Hello, better to avoid banana and coconut water for any diabetic patients. Because it contains much glucose levels.

Mr. lybrate-user, Thanks for the query. Fruits which can be consumed are papaya, water melon, orange, sweet lime, apple, pear, guava, pomegranate, other melons. Fruits to be avoided or taken in a very small quantity are banana, mango, chikkoo, grapes and custard apple. It always advised to eat the whole fruit. Juices are not good as all the fiber contents are removed. Another importan aspect is it is necessary to achieve a strict blood glucose control before consuming the fruits. Plus not more than 2 servings a day. One can take more of green leafy vegetables. Thanks.

Erectile Dysfunction Due To Diabetes - How Counselling Can Help?

Diabetic men usually are prone to suffering from erectile dysfunction, i.e., they are unable to maintain an erection while having sex. If you are suffering from diabetes for a long time, your nerves and blood vessel might get damaged. That will cause you to suffer from erectile dysfunction. Almost 35% to 75% diabetic men suffer from erectile dysfunction. This situation becomes more serious with age.

Causes

It involves various factors like impairment in the nerves, blood vessels, and also cause problems in proper muscle functioning. You need healthy blood vessels, nerves, as well as healthy male hormones and sexual desire to get an erection. Diabetes damages the nerves and blood vessels responsible for an erection. So even if you have the required amount of hormones and the desire, you might not be able to get a proper erection.

Treatment

If you are suffering from diabetes, and have a condition of erectile dysfunction, then the most common treatment for you is Sildenafil. Other medications, which you can take are tadalafil, avanafil, and vardenafil. But keep in mind that if you have any heart problems, then you should refrain from taking any of these medicines as they will further complicate situations for you. Consult your doctor in this case to know which will be the best treatment for you to cure erectile dysfunction. Other treatment methods are as follows:

1. Intracavernous injection therapy

2. Vacuum erection devices

3. Venous constriction device

4. Intraurethral therapy

5. Penile prostheses

The dosage of these medicines depends upon weight of the patient as well as presence or absence of other co-morbid conditions like hypertension, dyslipidemia, cardiac problems, kidney/liver diseases, etc. Prescription of these medicines also depends on the other medications that the patient is taking as these ED medicines have a lot of adverse drug-interactions. Hence, one must always consult a doctor before starting these medications.

The Role of Counselling for Treating Erectile Dysfunction

Erectile dysfunction can be caused by psychological factors and once a man experiences erectile dysfunction it can have a major impact on their emotional well-being. A man may feel a loss of manhood, a loss of confidence, low self-esteem, angry and depressed and it may also affect his relationship or sexual encounters. Therefore, counselling is a crucial part of treatment in order to address these issues.
Does psychosexual counselling work?

Psychosexual counselling is most effective when a man's sexual partner is willing to be part of the treatment. Studies have shown that for men with stress-related ED, when the partner is involved in the therapy, the problem is resolved 50%-70% of the time. When the man must go through counselling alone, the results are less successful.

Psychosexual counselling is unlikely to work if a man drops out of treatment after just one or two sessions.

Sometimes, several sessions of psychosexual counselling can be helpful if a man is due to receive medical or surgical treatment for ED. A counsellor can help guide a couple towards agreeing on treatment, or help them improve their sexual communication and lovemaking skills. A man may benefit from counselling on his own regarding how to talk to his partner about penile injections or a vacuum constriction device, which are both treatments for ED.

My husband is diabetic since 10 years and on medication. He is 50 yra old now. Current sugar levels are 170 fasting and 270 pp after medication. Please suggest a healthy diet chart for him. He leads very active life but lot of stress.

lybrate-user, Thanks for the query. I have seen the details given. It is clear that your husband has a very badly controlled diabetes. This may be due to inappropriate medication and/or excess dietary intake or not enough exercise. Ideally when diabetes is well controlled FBG should be <100 mg, PP 150 to 160 mg & HbA1c%<6.8. Therefore, just giving dietary advise is not at all going to be beneficial. His whole treatment has to be looked at and needs to be altered so that blood glucose is properly controlled. So if you are keen on it, then please come in a private conversation with all the details mentioned above, then I will be able to guide specifically. Please understand strict blood glucose control is absolutely essential for having a normal complication free life. The choice is yours. Thanks.

My fasting was 240, pp was 320 when diagnosed on 21/12/2018, my height 168 cm and weight was 70 when diagnosed. Hba1c is 10℅ as on 17/1/2019, after diagnosis I was given azadu 10/25 and gladly m forte at night and morning respectably. Later I faced problem with acute 10/25 and the doctor suggested me to take half of the pill. And it was fixed. After that gld some medicine was given and it was wise for me making my glucose levels down very often. And doctor suggested me to take glytrin 20 with regular exercise. As sir you have mentioned no treatment is advocated for the 8.67, but my doc prescribed me eltroxin 25 for months. Should I continue the medicine or stop and do some physical exercise. As I have started my medicine on 20/02/2019. It is only 2 days. Please suggest both.

Mr. lybrate-user, There two different things in this question, 1) high glucose levels being treated by some drugs & 2) treatment advocated for subclinical hypothyroidism, (tsh 8.67 mu/l). With fasting and pp glucose being very high and hba1c% of 10%, it is clear that diabetes is is uncontrolled. But there is no mention whether now with treatment there is any better control achieved or not at all. In a well controlled diabetes fasting glucose should be < 100 mg, pp 150 to 160 mg & hba1c% < 6.8. For achieving those levels, appropriate medications, diet plus exercise are needed. As regards taking thyroxine which you have already been taking for quite some time, so that needs to be continued. Only thing is get tsh checked now. It should be around 3 to 4 mu/l. Thanks.

I am 31 year old woman and recently got diagnosed with diabetes and hypothyroidism. My fasting sugar was 129 and pp 170. Hba1c 7.7. Tsh 19. I am taking prescribed 75 mg thyroid pill and glycomet 1gm sr. Also following diet advised by dietitian. My father have diabetes. He was diagnosed with it when he was around 55. He is taking glynase mf. I wants to ask if is it possible for me to reverse the condition? I do not want to be on medication as I have long way to go. I was recently faced with high emotional stress. Now I am ok emotionally.

lybrate-user ji, Thanks for the query. Diabetes is a condition occuring due to either deficiency of Insulin or resistance to its action due to fat and high glucose. Plus being overweight and being inactive adds to the problem. Reducing the weight and strictly following a proper diet, doing regular exercise can definitely help in better control But drug therapy is almost always needed.

Stay away from sugar containing foods and liquids. Daily walk is good. Eat raw bitter guard fruit on empty stomach every day. Take amla juice. Practice yoga and pranayama every day. Involve in religious and spiritual activities.

Please ensure that you follow all the good practices which will help you to balance every thing and thereby maintain good sugar control. Please avoid junk food, processed and packaged food and outside beverages. Of-course you can eat a bit of sweet or cake occasionally. Be careful about the calorie intake and exercise every day. Adapt yoga and pranayam into your routine life for better control and coordination of the mind and body. Have more of natural food products (fresh fruits and vegetables) and avoid packaged food items. Go for regular health check-ups and follow up with your doctor. Read about healthy lifestyle and adapt the good habits that appeal to your nature. (Health magazines and the internet are great sources of knowledge if you do not expect shortcut methods to help you.) Understand the importance of being healthy and balance your routine accordingly. You can revert after that for further clarification or feedback.
