I'm 30 years old. I have vertigo. I feel very giddy tired and have a neck ache. What should I do?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned pain in neck region as the main problem. You are actually very young to have persistent pain. If there is only localized pain in neck region with no other associated symptoms like radiating pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture of your neck while working and sleeping and observing principles of neck ergonomics - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. If there is no improvement in 2-3 weeks with this conservative management, we will need to evaluate you further in form of dynamic x rays to look for any instability as a cause for pain and some blood investigations to rule out any deficiencies etc. You can also have vertigo due to neck issues and other causes as well, you can take tab vertin or stugeron on sos basis if you unbearable vertigo but in that case we ill have to get mri cervical spine done in your case. Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

Investigations advised - hb, tlc, dlc, esr, sugar f & pp, uric acid, vit d3 urine r/e x ray of the affected part. Anyway you may try- sleep on a hard bed with soft bedding on it. I am 82 male. I was to undergo knee surgery for arthritis but the pre-operative tests showed that my ascending aorta is enlarged (6.2 cm). Apprehensive that the enlarged portion may rupture during the surgery, the surgery has been cancelled. I will be grateful for any advise/suggestions.

You can go for physiotherapy or any kind of other alternative therapies. Use high quality graphite knee braces for sharing body weight & use walker for standing & walking.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. I have been diagnosed with spiking of tibial spine bilaterlly, tiny marginal ostophytes formation seen at right patellar polar region. I feel some hard object while lying down at night and also burning sensation in my anus region. Pl advise?

Spiking of tibial spine with marginal osteophytes are indication of degenerative changes in your knee region. Treatment of osteoarthrosis depends upon what grade of degeneration has occurred, what is your knee range of movements and what medications like analgesics do you use on a daily basis? recommendations for early oa are: - start doing daily brisk walks for at least 2-3 km in a day in mornings. - intensive knee - quadriceps and hamstring strengthening exercises should be initiated initially in presence of a trained physical therapist and then at home on your own - avoid sitting cross legged, not to use indian style washrooms, do not do anything which eccentrically loads the knee and increases cartilage breakdown. - adequate calcium and vitamin d metabolism in terms of appropriate intake is important to keep bone metabolism ok - you can take short course of anti inflammatory medication like tab etoshine on sos basis especially when there is an episode of inflammatory pain, swelling etc. Do not hesitate to contact me if you need any further assistance.

Hi if you have localized back pain with - no neurological symptoms like numbness, paresthesia, weakness in limbs etc, my advice is: - intensive physical therapy initially under supervision of a physiotherapist and then at home. - avoid forward bending, lifting heavy weights etc - take an analgesic like diclofenac or something which suits your body for 5-7 days - take muscle relaxant for same time as analgesic to relieve muscle spasm - ice packs at the site of pain, muscle spasm - local analgesic spray can be used but no massage is required let me know in a week time how you are feeling. I will actually ask you to get x ray of involved region of spine (ap & lateral) and send reports across to me so that I can advise further in a better manner. Hello apply prasarini oil or pranacharya restopain oil on your affected part and give hot water fomentation .take yograj guggul and agni tundi vati after lunch and dinner with warm water. Piles occur when the veins surrounding the anus are inflamed. Occurrence of haemorrhoids can be due to pregnancy, aging, chronic diarrhoea, constipation, straining to pass stool, sitting for long periods,, obesity and genetics. Bleeding occurring in the anal area, itching, discomfort, and swelling may indicate haemorrhoids. Precautions- Have regular food habits as well as habit of going for motion. Don’t avoid when you have an urge for going to toilet. Take 2 to 3 glasses of warm water in the morning. This will help in reducing acidity as well as hardness of stool and you don’t have to strain your pelvic muscles. Keep hydrated by drinking lots of water and having fruits and vegetable juices Avoid sitting for long time at one place. Sedentary habit causes constipation. Driving bike or cycle for long duration at a time may be avoided. Travelling in a car or bus for long journey is also not good. Train is better as you can change positions. Avoid chillies, masalas and such hot foods. Take easily digestible foods including curds and fruits. We have very good treatment in homoeopathy. Take Nux vom 200 twice daily morning and evening and give feedback after 7 days.

I am 26 year old female. I fell on the stairs 3 weeks ago and have hurt my back on right side. The pain is not much, but it has not vanished ever since. It is subsiding gradually, however, if I do any strenuous activity like washing clothes, the pain reoccurs. Please advise.

There are many reasons for pain in lower back region, so it can be due to a bony issue or a soft tissue injury at the back region. Normally even soft tissue injuries will take around 3 weeks to heal properly, but you can take gentle anti inflammatory medication like tab etoshine or tab paracetamol during this time. Also you can start with physical therapy, avoid lifting any heavy weight or lifting any heavy object especially when you get up in mornings.
