Hello, Your doctor has given you ovulation induction drug clomiphene citrate which will initiate follicular growth. After taking ovofar, go for follicular studies to examine the growth and maturation of follicles. Once follicular monitoring is done and the follicles are of a certain diameter then you can have timed intercourse. This will increase your chances of having a successful conception.

Hi lybrate-user, first do a general check up go know the cause of fatigue. Sometimes deficiency of vitamins or heamoglobin might cause weakness. Reduce initially to 11 hrs and then 10 and 9 gradually. Keep something excited as soon as you wake. Like walking, listening to music etc to motivate yourself. Take care.

Sir, I'm 22 years old. I have completed my graduation but still I have not built myself well. Looking at my physique it feels like I'm still in 12th std. What should I do?

Dear Lybrate User, Increasing weight does not mean that you will go fatty. In fact, extra fat serves as a precursor of many diseases. In order to increase your weight as well as stay healthy you have to increase the muscle mass of your body. First of all let me tell you that there is no shortcuts in the process of muscle gain. Increasing muscle volume & strength is a highly complicated process which is influenced by various internal & external factors. In the following article you will get a glimpse of that highly complicated process as well as some of its influencing factors. I have made utmost effort in the following article to oversimplify the total process & the factors so that it would be easy to understand for you Normally a muscle undergoes an increase in size when it gets hypertrophied due to weight training. Weight-training leads to the pumping of blood to the muscle resulting in increased protein synthesis in the muscles. This applies to every muscles of human body. So, weight training is a must to increase weight. All type of exercises can be classified under two broad headings:- (1) HIIT or High Intensity Interval Training (2) LISS or Low intensity Steady Stamina. In short just remember that HIIT is weight training & LISS is cardio exercise like swimming & running. Both the form of exercise increases the rate of metabolism. But if your goal is to increase lean muscle mass of your body then you should stick to weight training aka HIIT The basic role behind the process of increase in lean muscle in human body is played by Metabolism. Metabolism is the set of life-sustaining chemical transformations within the cells of living organisms. The three main purposes of metabolism are the conversion of food/fuel to energy to run cellular processes, the conversion of food/fuel to building blocks for proteins, lipids, nucleic acids, and some carbohydrates, and the elimination of nitrogenous wastes. In order to add mass to your muscle you need to increase your rate of metabolism. Once your rate of metabolism or BMR is increased you will be able to achieve your goals There is an internal factor which controls & triggers metabolism. It is the TESTOSTERONE hormone secreted by your body. This TESTOSTERONE hormone plays an important role in the development of the lean muscle. An increased TESTOSTERONE level will help you to achieve your goal of muscle development in a considerable less time. Just follow these three basic rules during exercising:- 1) Do not rest for more than 1 minute between sets 2) Do not exercise for more than 60 minutes continuously 3) Progressive overload is necessary to increase your muscle mass Now lets come to your diet. Whenever a muscle gets hypertrophied it demands protein to expand. Here comes the need of excess protein in your diet. It is another vital part of increasing muscle mass. You may have noticed that professional body-builders take supplements according to a fixed schedule. They take 40 grams of whey just after waking from sleep at morning. They take another 20 grams of whey just 30 minutes before workout. Again they take 40 grams of whey just after finishing workout. Now you are a normal person & unless you have any intention to compete in bodybuilding competition you do not need to follow the above timings of whey protein. In fact you can get the extra protein from your diet only. Try to take 50- 70 grams of protein daily. That would be sufficient for your daily need in initial stage. As you continue progressive overload then you should also increase your protein intake. Excess protein produces excess harmful urea. Our kidney has to function more to excrete this excess harmful urea from our body. So excess protein creates excess pressure on our kidneys. For better reasons try to take protein from plant origin. Protein obtained from animal sources like meat & egg are found to play an important role in cancer. Just after exercise the rate of metabolism is increased but to maintain that increased rate of metabolism or BMR throughout the rest of the day you should need to make some little changes in your food habit. Just think of your body as a furnace & the fire as metabolism. If you add 10 kg of wood to the fire all of a sudden then the temperature of the fire will be increased suddenly but it would not last long. So in order to maintain the constant temperature of the fire it would be wise to add 1 kg of wood after every interval of 1 hour. Same happens with our body. You need to provide food to your body at certain intervals so that the rate of metabolism remains constant. I am giving a description of your food timings:- Drink a glass of water as soon as you wake up Take some pre workout nutrition Exercise for not more than 60 minutes Take a heavy breakfast within 30 minutes of exercise Take a midday snack comprising of fruits or nuts after 2 hours of breakfast Take your lunch within 2 hours after having midday snack Take an evening snack comprising of fruits or nuts after 2 hours of lunch Take your dinner at least 2 hours before going to bed Get an uninterrupted sleep for at least 7-8 hours daily Throughout the rest of the day try to drink a glass (300 ml) of cold water after every one hour If you have a habit of masturbation then you should keep a minimum gap of 5 hours between exercise & masturbation. Just after ejaculation of sperm a sudden decrease in the amount of TESTOSTERONE hormone occurs. So it would not be wise to masturbate either just before or just after exercising. Kindly remember gaining weight is a time -taking process. So do not expect any result within fortnight.

7 Myths And Facts About Asthma That Everyone Should Know

Living with asthma could be debilitating for many as the symptoms can wax and wane if the condition is not managed effectively. This chronic condition sets off a wheezing and coughing episode each time a trigger irritates the sensitive airways, but it can still be managed effectively says experts and doctors. However, educating the patients about the condition is paramount. Since without proper knowledge, prevention may not work. So, Dr. Ashok mahashur, consultant chest physician, p d hinduja hospital and research centre, mumbai debunks some myths on asthma and offers some practical tips to handle the condition better. Here goes the list:

1. Asthma is controllable but not curable:
This isn’t news for anyone. But the many times you hear that asthma is not curable it hits you hard. Particularly for people who are chronic sufferers. But the silver lining is that it is manageable. But most people fail to control the symptoms because management calls for too much effort. However, if one tries to keep the symptoms at bay, it isn’t a difficult task. Proper lifestyle changes, medications and watching over the triggers can help one go into remission. This is not to say that the symptoms will never recur, but one can control the severity and manage it well at the time of an attack. Here are 10 things to avoid to prevent asthma attack.

If one is diagnosed with asthma during childhood and taught how to manage the condition with corrective lifestyle habits and medications, he can keep the symptoms in check even in adulthood. One can overcome the condition in adulthood, provided they have controlled the condition well since childhood. Here are eight steps to manage childhood asthma.

2. Inhalers are the most effective way of controlling asthma:

Apart from lifestyle changes, which means keeping the triggers at bay, drugs are the mainstay of treating asthma effectively. There are oral medications and inhalers to manage the symptoms of the condition. However, inhalers are more efficient in treating and controlling the symptoms. While oral medications do their job well, inhalers have a direct effect on the airways and the lungs which act quickly to reduce the symptoms during an attack. For instance, how eye drops are most effective in treating conjunctivitis over oral medications the same holds true for inhalers and asthma. Here is a guide to choose the best inhaler for asthma.

3. Of the inhalers, controllers are better than relievers:

There are two types of inhalers one group is called controller, and other is the reliever. As the names suggest, the controllers control the condition and relievers give symptom relief during an attack. The relievers give instant relief while the effect of controllers is not immediate. A reason why most asthmatics swear by relievers, however, long time use of relievers doesn’t help to control the disease and so one becomes dependent on them. But if one uses controllers it helps in the long run to manage asthma.
The reason people avoid using controllers is that it contains steroids and people are wary about its side effects. However, the impact of the side effects is very less compared to the benefits it offers. If the controllers are taken regularly, and the lung functions are monitored time-to-time the dosages can also be lowered over a period of time. Know what is best for you dry inhalers or metered dose inhalers.

4. Supplements or natural therapies cannot cure asthma:
While many people believe that natural remedies and oral supplements can cure asthma, the fact remains that the condition can only be controlled and not cured. Read to know if natural remedies for asthma really work.

5. Running isn’t advisable for asthmatics:
It is necessary for asthmatics to stay healthy and exercise as it boosts immunity. However, running isn’t advisable for them. The simple reason being it can trigger an attack. Instead, other cardio exercises like swimming, cycling can help rev up their sensitive airways.

Remember asthmatics have sensitive and hyper-reactive airways and the triggers can make the symptoms worse. Certain triggers aggravate them. The most common are dust, dust mites in old cotton clothes, cockroaches, smoke, pollen (if there is lot of greenery around), pets, foods with preservatives like msg, sour or citrus foods. Even some of the healthy things can turn to be triggers for them.

6. Humid climate might not always favour asthmatics:
Most people believe that hot and humid climate might suit people suffering from asthma. However, this is not true for everyone. The triggers are not the same for everyone. For some the symptoms might be in check in a humid area for others, it might aggravate. However, one can experiment with the different climatic conditions to see what suits one. Non-fluctuating weather is better for most of them.

7. Lifestyle changes are as important as following the right medication:
Drugs are the mainstay when it comes to asthma management; however, lifestyle also plays an important role. A disciplinary diet pattern also matters. A lot of people have two heavy meals a day instead of six small meals that could lead to acid reflux. A late night dinner for asthmatics is perilous. Having a full-plate and going to bed immediately can trigger the reflux which in turn can trigger the symptoms at mid-night and can make it worse.

Managing grief by free expressive writing
The loss of a loved is often painful. The resultant grief makes it hard to eat, sleep and leads to loss of interest in routine life affecting behavior and judgment.

Some can feel agitated or exhausted, to sob unexpectedly, or to withdraw from the world and others may find themselves struggling with feelings of sorrow, numbness, anger, guilt, despair, irritability, relief, or anxiety.

It is well known that disclosing deep emotions through writing can boost immune function as well as mood and well–being. Conversely, the stress of holding in strong feelings can ratchet up blood pressure and heart rate and increase muscle tension.

One can write on a piece of paper, in your personal book, on the open website with nick name or keep it in the mind. One doesn’t have to preserve the emotions and can through away the writings

In absence of deeply troubling situations, such as suicide or a violent death which are best explored with the help of an experienced therapist, one can choose writing as a way to express out the grief.
Start writing for 15 to 30 minutes a day for three to four days
Continue up to a week if it is helping.
Continue writing for 15 to 30 minutes once a week for a month.
Writing has stronger effects when it extends over more days.
Remember writing about grief and loss can trigger strong emotions (one may cry or feel deeply upset).
Many people find journal writing valuable and meaningful and report feeling better afterward.
Don’t worry about grammar or sentence structure.
Truly let go. Write down how you feel and why you feel that way. You’re writing for yourself, not others.

I have high cold, cough and. Little fever. What i'm suppose to do to get reed of this? please advise.

AUTLS, CCEDM, MD - Internal Medicine, MBBS
General Physician, Faridabad
There are several things you can do to prevent a cold—they are all easy and inexpensive and worth the effort. Wash your hands often-with soap and warm water. The germs stick to the soap and get washed down the drain. Always wash your hands after coughing or sneezing into your hands, blowing your nose, after touching someone who is sick, or when using public restrooms or telephones. Cover Your Mouth and Nose When You Sneeze or Cough- Clean your hands every time you cough or sneeze. Avoid Touching Your Eyes, Nose, or Mouth-Germs are often spread when a person touches a contaminated surface and then touches their eyes, nose, or mouth. Stay Home When You’re Sick-Get plenty of sleep, be physically active, don’t smoke, manage stress, drink plenty of fluids, and eat nutritious food so you can fend off viruses. Take healthy balanced boiled eggs.daily in breakfast .. Avoid people who have a cold or cold symptoms. Routinely clean and disinfect common areas of your home. Treatment:There is a gradual 1-3 day onset of symptoms. It often starts out as a sore throat, then a fever and coughing. As the cold progresses, the nasal mucus may thicken which is the last stage before the cold dries up. A cold usually lasts for a week or two. But, taking good care of yourself at home can relieve symptoms and help prevent complications if you are otherwise healthy. Unless you develop complications, you shouldn’t need medical treatment. Complications include bacterial infections of the ears, throat, sinuses or lungs which must be treated with antibiotics. In general you should: • get plenty of rest so your body can use its energy to fight the cold • staying at home will keep you from spreading germs • drink plenty of liquids like herbal tea or hot broth/vegetable/tomato soups. Avoid foods or drinks with caffeine which can keep you from getting enough sleep Runny Nose and Sneezing- if you have post nasal drip, gargle to keep from getting a sore throat. Drink extra fluids. (avoid sugary and high calorie drinks) Use saline nasal sprays or nasal irrigation • Tablet Nasorest-P once/twice daily may reduce the amount of mucous. • Humidify the rooms you are in most or your whole house if possible. • Do steam inhalation once or twice daily Cough-Coughing is how your body gets rid of debris and mucus from the airways during a viral infection. Unless your cough is severe or is keeping you from resting at night, it is better not to treat it. A severe cough can be treated with a cough syp, but don’t try to completely stop the cough. There are two types of coughs. Productive and non-productive or dry cough. Non-productive, dry cough. A dry, hacking cough may develop near the end of a cold. Dry coughs that follow viral colds are often worse at night and can last up to several weeks. Take syrup Alex-D for a dry cough. Cough drops can sooth an irritated throat, but most don’t affect the cough. Productive cough. A productive cough is one that brings up mucus from the back of the throat or the lungs. Productive coughs should not be suppressed to the point they no longer bring up mucus. Use syrup Alex. Drink lots of water if you have a productive cough. Head and Body Aches- Take tablet dolo-650 mg for fever once or twice daily

All About Nose Surgery

ENT Specialist, Hyderabad
All About Nose Surgery

A nose surgery (rhinoplasty) is performed to alter the structure and shape of your nose and improve its function and or appearance. Here are a few things you should know about nose surgeries:

Consult your doctor: Before deciding on undergoing a nose surgery, talk with your doctor regarding issues with your nose. Your doctor will check your problems and judge whether or not surgery is a feasible option. It is only with your doctor's confirmation that you should go for the surgery.

  1. The surgery is done either with local anesthesia (your nose and surrounding areas will be numbed) or general anesthesia (you will fall asleep).
  2. The surgeon will make incisions in your nostrils and base of the nose and alter the shape, structure and position of the cartilages in your nose for appealing shape and better functionality. Polyp removal is also done by rhinoplasty.

Recovery after the surgery: You will be required to wear a nose splint. You can expect swelling around the eyes, which will begin to subside after the third day. Swelling in the nose will go away after six months. Therefore, do not expect to see your results immediately, final condition of your nose will be apparent after the recovery period is over. You should avoid strenuous activity for 3 to 6 weeks. Do not smoke or drink alcohol during recovery. Smoking slows recovery time and alcohol reduces normal effects of drugs.

Will you develop scars? As with all cosmetic procedures, rhinoplasty can leave scars.

Closed rhinoplasty: Incisions are made inside the nostrils and hence there is no scarring.

Open rhinoplasty: An additional cut is made along the columella and this may result in scarring. However, the position of the scar is such that it is almost unnoticeable.

Risks: Any surgical procedure has certain risks. These risks associated with nose surgery include:

  1. Infection
  2. Adverse effect to anesthesia
  3. Unfavorable outcomes
  4. Visible scarring
  5. Numbness
  6. Difficulty breathing
  7. Chronic nosebleeds
  8. A perforated septum
  9. Hematoma
  10. Burst blood vessels
  11. Nervous damage
  12. Possibility of revision surgery
