Mr. Lybrate-user, the present readings suggest there is sub- clinical hypothyroidism. This normally is not treated. Only when tsh levels cross 10 milli units/l treatment is given. Only certain precautions need to be observed like avoid soy beans, soy containing products, cabbage, cauliflower, broccoli, radish, peanuts, mustard and coffee. Because these interfere in the thyroxine action. Since you are also having grade 1 obsity (bmi 36.69 kgs/m2), there appears to be a need for treating the present condition even though it is apparently sub - clinical. Thanks for the question.

I am diabetic for last 9 years I am on victoza1.2 with 4 mg glimipride & 1000mg metformin twice daily. Still my pp remains around 200, what to do ?

Hi , Along with medication ,try dietary , exercise option as well. 1. Consume a low-calorie diet (often less than 1,400-1,800 calories a day). Most people cannot adhere to this kind of low-calorie diet. 2. Consume a Mediterranean diet, with lots of fresh fruits and vegetables, fish and beans as protein sources, and omega-3 and monounsaturated fats (olive oil), while avoiding saturated fats, refined carbohydrates, cholesterol-laden foods, excess omega-6 fats, and most animal products. An increasing percentage of health-conscious Americans are adopting this kind of diet 3. Avoid sugary fruit juices (almost all fruit juices contain too many sugars) and beverages spiked with fructose,80-84 sucrose,85-89 and/or high-fructose corn syrup.Consume a low-glycemic index and low-glycemic load diet. Brisk walks for 20-30 mins at lest 5 times a week

Dear lybrate user, protein is one of the most vital nutritional substances which are required for human body. Almost every high protein diet contains purine. This purine produces uric acid on oxidation. Excess uric acid is accumulated at the joints of human beings as urate salts. This accumulation of urate salts lead to pain & swelling of those affected joints & this condition is clinically known as gout. You should avoid high protein rich diet like mutton, chicken, beef, eggs, sea fish, spinach, garden pea, tomato, lentils etc. Also you need to take homoeopathic mother tincture urtica urens q, 30 drops, thrice daily, after meals, in a cup of water. This medicine will help to dissolve the accumulated excess urate salts from the joints.

Hello doctor I am suffering from thyroid I am using medicines every day so is their any chance to avoid this problem for ever.

MBBS, CCEBDM, Diploma in Diabetology

Mr. lybrate-user, If one has hypothyroidism, then the treatment is to take thyroxine tablets for lifetime. Dose of thyroxine should be such that TSH should be maintained between 3 to 4 mU/L, to be free from all the symptoms. Since it is a hormone deficiency condition, treatment has to be supplementation of the hormone for lifetime. There is no other option. Thanks.

The TSH was 7-8 (3-4 years back) I was prescribed Thyroxine 75 OD but unfortunately I discontinued it 2 years back. Now these days, I felt overweight (100 Kg / 5' 10.5' Hight) and less energy so I get the test again by self. The results were. 27/Sep/16 T3 94 (Ref: 60 - 200) T4 3.1 (Ref: 4.5 - 12.0) TSH 18.92 (Reff: 0.30 - 5.5) I met my doctor and he advised to continue with Thyroxine 75 OD. Later after one month I get these tests done again. 7/Nov/16 T3 94 (Ref: 60 - 200) T4 6.2 (Ref: 4.5 - 12.0) TSH 7.45 (Reff: 0.30 - 5.5) I was advised to get Lipid profile as well, so reports are. TOTAL CHOLESTEROL 190 mg/dl (Ref: 125 - 200) HDL CHOLESTEROL - DIRECT 30 mg/dl (Ref: 35-80) TRIGLYCERIDES 506 mg/dl (Ref: 25 - 200) LDL CHOLESTEROL - DIRECT 100 mg/dl (Ref: 85 - 130) VLDL CHOLESTEROL 101.12 mg/dl (Ref: 5 - 40) TC/ HDL CHOLESTEROL RATIO 6.3 Ratio (Ref: 3.0 - 5.0) LDL / HDL RATIO 3.3 Ratio (Ref: 1.5 - 3.5) NON-HDL CHOLESTEROL 159.7 mg/dl (Ref: < 160) Please suggest if I am suffering from something serious. No need to worry about these medical results. First of all try to take a curative treatment for your thyroid problems. Do not worry about High triglycerides. It will come down gradually with some simple medicines. Do regular exercise and follow low fatty diet habits. Include more salads in your plate. Avoid fluorine products from your day to day activities. (eg. High flourine Tooth paste, etc)

Red meat is probably the most well known foods high in uric acid causing purines. Try to avoid beef, lamb, venison and mutton and instead replace them with white meats such as chicken or duck, which don't contain as many purines but should be eaten only occasionally. Even better replacements are vegetarian alternatives. Also alcohol. Repeat test after fasting overnight.

lybrate-user, If hypothyroidism is in control (TSH between 3 to 4 mU/L), then efforts to control weight can be successful. Only way to reduce weight is exercise and take a restricted diet. Your caloric intake has to be based on ideal body weight (60 kgs X 25 calories = 1500 cals, since you want to reduce weight, then the intake has to be cut by another 500 cals per day. So daily food intake should give only 1000 calories. Plus at least 45 minutes of walking also is required. Without this there is no other way to reduce weight. Thanks.
