Dear Lybrate user, You have mentioned that she has thyroid at the beginning of your statement and that she has normal thyroid at the end. If she has thyroid, then she should be put on appropriate treatment and then investigated whether she can get pregnant. Most of the ladies with thyroid problem conceive.

I am 60 years old type 2 diabetic for 20 yerars. Most of the time range will be h1absc will be 8. Now my doctor says you can go in for insulin which is easier and safer than tablet. I can control with tablet also due advice me which one is conveiient

Take new generation medicines before go for insulin ask your consultant to change salt or combination drugs which are suitable, risk of hypoglycemic shock is always there with insulin.

Sir I am using 50 mg thyroxine tablet for one year. But recently my tsh level is 8.9 so how much taken thyroid mg .& my b12 result is 185 pg/ml this is normal ya low please advice .thanks.

My comprehensive approach for a case such as yours would be to measure all metabolic parameters and optimize them after corelating the findings to your clinical picture. My preference is to switch to compounded Thyroids that contains both T4 and T3 (Thyroxine contains only T4). I would definitely optimize your vit B12 by giving you SUBLINGUAL vit B12. Many of these meds are not available in India in local pharmacies, hence we supply these through our compounding pharmacy.

Many people ask me "Can I eat cabbage or cauliflower/broccoli since I have a thyroid condition?" let me try to give the answer in scientific way.

What are goitrogens?

Goitrogens are substances that interferes with thyroid hormone production causing enlargement of thyroid. This enlargement also known as a goiter.

Different goitrogenic substances are present in various foods. 

Cruciferous vegetables such as cabbage, broccoli, and cauliflower contain glucosinolates, which blocks iodine uptake into the thyroid.
Luckily, these vegetables are only goitrogenic in the raw state. Cooking or lightly steaming will deactivate the glucosinolates,
Thus diminishing the goitrogenic activity. While consuming cooked cruciferous vegetables is preferred, occasionally eating small amounts of these foods in the raw states should not aggravate autoimmune thyroid conditions. 

1. Iodine deficiency induced hypothyroidism should not consume as they cause further suppression of thyroid activity

However

2. Hashimoto's (most common cause of hypothyroidism) do not have iodine deficiency, and eating such otherwise very healthy vegetables is not of particular concern.

People with sub-clinical hypothyroidism (when their thyroid is still producing its own thyroid hormone and TSH is slightly elevated) may find that eating lots of goitrogens may suppress some of their own thyroid hormone production. 

Soy and canola are two goitrogens that should be avoided in those with hashimoto's.

Canola-- should be avoided in all individuals with hashimoto's- it's highly processed and made from genetically modified crops.

Soy- is a goitrogen. It blocks the activity of the TPO enzyme (important for thyroid hormone formation) soy has been linked to the development of autoimmune thyroiditis and should be avoided by those with hashimoto's.

Broccolini
Brussels sprouts
Cabbage
Collard greens
Horseradish
Kohlrabi (Ganth Gobhi)
Mustard greens (Sarso ka Saag)
Peaches
Peanuts (Moongphali)
Pears
Pine nuts (Cheedh ki Supari)
Radishes (Muli)
Spinach (Palak)
Strawberries
Sweet potato
Turnips (White beetroot- Saljam)

Therefore, although everyone is different, most people with hashimoto's should be able to eat most goitrogens, even in their raw state, (with the exception of soy and canola).

For pain take tablet paracetamol 650 mg and avoid spicy food in your diet and also avoid peanuts and potatoes in your diet and for pain we have to examine you for further guidance.

You can control your sugar level with medication, diet and lifestyle medications. Medicines can only be suggested after knowing your fasting and pp sugar levels. For diet increase your intake of green leafy vegetable like spinach, peas, beans etc. Avoid oily fatty, junk food, exercise daily including brisk walk of 20 min with thin soled shoes.

Mr. lybrate-user, Thanks for the query. Blood glucose level 270 mg (fasting or after food is not mentioned), is high. It means that present treatment is not really helping in controlling the glucose level properly. He does need some additional drugs. Plus there needs to be a close given to diet and extent of exercise being done daily. Thanks.

Body built depends on genes acquired but you can gain wait by regular moderate physical activity like suryanamaskars yoga etc along with proteinaceous organic diet like sprouts, fruits, multigrain roti with chana / rajma, dry fruits n milk.

For diabetes follow a diet low in carbohydrate, low in fat and high in fibre. Walking for 30 mins a day for at least 5 days in a week is recommended. Follow a stress free life. Try meditation or yoga to relieve your stress.

I am 62. Diabetic and taking 30/70 huminsulin 24 units before breakfast and 20 units before dinner. By mistake I was delivered N100 huminsulin and I broke the seal. Can I use N100 and if so what and when will be my dose?

Yu have to use 100 U syringe and take the correct dose. N 100 is different as it contains long acting Insulin Isophane (NPH)(100IU). There is no short acting part in it. It is taken once daily with additional regular insulin if needed before meals. Need to discuss.
