Hi Doctors, I have migraine and gastric problem what is permanent solution for it. Can you suggest me any medicine or any regular excise which can reduce migraine problem, which I get regular every fifteen days?

I am giving some health tips for Migraine headache •1.You can turn off light for some time •2.Apply hot or cold compresses to your head or neck. •Ice packs have a numbing effect, which may dull the sensation of pain. •Hot packs and heating pads can relax tense muscles. •3.Warm showers or baths may have a similar effect. •4.You can take Tea or coffee( but not excess) 5.Sleep well Here are some tips to encourage sound sleep. Establish regular sleep hours. •Minimize distractions. •Your eating habits can influence your migraines. •Be consistent. Eat at about the same time every day. Don't skip meals. Fasting increases the risk of migraines Avoid foods that trigger migraines Meditation This problem can be solved by Meditation, i can give you address of rajyoga meditation center near your house Consult Physician/me for further management

AVM is not cancer. It is a kind of defect in the blood vessels of the brain. There is a abnormal connection between artery and veins which causes it to bulge .rarely it can rupture. Nowadaya gamma knife is being used to treat AVM and is fairly good with minimal chances of complications. She needs to be on regular follow up though.

This is regarding fainting (Syncope). My brother has this problem from last year and around a month back he became unconscious for around 5 to 7 minutes. It happens when he woke up in the morning and went for urination. After urination he stood and suddenly went to unconscious state. Can some please help on what could be the cause and what should be precautions and what should be the next steps (in terms of check up to be done)?

Treatment should address the underlying cause of syncope. Depending on the diagnosis, cardiovascular syncope may be stopped or controlled with one or more of the following therapies: Insertion of a pacemaker is the most common treatment for syncope caused by a slow heartbeat (bradycardia) While standing he needs to keep legs moving Need blood HB and TSH test.

I have severe migraine since 10 hrs. Please suggest what to do for instant relief.

For headache take tablet paracetamol 650 mg eight hourly and migrane- it is characterised by one sided headache which is pulsatile in nature and with a throbbing pain usually with an aura and we can start with tablet propranolol after personal examination.

Epilepsy is basically due to excessive electrical discharges in brain which gets dissipated and leads to abnormal involuntary jerk limb movt with loss of consciousness. It can be due to many causes like trauma, brain injury or any cortical brain disease. Medication usually can very well control it in 60 to 70 percent of patients, well you need to continue medications longterm, may be few years or sometime lifelong and it depends on type of epilepsy.

Lithium toxicity caused tremors, get your serum lithium and creatinine checked. May be you need to stop or reduce dosage of lithium if its the reason.

Trigeminal neuralgia is a very annoying recurrent illness. Fortunately treatment is available. The permanent solution is nerve block. You may consult your neurologist for the same.

Epilepsy - Is Surgery An Option?

Living with epilepsy can be highly challenging. This is a medical condition that can be controlled with medications in most cases, but it might not be useful for all patients due to severe side effects. It is estimated that up to 30% people who have epilepsy face this issue, and in such cases, surgery might be considered as an option.

This condition typically results in seizures which is a result of sudden electrical activity in the brain. So, certain complicated epilepsy operations on the brain can help patients with their seizures and contain their symptoms.

The goal of epilepsy surgeries involves removing part of the brain that causes these seizures and disrupting parts of the nervous system in the brain that contribute to the same. The procedure also involves the installation of a device that helps control the side effects of epilepsy.

When is surgery not an option for epilepsy?
Doctors would accurately identify the part of the brain that is causing seizures as the first step. These parts are known in medical terms as the ‘seizure’ focus. This area should not be related critical functions of the brain like movement, sensation, and language involved – else surgery would not be possible.

Surgery is not viable when seizures are severe and impair bodily movement or if the patient has serious medical conditions (like cancer or heart diseases).

Common Surgery Options Available

  1. Lesionectomy: This is one of the most commonly used procedures. This form of surgery involves the surgeon removing brain lesions from the brain or areas of injury due to malformed blood vessels or tumors. These injuries can often cause seizures and epilepsy and can be treated effectively with lesionectomy.
  2. Lobe resection: This is another form of treatment which is possible only if the epilepsy is located in one of the four parts of the brain – the temporal lobe. It is quite common in younger people and can easily be treated by expert neurosurgeons by removing the brain tissues that are causing epilepsy.
  3. Multiple Subpial Transections: While the above methods involve cutting out brain tissues, sometimes such surgeries just might not be possible due to other complications. If the parts of the brain cannot be cut out, surgeons tend to adopt Multiple Subpial Transections. This procedure can help control seizures without requiring cutting out parts of the brain. Small intersections are made in the brain to disrupt the impulses from affected brain cells that cause seizures, and they do not affect normal brain activity. It helps leave all abilities intact in patients and aids in treatment very effectively.

Few things related to headache so that we can plan the treatment accordingly - if pain is associated with blurred or decreased vision than check your eye sight for refractive error - if the pain is pulsatile in nature and with a throbbing pain usually with an aura than we have to rule out migrane as the cause - if the pain is associated with neck problems than we have to rule out cervical spondylosis as the cause - if the pain is associated with either low or high bp or due to hyperacidity than we have to treat the cause after examination - if the pain is associated with cold and fever than we have to rule out sinusitis as the cause.

It is actually a two edged sword" rumination of negative thoughts in mind" leading to" insomnia" and vice versa. This is usually a part if anxiety disorders caused due to unmanaged stress and other problems. You should consult a psychiatrist with exact and complete details of your problems for some goal oriented counselling, behaviour therapies and medications if required for insomnia.
