My wife is 42 years old. She has a mechanical MVR surgery done in 2006. 13 days ago she had a Left MCA Brain stroke. ICA, MCA block. Now she is completely fine. She is taking warf still her previous surgery. What PT INR range should we kept? We confused. Thanks.

Hello, for mechanical mitral valve replacement the recommended inr is 3. Adding low dose aspirin 75 mg or so to warfarin is another way forward, however there is a slight chance of increased bleeding, though all studies have found benefit more than the risk, particularly for your wife as she is young. 1. For your age pain chest could be due to anxiety, stress, depression, physical and mental strain, hyper acidity, muscular problem 2. Avoid stress and strain 3. Do regular exercise, reduce your weight if over weight. 4. Control your diet. Avoid oily/spicy/fried and junk food, high calorie and cholesterol rich diet. Take small frequent meals (5-6 meals/day instead of three heavy meals. If you have hyper acidity take one tablet of pan 40 in the morning before breakfast daily for few days 5. Do yoga, meditation, deep breathing exercise like pranayama etc. It will calm your mind, control your emotion and relieve stress. 6. Have sound sleep for 7-8 hours daily. 7. Avoid excess tea/coffee, if you are having 8. Drink plenty of water and take rest 9. Take crocin advance one tablet sos after food up to a maximum of three tablets per day at the time of chest pain 10. If you have recurrent attack of chest pain check your bp, take ecg, x ray chest and consult physician for advice and treatment

Dear, The possible cause of low blood pressure are- * Improper diet * Stress * Low level of Haemoglobin (Anemia) * Hypothyroidism For Recovering from low BP follow these best methods:• Anaemia is a cause of low BP so check your hemoglobin level (Hb) if this is low so eat green leafy vegetables, beetroot and fruits like pomegranate. • Eat an apple daily it maintain your BP normal.• Take your meal daily do not skip meal.• Drink 8 - 9 glass of water daily. You should get your father a book written by Dr. Dean Ornish entitled, 'Reversing the Heart Disease'. It will give you all the guidance and more about dealing with heart disease. As for smoking I am sure he would have been told strictly to avoid consumption of tobacco smoke by the heart surgeons. Here are the things you can do for your father to quit smoking: Ideally you must spend a lot of time convincing him to stop the smoking. Of course he will agree sometimes and sometimes he will change his mind. Giving up is difficult but if you highlight the dangers of smoking regularly he may one day decide to quit. Never give up on him. Because of his age and quantity of cigarettes smoked and the length of abuse, he will find is even harder. When quitting, he must take one day at a time. If he looks too far ahead he will get discouraged and will give up. If he sits and counts the number of days he has given up, that will also focus too much on achievement. He must focus on the here and now, on that day to be precise. After all quitting is not that easy due to many factors. So even if he fails one day, he must try and try again. Very few people succeed in their first attempt. His persistence and yours will eventually pay off. The urge to discontinue is hampered by many factors but the worst of them is the conditioning. In fact these conditions can exist all through the day unlike other addictions. But his desire to want to stop is the most important factor. It is important to find out about the ill-effects of smoking to have a reason that alarms him about continuing in the habit. The information is really scary but true and he must remind himself about this every day. The nicotine in tobacco smoke stimulates the heart beat to raise the blood pressure in no time at all. Having had a heart surgery can you imagine the extent of damage he still doing to his heart?! So he must avoid it like the plague: it is just very bad medicine for him. In fact it is highly toxic too. He will need to stop smoking or consuming any tobacco related items with immediate effect. He should start exercises, gradually increasing it over a period of time. Do deep breathing exercises every time he feels like smoking (try Yoga), and replace the oral urge with some healthy food substitute to satiate the need. He may also take hard candy or chewing gum. If the urge is too strong, then use nicotine chewing gum or nicotine patches, for a little while until the smoking drive reduces, substantially. He may also join Smokers Anonymous in your city where the support is really extraordinary for like-minded people who are also in the same situation. Keep him occupied or engaged with interesting activities during the times when it is most tempting. Keep the company of non-smokers for some time i.e. at least for the first 21 days. Above all announce it to everyone that he has quit smoking. Here’s a piece of very good advice: even if he accidentally/willfully takes a cigarette, it is not the end of the world – he can start the cessation all over again. He must persevere with the best support until he defeats this addiction. Counseling is very useful in the initial stages and when temptations are at their highest. Everyone who can help him should be involved until he succeeds. If all this does not work, you can admit him into a rehabilitation center and let him stay there until he has completely quit the habit.

Hello, please share your thyroid reports with me to get the proper medication. Ask your mother to follow the below treatment for high BP:- Take Rauvolfia 1x, 2 tabs daily morning empty stomach. Drink plenty of liquids and cut down your fat, salt and sugar intake. Go for a morning walk daily at least for 30 minutes. Take a sound sleep and try to avoid anger. Revert after 15 days with feedback.

You should always bath in normal room temperature water - no cold water It can make your skin dry and itching due to the effect of cold. Take Antim crud 30 - once daily for few days and revert back.

If you are new to exercises, it will cause pain. Don't worry. Take exercises easily till your body adapts. Pain will disappear after some time.

- Skip the aerated drinks as well as the caffeine. Opt for herbal tea instead.  - Have a glass of lukewarm water everyday.  - Include banana, watermelon and cucumber in your daily diet. Watermelon juice is great for curing acidity.  - Nariyal paani is known to soothe the system if you suffer from acidity.  - Drink a glass of milk - everyday.  - Have your last meal at least two to three hours before you hit the sack.  - Keeping long intervals between meals is another cause for acidity. Have small but regular meals.  - Try to avoid pickles, spicy chutneys, vinegar, etc.  - Boil some mint leaves in water and have a glass of this after meals.

high cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Avoid fast foods,oily foods,junk foods,alcohol,smoking,fatty foods, non-veg.,avoid stress, take green veg., whole grain. Take anti cholesterol medicines .

Screening for Prehypertension

Guidelines for vary from time to time, but generally accepted classification of hypertension ( JNC7)is

Average of two or more properly measured readings at each of two or more visits after an initial screen:

Normal                  < 120 mm Hg SBP             AND   DBP < 90 mm Hg

Prehypertension    120 -139 mm Hg SBP     OR      DBP 80 to 89 mm Hg

Stage 1 HTN            140 -159 mm Hg SBP     OR      DBP 90 to 99 mm Hg

Stage 2 HTN            >/= 160 mm of Hg SBP   OR     DBP >/= 100 mm Hg.

Prevalence of prehypertension among adults in United States is approximately 37 percent.

Study done by Yadhav S et al showed prevalence of prehypetension was 32.3 percent in India with highest  36% among 30-39 yrs age group, indicates that awareness is necessary for regular blood pressure check up.

People diagnosed with hypertension represents “tip  of an iceberg”.

Many patients get to know about their raised blood pressures at the time of diagnosis, heart attack, stroke or kidney disease( end organ failures) which would have been preventable  if treated at an early stage.

Why hypertension should be diagnosed and treated ?

Hypertension currently causes  2/3rd s of all strokes and half of all cases of ischemic heart disease.

Reduction in high blood pressure leads to large reduction in stroke, heart failure, renal failure, aortic dissection, coronary events and death.

Prehypertension: It is an entity where SBP >120 -139 mm Hg or DBP 80-89 mm Hg.

Multiple epidemiological studies demonstrated increased cardiovascular risk in patients with prehypertension.People with prehypertension have increased risk of progression to sustained hypertension, the prevalence of hypertension increases from approximately 10 percent at age of 30 yrs to as high as 90 percent after age of 65 yrs.

Framinham hypertension risk prediction score, may help identification of prehypertensive patients who are at gretest risk for pregression to hypertension. Risk calculator includes variables like age, sex, family history of hypertension, body mass index and smoking.  Most important predictors of these were higher baseline blood pressure and older age.

Screening for hypertension: optimal interval for screening is not known.

2007 USPSTF( United States Preventive Services Task Force) guidelines recommend

Screening every    two years for persons with SBP <120 mm Hg and DBP < 80 mm Hg     

                                 Yearly for persons with SBP 120-139 mm Hg or a DBP 80 -89 mm Hg

How to manage Prehypertension:

TROPHY stydy( TRial Of preventing Hypertension)  results showed that there is no role for pharmacotherapy in prehypertension except in special conditions like diabetes, chronic kidney disease etc.

Treatment:As per JNC 7 reccomendations patients with prehypertension who do not have diabetes, chronic kidney disease , end organ damage , or clinical evidence of cardiovascular disease are generally treated with non pharmacological therapies.

Major non pharmacological therapies that aid to decrease blood pressure are: 

Weight reduction:  Maintain normal body weight ( BMI 18.5 to 24.9 kg/m2)  this can reduce BP by 5 to 20 mm hg per 10kg weight loss.

Adopt DASH eating plan : (DASH- Dietary Approach To Stop Hypertension) consists of fruits, vegetables, legumes, low-fat dairy products with reduced saturated and total fat.This can reduce BP by 8 to 14 mm Hg.

Dietary sodium Restriction: Reduce dietary sodium intake to no more than 100meq/day(2.4 gm sodium or 6 gm of sodium chloride). This can reduce BP by 2 to 8 mm Hg.

Physical activity: Engage in a regular aerobic physical activity such as brisk walking (at least 30 min per day, most days of the week). This can reduce BP by 4 to 9 mm Hg.

Moderation of alcohol: Limit consumption of alcohol to no more than 2 drinks per day in most men and no more than 1 drink in women and lighter weight persons. This can reduce BP by 2 to 4 mm Hg.

Mr., if he has diabetes (high blood sugar) and high bp, then proper treatment with appropriate drugs should be taken. In addition he should take a restricted diet, that gives exact calories based on his ideal body weight (70 kgs), extent of daily exercise and blood sugar levels. He should exercise for at least 1 hour, brisk walking is a good option. That helps better sugar control and also in reducing bp. He should consume less salt in the diet and avoid papad, pickles and extra salt while taking food. To control sugar level he should avoid sweets completely, avoid deep fried foods, pastries, bakery items, farsan, high fat milk products and sweetened aerated drinks (colas). Should consume more of green leafy vegetables and salads and permitted fruits about 100 g a day. If you are looking for specific drugs to be taken then please come in a private conversation with details like fasting, pp sugar levels, hba1c%, lipid profile, bp readings, amount of walking done daily and dietary pattern. Looking at those details I will be able to give specific guidance, thanks for the question.
