesics and phosphodiesterase-5 inhibitors You can ask me personally if you want these tablets with your full health details.

Dear Lybrate user. I can understand. Please don’t panic. You must understand premature ejaculation. Premature ejaculation is diagnosed when you have premature ejaculation within one or two minute or sex, continuously several times with the same partner for over six months. There is nothing called premature ejaculation while doing masturbation. Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common problem which happens due to psychological stress and anxiety in majority of people. After confirming that these are not due to diabetics, hypertension or any other organic problems, for this reason, selective serotonin reuptake inhibitors (SSRIs), such as sertraline (Zoloft), paroxetine (Paxil) or fluoxetine (Prozac, Sarafem), are used to help delay ejaculation. If SSRIs don't improve the timing of your ejaculation, your doctor may prescribe the tricyclic antidepressant clomipramine (Anafranil). I don't see any requirement of medicines for you. Two natural treatments for PE that have shown promise are red ginseng and pomegranate juice. Ginseng is thought to increase nitric oxide production, leading to improved blood flow. Pomegranate juice is a potent antioxidant and can help too. Japan oil can help if massaged externally. Psychological reasons usually get cleared with sex therapy. I suggest online sex therapy. 1 normal size of penis in flaccid stage varies from 2.8 inch to3.9 inch with average length of 3.5 inch in length 2. Normal size in erect state varies from 4.7 inch to6.3 inch with average length of 5.1 inch. 3. A person is said to have micro penis when erect length is less than 2.8 inch. 4. Scabies is an itchy skin condition caused by the microscopic mite Sarcoptes scabei. It is common all over the world, and can affect anyone. Scabies spreads quickly in crowded conditions where there is frequent skin-to-skin contact between people. Hospitals, child-care centers, and nursing homes are examples. Scabies can easily infect sex partners and other household members. Sharing clothes, towels, and bedding can sometimes spread scabies. This can happen much more easily when the infested person has crusted scabies. You cannot get scabies from a pet. Pets get a different mite infection called mange. Symptoms are Pimple-like irritations or a rash Intense itching, especially at night Sores caused by scratching The infected person's clothes, bedding and towels should be washed in hot water and dried in a hot dryer. Treatment is also recommended for household members and sexual partners.Several lotions are available to treat scabies. Medicines that may be prescribed include: 5% permethrin cream: This is the most common treatment for scabies. It is safe for children as young as 1 month old and women who are pregnant.  25% benzyl benzoate lotion. 10% sulfur ointment.  10% crotamiton cream. 1% lindane lotion.

Small boils are occurring daily on my breast and stomach area. Sometimes they dissolve by themselves but sometimes they create problems by swelling more. Please advise.

If boils keep occuring, then I would advise you to see a doctor, because reoccurence of boils is not a good sign. It could be something serious. Meanwhile, try this at home - apply warm compresses, and soak the boil in warm water. This will decrease the pain and help draw the pus to the surface. Once the boil comes to a head, it will burst with repeated soakings. This usually occurs within five to seven days of its appearance. You can make a warm compress by soaking a wash cloth in warm water and squeezing out the excess moisture. When the boil starts draining, wash it with an antibacterial soap until all the pus is gone. Apply petrolatum (vaseline) and a bandage. Continue to wash the infected area two to three times a day and to use warm compresses until the wound heals. Do not pop the boil with a needle. This may result in making the infection worse.

Lybrate-user, blood glucose 102 mg/100 ml, is normal level. Fasting 70 to 110 mg, pp up to 140 mg hba1c% 5.9 are all normal levels. Fasting 108 to 125 mg, pp 141 to 199 mg, hba1c% 5.9 to 6.4, these are all pre - diabetes levels. Fasting equal to or above 126 mg, pp equal to or above 200 mg and hba1c% equal and above 6.5 % are all confirming diabetes.

I am 58 years old. My fasting blood sugar is 100, and pp is 168. My hba1c is 5.5. Average blood sugar for last 3 months as par hba1c report is 111. Am I a diabetic patient? should I take any medicine?

You have impaired glucose tolerance, a pre-diabetic stage. Start diet and exercise. No need for medications now. Repeat test after 2-3 months of diet and exercise.

Absolutely, you can control all your imbalances without any medicines. If your hba1c is below 7, there is nothing to worry. Your tsh is just above normal range, but don't make a mistake to start thyroid medicine at this level. The only thing you have to manage is your dirt and lifestyle. 1. Take small meals on regular interval (say 2hrly) 2. Avoid sugar in diet. 3. Avoid fatty, oily, spicy food items. Take light easily digestible food items. 4. Keep your mind happy and relaxed. Start yoga and meditation. For specific diet you may visit any good dietitian.

You need to be examined by surgeon and proctoscopy to be done to know what exactly it is, by the time keep stool soft by taking plenty of water orally and syp cremaffin 3 to 6 tsf hs, hard stool cuts the piles and blood comes out, you may be having 2 degree pile or fissure along with.
