The most common form of morning stiffness and pain is due to muscle imbalances and poor conditioning of the muscles themselves. Here are some tips for how to handle your back pain: -while sleeping, try lying in a curled-up, fetal position with a pillow between your legs. If you usually sleep on your back, place a pillow or rolled towel under your knees to relieve pressure. -find a mattress with back support. A good mattress should provide support for the natural curves and alignment of the spine. Medium-firm mattresses usually provide more back pain relief than firm mattresses. -do not perform activities that involve heavy lifting or twisting of your back -avoid bending forward and sitting in a place for long time -stretching and strengthening exercises are important. -a physiotherapist can tell you when to begin stretching and strengthening exercises and learn how to safely, easily, stretch and strengthen the weak and tight areas of your lower back and hips which cause pain while sleeping. Hello, I am Dr. Mathur (orthopedic surgeon) from new delhi. Welcome to lybrate. Feeling tired and lethargic can be due to a number of reasons. So you're right in going through a routine executive check up to find the cause of the same. I would suggest the following blood tests for the same: complete hemogram with esr, rft, lft, free t3, free t4, tsh, blood sugar fasting, vitamin d levels, vitamin b12 levels along with ecg and x ray chest pa view. Revert back with investigation reports.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. 12 Tips to Manage Back Pain

Back pain is a common health problem that affects most people at some point in their life. It may be triggered by bad posture while sitting or standing, bending awkwardly, or lifting heavy objects incorrectly

Common causes:

Muscle spasms and strains

Disc problems: a sudden back movement may cause the disc to bulge or rupture completely. This causes pain in the back or along the course of the nerve that has been pressed-usually the buttocks, leg or foot.

Osteoarthritis and osteoporosis are ( Above> 50 years)

Ankylosing spondylitis: this is a type of arthritis that affects the spine. It occurs mostly in young men. It causes back pain, stiff back and difficulty in bending

Tips to manage Back pain

When you have back pain, use ice or heat. Ice is best in the first 24 to 48 hours after an injury because it reduces inflammation. After 48 hours, you can switch to heat .
Whether you use heat or ice — take it off after about 20 minutes to give your skin a rest.
It is important to keep moving as much as possible. You may require bed rest when the pain in acute, but prolonged bed rest is not helpful.
Exercise regularly to strengthen the abdominal muscles that work with back muscles to stabilize the spine
Always consult a professional trainer before starting a workout regimen
Improve your posture. Keep your spine straight while sitting and standing. Avoid slouching
If you have a desk job, get up every 20 minutes or so and stretch backwards
Design your workplace so you don’t have to hunch forward to see your computer monitor or reach way out for your mouse
Use a desk chair that supports your lower back and allows you to keep your feet planted firmly on the floor
When driving, adjust your car set and steering wheel to a correct height and distance
Be especially careful of your posture when lifting heavy objects. Never bend over from the waist Instead, bend and straighten from the knees

If your pain does not improve, consult a doctor. Your doctor may prescribe anti-inflammatory medicines for a few days till the acute stage is over

You have anterior cruciate ligament ( acl) injury of knee. There may be associated meniscus tear. Please get mri of your knee at best center having 3 tesla mri. You are very young. This seems to be a deficiency of vitamin d. Kindly get your vit d levels checked. If low, you will require vit d supplements. Also, before sleeping at night you should keep you feet soaked in warm salt water for 15-20 min.

Avoid forward bending, lifting weights and sitting posture. Practice sitting in vajrasan. Do yogasanas (bhujangasan, shalbhasan, makarasan, markatasan, chakrasan, kati uttanasan). Consult orthopaedic surgeon if symptoms persist.

Advice... Avoid sitting Cross legged. Avoid Squatting- Quadriceps Exercises- Lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core Strengthening Exercise- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Hams Stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. repeat 10 times, twice a day. Sports Taping- stretch the tape from both ends and apply on the affected area Contrast Fomentation (Hot and Cold).
