I am 40 yrs old. And in 2012 I was diagnosed as a patient of apla. From till I am taking warfarin 5mg. But in these last three years I m having senstivity problem in my teeth. One by one I have gone for rct for 7 teeth. Still the problem is continuously coming in other teeth too. Is any connection of this with my disease & what is the remedy. Please guide me.
Often paraesthesia is caused due to the needle injury. When the dentist inserts the needle for a block, the patient might experience a sharp sensation on the tongue equivalent to that of an electric shock. This is known as paraesthesia and is defined as a change in the sensation or anesthesia that is persistent and the duration generally extends. This condition cannot be prevented and is a complication in some patients who undergo a dental treatment. Though it is commonly seen in the implant therapy, paraesthesia is more of a dental malpractice.
The feeling of the electric shock is felt when the needle comes in close contact with a nearby nerve. This is enough to develop paraesthesia. Severing of the nerves with a local anesthesia and small gauge needle is uncommon. The exact cause of paresthesia has not been ruled out, but it is believed that the block happens because of using 4% solutions of local anesthesias. In case a paresthesia occurs, then it usually gets resolved within some days, weeks or months, but if it lasts for more than 6-9 months, then it is considered to be permanent.
When the paraesthesia is due to a surgical trauma, then getting help from an oral and maxillofacial surgeon is recommended. They assess that if a surgical intervention can be used to fix the problem. In case the paraesthesia is due to the anesthesia given locally by the dentist, then he/she does the following:
- Patient Management: Reassurance of the patient is must as they get jittery about the situation. The dentist often speaks about the condition with the patient personally. The patient is made to understand how a paraesthesia might have occurred and how much time it would take to resolve. This is also recorded in the patient book so as to avoid any such incidents in the future.
- Examination of the patient: The dentist should discuss the whole phenomenon and procedure of how paraesthesia can take place. It is important to let the patient know that the condition subsides, but it might take some time. The extent and the degree of paraesthesia should be assessed and the findings should be noted in the examination records.
- Follow up with patient: It is the dentist's duty to keep a tab on the follow up with the patient. The patient should go for a re-checkup within 1 month of developing a paraesthesia and then again in 1-2 month intervals. The visits could be more and can last until the paraesthesia completely resolves. Improving signs and symptoms usually promise that the paraesthesia is getting better. If paraesthesia is still persistent, then help from an oral and maxillofacial surgeon should be taken for a surgical approach.
My Orthodontic treatment is going on. First the dentist charged Rs. 15,000 for the braces (removable metal plates) and then Rs. 1,500 every month for consultation. And now he said that he will convert that braces into retainer and I will have to give around Rs. 3,000 for material cost. Other than this for Metal filling (cavity) and scaling he charged about Rs. 5,000 I want to know whether he is charging more than the actual cost.

I am facing mouth ulcer problem, when I ill use quickcool gel then it ill recover in 3-4 days and it ill happen again after every 2 weeks. Yesterday when I was cleaning my tongue then little blood was coming and my tongue become little rough. I can not eat spicy food.

Bad breath is embarrassing. It inconveniences you and everyone around you. It affects your life in more ways than you might be aware of. It is crucial that you give due care to your oral health well in time to save difficult procedures and unbearable decay in time. Here are a few ways how you can do that:
1. Brushing Twice: This is the first and the most important. The reasons of odour in the mouth are mainly gum disease, tooth decay, and cavities. You should brush your teeth twice, once right after you wake up and just before you go to bed.
2. Tongue Cleaning and Flossing: It’s not just brushing, you need to clean your tongue too. This is the area where harmful bacteria are present. To avoid odour and mouth diseases, ensure that you have a tongue cleaner and that you clean your tongue every time you brush your teeth. Also remove the in-betweens of the teeth with a flossing thread. It ensures complete deposit free oral hygiene.
3. No to Mints: Avoid using mints and gums to disguise the odour in your breath. They are temporary disguises and do more harm than good. They have sugar which stays back in your mouth and spoils your oral health, drastically. If you have to choose ones which are free of sugar.
4. Drink Ample Water: If your mouth is dry, it is a bad sign. Saliva has the important enzymes which help kill bacteria. If your mouth is dry, bacteria are increasing. So, drink water. Ensure you drink at least eight glasses of water in a day.
5. Rinse After Every Meal: Always wash the insides of your mouth well, after eating anything, esp. chocolates and other sugary foods. A good swirl with water inside your mouth will get rid of the food lodged in corners.
6. Pick The Right Fruits: Certain fruits and vegetables are very good for oral health. Juicy and crunchy Vitamin C rich fruits like bell peppers, broccoli, berries and apple have a cleansing effect on your teeth.
